Characterization and pathological characteristics of spontaneous osteonecrosis of the knee.

OBJECTIVE: Spontaneous osteonecrosis of the knee affects patients typically over the age of fifty-five years. Evidence exists that this process may not be true necrosis. The purpose of this study was to characterize the demographic, radiographic, and pathologic features of this condition. MATERIALS AND METHODS: Twenty-one patients (twenty-two knees) consecutively treated for spontaneous osteonecrosis of the knee were studied. RESULTS: Only one of twenty-two specimens demonstrated evidence of bone necrosis. No specimens showed fat necrosis, marrow necrosis, fibrous change or appositional bone

Aneurysmal bone cysts of the sacrum: a report of ten cases and review of the literature.

Ten cases of aneurysmal bone cysts are presented. Patients ranged in age from five years to 64 years. Treatment was resection or curettage; four patients had preoperative embolization. Treatment results were excellent. Six patients had no recurrence while recurrences in the other four patients were successfully treated by recurretage, two with adjunctive CyberKnife therapy. All ten patients are currently disease free for at least two years. Only two patients have residual neurologic deficit in the form of bowel or bladder dysfunction.

Bilateral patellar tendon rupture at different sites without predisposing systemic disease or steroid use.

Simultaneous bilateral patellar tendon ruptures are extremely rare, and even more rare in patients without systemic disease. We describe bilateral simultaneous patellar tendon disruptions in the absence of systemic disease or steroid usage, with one tendon disruption at the inferior pole and the other an intrasubstance tear. The different locations of the ruptures are also exceedingly rare, as only two cases of non-identical ruptures have ever been reported. We also review all bilateral patellar tendon rupture case reports from English and German literature.

Accessory anterolateral talar facet as an etiology of painful talocalcaneal impingement in the rigid flatfoot: a new diagnosis.

A retrospective review identified six patients with seven painful rigid flatfeet. In each case, pain was localized laterally to an accessory facet of the anterolateral talus. cross-sectional imaging demonstrated no evidence of tarsal coalition. In five of the six, preoperative magnetic resonance imaging (MRI) was obtained and in each case demonstrated focal abutting bone marrow edema consistent with impingement between the accessory facet and the anterior calcaneus.Seven feet in six patients underwent resection of the accessory facet with additional subtalar joint-sparing reconstructive proced

Natural history of type III growth disturbance after treatment of developmental dislocation of the hip.

BACKGROUND: Type III growth disturbance (T3GD) following reduction for developmental dysplasia of the hip (DDH) is a relatively rare, but potentially devastating complication. This study evaluated the long-term outcomes of patients treated for DDH who developed a T3GD hip compared to those who didn't, with an emphasis on possible risk factors. METHODS: A case-control design was used. All radiographs of a consecutive set of patients with DDH were evaluated. Twenty-two patients (29 hips) developed T3GD. The control group consisted of 57 patients (72 hips) without any sign of growth disturbance.

Neuromuscular disease as the cause of late clubfoot relapses: report of 4 cases.

Following correction with the Ponseti method some idiopathic clubfeet still will relapse even after six years of age. A better understanding of the cause for these late relapses will greatly help in the management of this condition. We evaluated a consecutive case-series from 1948 through December 1984 including 209 patients (321 clubfeet). Patients were treated following the Ponseti method. Initial number of casts, age at relapse, neurological evaluation, and final treatment for the late-relapses were recorded. There were 12 patients (6%) having a relapse after the seventh birthday. In 4 of t

Habitual hip joint activity level of the penned EMU (Dromaius novaehollandie).

Orthopaedic management of femoral head osteonecrosis remains problematic, partly because of inability to systematically compare treatments in an animal model whose natural history parallels the human in terms of progression to femoral head collapse. Recently, it was determined that collapse could be consistently achieved for cryogenically induced osteonecrosis in the emu. Toward delineating the comparative hip joint biomechanics of emus versus humans, for purposes of establishing the emu as a model for human femoral head osteonecrosis, habitual hip joint activity level was quantified for a gro

Subperiosteal chondromyxoid fibroma: a report of two cases.

