Carney triad: a syndrome featuring paraganglionic, adrenocortical, and possibly other endocrine tumors.

BACKGROUND: Two young women, each with paraganglioma and gastric stromal tumor, were encountered in the middle 1970s. One also had an adrenal cortical adenoma and the other pulmonary chondroma. OBJECTIVE: To test the hypothesis that the combination of tumors might represent a rare syndrome, similar cases were searched for. Five additional patients with gastric stromal tumor, paraganglioma, and pulmonary chondroma were found, and all were young women. None had a family history of the tumors. The combination of the three tumors was later referred to as the Carney triad. FEATURES OF THE SYNDROME

Macroscopic venous tumour thrombus in paediatric adrenocortical tumours.

INTRODUCTION: Macroscopic venous tumour thrombus is recognized in adult adrenocortical tumours (ACT), but its incidence, surgical implications, and prognostic impact in children remain poorly defined. MATERIALS AND METHODS: We retrospectively analysed prospectively collected registry data from 104 children and adolescents with adrenocortical carcinoma or tumours of uncertain malignant potential treated between 1997 and 2025. Patients were classified by vascular status: no invasion, microscopic invasion, macroscopic venous tumour thrombus, or unresectable disease. The study was non-randomized;

Circular RNA circ-CCAC1 Facilitates Adrenocortical Carcinoma Cell Proliferation, Migration, and Invasion through Regulating the miR-514a-5p/C22orf46 Axis.

Adrenocortical carcinoma (ACC) is a rare but clinically aggressive endocrine malignancy. Circular RNAs (circRNAs) were found to play key roles in tumorigenesis. In the current study, we aimed to investigate the functions and mechanisms of a novel circRNA, circ-CCAC1, in ACC cells. circ-CCAC1 expression levels in ACC tissue specimens and cell lines were evaluated by RT-qPCR. Kaplan-Meier analysis was applied to explore the relationship between circ-CCAC1 and patients' prognosis. Cell counting kit-8 (CCK-8), colony formation, acridine orange/ethidium bromide (AO/EB) double fluorescence staining,

[Pseudovasculitis--confusingly alike systemic vasculitis...but completely different treatment is required].

"Vasculitis mimicry" or "pseudovasculitis" are terms used for the description of a variety of disorders that simulate systemic vasculitis and often represent a challenge in daily rheumatological practice. These conditions may present with clinical, radiological, and laboratory features resembling those of systemic vasculitis. Nevertheless, the pathogenesis, therapeutic approach, and prognosis are different. The pseudovasculitides constitute an extremely heterogenous group of disorders with conditions caused by infections, malignancies, drugs and thromboembolism. A detailed history is mandatory

[Treatment of metastatic adrenal carcinoma with suramin].

A right adrenocortical carcinoma (weighing 978 g) was removed from a 45-year-old man in April 1986, the tumour bed then being irradiated with 40 Gy. Subsequently discovered multiple lung metastases were treated with cisplatin, etoposide and bleomycin, without improvement. Treatment with mitotane (Lysodren) was also without effect and had to be discontinued because of severe side effects. Treatment with suramin (Germanin) was begun in August 1987. After a loading dose of 10.7 g for six weeks the lung metastases regressed almost completely. But lung metastases were again demonstrated in January

A Phase II Trial of Cytoreduction and Hyperthermic Intraperitoneal Chemotherapy for Recurrent Adrenocortical Carcinoma.

BACKGROUND: Recurrent adrenocortical carcinoma (ACC) is an aggressive disease with few options offering durable survival benefit. Despite metastasectomy, recurrence is common. Cytoreduction and intraperitoneal chemotherapy have offered improved survival in other advanced cancers. We sought to evaluate the use of cytoreduction and hyperthermic intraperitoneal chemotherapy (HIPEC) for the treatment of recurrent intraperitoneal ACC. METHODS: A phase II, single institution clinical trial was approved for patients with radiographic evidence of resectable ACC limited to the peritoneum. Patients und

Myxoid and Sarcomatoid Variants of Adrenocortical Carcinoma: Analysis of Rare Variants in Single Tertiary Care Center.

The aim of this study is to describe rare variants of adrenocortical carcinoma (ACC) and to compare the prognosis with that of conventional ACC. We retrospectively reviewed 8 cases of myxoid variant, 1 sarcomatoid variant, and 14 cases of conventional ACC, who underwent surgical resection at the Asan Medical Center between 1996 and 2014. An analysis of the clinicopathological characteristics, including the Weiss score, Ki-67 labeling index, and reticulin framework assessment is presented. The mean age of patients with myxoid/sarcomatoid ACC was 45 years; 4 out of 9 patients were women. Mean pr

The crosstalk between aldosterone and calcium metabolism in primary aldosteronism: A possible calcium metabolism-associated aberrant "neoplastic" steroidogenesis in adrenals.