Chondromyxoid fibroma is a rare cartilage tumor that represents less than 1% of all bone tumors. When in a long bone, it is usually an intramedullary lesion that is eccentrically located in the metaphyseal region. Chondromyxoid fibroma may also have unusual presentations. These include intracortical lesions and subperiosteal lesions. There have been 14 reported cases of intracortical chondromyxoid fibroma, but there have been only four reports of subperiosteal lesions. A subperiosteal location, therefore, is extremely rare for a chondromyxoid fibroma. We present two new cases of subperiosteal

Intraosseous neurilemmoma involving the distal tibia and fibula: a case report.

Schwannoma, or neurilemmoma, is a benign nerve sheath tumor most commonly located in the soft tissue. Occasionally, Schwannomas involve osseous structures. Involvement of two adjacent long bones was not found in a review of the English language medical literature. We present the case of a neurilemmoma affecting both the distal fibula and tibia. Although rare, intraosseous neurilemmoma should be included on the differential diagnosis of painful, radiographically benign-appearing lesions arising in long bones.

Bone plating in patients with type III osteogenesis imperfecta: results and complications.

The results of bone plating in four children (6 femurs, 2 tibias) with osteogenesis imperfecta type III were analyzed. Average age at time of operation was 44 months. In three of the femurs, multiple platings were performed for a total of 13 bone platings in the eight bones studied. Average time to revision following plating was 27 months. Indications for revision included fracture (6), deformity (3), hardware failure (3), and nonunion (1). Other complications included one case of compartment syndrome. All eight bones were ultimately revised to elongating intramedullary Bailey-Dubow rods. Bone

Dislocation of the posterior tibial tendon: a literature review and presentation of two cases.

Dislocation of the posterior tibial tendon has rarely been reported in the English literature. The most common mechanism is a traumatic injury. We present two patients with a traumatic dislocation. One patient was delayed in presentation to the treating physician by seven months. The second patient presented within one week. Both underwent surgical stabilization with repair of the torn retinaculum and deepening of the groove posterior to the medial malleolus. They have both returned to their pre-injury level of activity without any recurrence of dislocation.

Comparison of fracture versus radiolucent table in the treatment of slipped capital femoral epiphysis.

PURPOSE: Compare slipped capital femoral epiphysis stabilized on fracture versus radiolucent table. METHODS: Twenty unilateral stable mild slipped capital femoral epiphyses were stabilized in-situ with a single screw. Age, sex, side, body-mass index, type of table, anesthesia time, surgery time, fluoroscopy time, number of fluoroscopy images, preoperative/postoperative lateral head-shaft angle, and number of screw threads engaging the epiphysis were noted. RESULTS: There were no differences in the measured parameters. CONCLUSIONS: Stabilization of mild slipped capital femoral epiphysis can

Limitations of isokinetic testing to determine shoulder strength after rotator cuff repair.

Some investigators have reported incomplete data when using isokinetic testing as a means of analyzing shoulder strength after rotator cuff repair. An explanation provided has been that the subjects could not reach the speed at which the machine was set. The purpose of this study was to determine if strength data could be generated for all motions being tested by using not only the one or two speeds employed by others, but three speeds across the spectrum of those available. Inclusion criteria were a minimum of two years since surgery with a normal contralateral shoulder. All eligible subjects

The clinical performance of metal-on-metal as an articulation surface in total hip replacement.

The metal-on-metal articulations in total hip arthroplasty (THA) were widely used between 1960 and 1975. The McKee-Farrar and other first-generation prostheses failed at a high rate because impingement caused early component loosening. The problem of early component loosening was corrected by improved component design and better manufacturing quality. Second-generation metal-on-metal total hip replacements have experienced short and medium-term success as assessed by Harris Hip Scores and patient self-assessment. The combined annual linear wear of the metal-on-metal femoral head and acetabular

Gluteus medius tendon rupture as a source for back, buttock and leg pain: case report.

A 67-year-old woman with chronic lumbosacral and hip symptoms involving gluteus medius tendon rupture and strain injury is presented here. We report her work-up and management. Although this is an uncommonly reported pathology, many patients with back, buttock and leg pain see physicians who often focus on lumbar spinal stenosis, lumbar radiculopathy or hip/knee osteoarthritis. Careful physical examination guided us to this patient's diagnosis.