Intracellular calcium (Ca) levels play pivotal roles in aldosterone biosynthesis. Several somatic mutations of ion channels associated with aldosterone over-production were reported to result in over-inflow of Ca ion. Recently, the main regulators of extracellular Ca including VDR, CaSR and PTH1R were also reported to regulate steroidogenesis including aldosterone production. Therefore, not only intracellular but also extracellular Ca levels could regulate aldosterone biosynthesis. In addition, primary aldosteronism (PA) is clinically associated with not only more frequent cardiovascular event

Changes in the clinical manifestations of primary aldosteronism.

BACKGROUND/AIMS: Primary aldosteronism (PA) is now widely recognized to have a higher prevalence than was once thought. In view of its increasing prevalence, we compared chronological changes in clinical manifestations of PA according to different times of diagnosis. METHODS: In total, 85 patients diagnosed with PA from January 1986 through March 2012 were reviewed retrospectively, based on their medical records. During two periods-1986 to 2005 and 2006 to 2012-41 and 44 patients, respectively, were diagnosed with PA. We compared the clinical and biological characteristics of PA between these

Elevated expression of luteinizing hormone receptor in aldosterone-producing adenomas.

CONTEXT: The mechanisms driving steroid production in aldosterone-producing adenomas (APAs) are poorly defined. However, previous studies have shown that steroid production in some cortisol-producing adenomas is regulated by aberrant expression of G protein-coupled receptors. Aberrant adrenal expression of LH receptors has been shown to cause Cushing's syndrome, but the role of LH receptors in Conn's disease (hyperaldosteronism) has not been studied. OBJECTIVE: The objective of the study was to determine whether APAs express elevated LH receptor, compared with normal adrenal (NA). DESIGN: Po

A large family with Carney complex caused by the S147G PRKAR1A mutation shows a unique spectrum of disease including adrenocortical cancer.

CONTEXT: Most tumors in Carney complex (CNC) are benign, including primary pigmented nodular adrenocortical disease (PPNAD), the main endocrine tumor in CNC. Adrenocortical cancer (AC) has never been observed in the syndrome. Herein, we describe a large Azorean family with CNC caused by a point mutation in the PRKAR1A gene coding for type 1-α (RIα) regulatory subunit of the cAMP-dependent protein kinase A, in which the index patient presented with AC. OBJECTIVE: We studied the genotype-phenotype correlation in CNC. DESIGN AND SETTING: We reported on case series and in vitro testing of the PR

Surgery for Li Fraumeni syndrome: pushing the limits of surgical oncology.

OBJECTIVES: Li Fraumeni syndrome is an autosomal dominant cancer syndrome due to a germline mutation in the p53 tumor suppressor gene. It results in multiple primary neoplasms in children and adults. A common question when faced with a Li Fraumeni patient who develops multiple primary cancers and/or recurrences is what is the proper treatment? Data suggests that ionizing radiation exposure increases the incidence of second malignancies in the Li Fraumeni population. Therefore, how much surgery can a cancer patient tolerate and still derive benefit from it? METHODS: We describe a representativ

Delineating Pediatric Adrenocortical Tumors by GC-MS Urinary Steroid Metabolome Analysis: Observations From the MET Study.

INTRODUCTION: Adrenocortical tumors (ACTs) comprise adrenocortical adenomas (ACAs) and adrenocortical carcinomas (ACCs); the latter are highly aggressive. Pediatric adrenocortical tumors (pACTs) are functional and thus symptomatic. We investigated whether gas chromatography-mass spectrometry (GC-MS) urinary steroid metabolome analysis can detect tumors and differentiate between adenomas and carcinomas. METHOD: We investigated 46 patients with pACTs (age: median 6.9; range 0.7-17 years; 36 females; 21 ACAs, 25 ACCs) from the German Society for Pediatric Oncology and Hematology for Malignant End

Early withdrawal of systemic corticosteroid following malignant glioma craniotomy: a real-world population-based cohort study.

BACKGROUND: Dexamethasone is commonly administered perioperatively in malignant glioma craniotomies to mitigate peritumoral edema and associated mass effects. However, prolonged steroid exposure impairs survival outcomes through immunosuppression. The optimal timing for postoperative steroid withdrawal remains unclear. This study investigates the association between early postoperative corticosteroid withdrawal and outcomes following malignant glioma craniotomy. METHODS: A retrospective cohort analysis was conducted using the TriNetX database. Propensity score matching (PSM) was performed to f
