The effect of wrist position sense and tactile recognition on manual skills in patients with upper extremity neuropathy.

BACKGROUND: Peripheral neuropathy affects fine motor skills in daily life. However, reports on the effects of position sense and tactile recognition on manual dexterity are quite scarce in the literature. The increasing focus on hand rehabilitation has created a need to examine the effects of sensation on manual dexterity. PURPOSE: The purpose of this study is to compare the effect of tactile recognition and wrist position sense on manual skills in healthy and neuropathy individuals and to investigate the relationship between them. STUDY DESIGN: Cross-sectional, non-randomized comparative cl

Postural motion perception during vestibular stimulation depends on the motion perception threshold in persistent postural-perceptual dizziness.

Patients with persistent postural-perceptual dizziness (PPPD) perceive postural instability larger than the observed sway. It is unknown whether the concept of postural misperception prevails during vestibular stimulation and whether it may account for the unsteadiness patients complain during body movements. We tested the hypothesis of an abnormal sensory-perceptual scaling mechanism in PPPD by recording objective, perceived, and the reproduced postural sway under various standing conditions, modulating visual and proprioceptive input, by binaural galvanic vestibular stimulation (GVS). We rel

Phantom hand and wrist movements in upper limb amputees are slow but naturally controlled movements.

After limb amputation, patients often wake up with a vivid perception of the presence of the missing limb, called "phantom limb". Phantom limbs have mostly been studied with respect to pain sensation. But patients can experience many other phantom sensations, including voluntary movements. The goal of the present study was to quantify phantom movement kinematics and relate these to intact limb kinematics and to the time elapsed since amputation. Six upper arm and two forearm amputees with various delays since amputation (6months to 32years) performed phantom finger, hand and wrist movements at

Multimodally specified energy expenditure and action-based distance judgments.

Previous research has demonstrated that perceived self-motion can be manipulated by the relation between optic flow rate and walking rate. Other studies have revealed that verbal reports of perceived distance are influenced by the energy that would be expended to traverse the distance in question. In an effort to integrate these findings, we investigated how action-based distance judgments are influenced by multimodally specified energy expenditure (MSEE)--the metabolic cost associated with traversing an optically specified distance--using a virtual-reality treadmill environment. The energy ex

Practicing a matching movement with a mirror in individuals with spastic hemiplegia.

This experimental study aimed to determine the effect of practicing a position matching task with (mirror) visual feedback of the less-impaired arm on the matching accuracy of the impaired arm in children and adolescents with spastic hemiparetic cerebral palsy. Practice consisted of 40 trials of bimanual target matching, where one group received regular visual feedback and a second group received mirror visual feedback of the less-impaired arm. On three occasions (pre, post, and after a 1-week-retention) position sense (matching accuracy measured as the endpoint error in cm) of the impaired ar

Depth of ankle inversion and discrimination of foot positions.

Ankle inversion injuries are common, yet little is known about the error associated with different positions as inversion depth increases. In this study, absolute judgments made without feedback were used to measure discrimination of different extents of ankle inversion which arose from active movements made to physical stops by 20 self-reported right side-dominant participants. Testing was conducted over three sets of five inversion depths that were within a range of 1.4 degrees and centered around mean depths of 8,11, and 14 degrees. Discrimination of ankle inversion movements decreased line

Sagittal alignment correction of the thoracolumbar junction in idiopathic scoliosis by in situ bending technique.

A long thoracolumbar sagittal rectitude is sometimes present in adolescent idiopathic scoliosis. The purpose of this study was to identify typical patterns, by comparing frontal plane deformities and vertebral rotation leading to this rectitude. Surgical thoracolumbar alignment correction by three-dimensional in situ bending of rods was then analyzed. Pre- and postoperative radiographs of 24 patients with scoliosis (36-104 degrees) were reviewed using Spineview software. Frontal curves and levels of sagittal rectitude were determined. Thoracic kyphosis, lumbar lordosis, sacral slope, pelvic in

Stretch reflex modulation during imposed static and dynamic hip movements in standing humans.

The purpose of this study was to investigate the effects of hip proprioceptors on soleus stretch reflex excitability in standing humans. A custom-made device to stretch the ankle extensors was mounted on the lower leg portion of a gait orthosis and was used to elicit stretch reflex responses while standing. Six subjects with motor complete spinal cord injury (SCI) and six spinal intact subjects were placed in the orthosis, and stretch reflex responses were elicited when static and/or dynamic hip joint angle changes were imposed. We found that static hip extension significantly enhanced the str

Visual perception and interception of falling objects: a review of evidence for an internal model of gravity.

Prevailing views on how we time the interception of a moving object assume that the visual inputs are informationally sufficient to estimate the time-to-contact from the object's kinematics. However, there are limitations in the visual system that raise questions about the general validity of these theories. Most notably, vision is poorly sensitive to arbitrary accelerations. How then does the brain deal with the motion of objects accelerated by Earth's gravity? Here we review evidence in favor of the view that the brain makes the best estimate about target motion based on visually measured ki

3: Rehabilitation principles for treating chronic musculoskeletal injuries.

Evaluation of patients for rehabilitation after musculoskeletal injury involves identifying, grading and assessing the injury and its impact on the patient's normal activities. Management is guided by a multidisciplinary team, comprising the patient, doctor and physical therapist, with other health professionals recruited as required. Parallel interventions involving the various team members are specified in a customised management plan. The key component of the plan is active mobilisation utilising strengthening, flexibility and endurance exercise programs. Passive physical treatments (heat,

Intersegmental and local interneurons in the metathorax of the stick insect Carausius morosus that monitor middle leg position.

1. In the stick insect, proprioceptive information from the middle leg is used to define the target for the swing movement of the adjacent rear leg ("targeting behavior"). To investigate the underlying neural circuits, intracellular recordings were made in the ganglion controlling the rear leg, the metathoracic ganglion, while systematically moving the tarsus of the middle leg. 2. Several intersegmental interneurons and one local interneuron were identified as possible contributors to the targeting behavior. The intersegmental interneurons code the position of the middle leg tarsus in a highly

Prehension movements directed to approaching objects: influence of stimulus velocity on the transport and the grasp components.

In this investigation we studied the influence of object velocity on the transport and on the grasp components of prehension movements directed to approaching objects. Three experiments were carried out. In the first experiment six subjects were required to reach and grasp a sphere that approached them with a constant velocity. The effects of four velocities were studied. The results showed that the end point of the arm movement changed with object velocity: nearer the body with higher than with lower object velocities. Transport velocity increased with movement amplitude and the deceleration

The effects of space travel on the nervous system.

The translation of man from terrestrial to an extra terrestrial environment is accompanied by an upset in the servo-control of movement engendered by the removal of the normal gravitational signal. Unfortunately the "natural" response of the nervous system, to ocular and vestibular confusion, is to cause varying degrees of sickness which can only be avoided by choice of suitable space travellers i.e., those who are least upset by gravitational chaos. This will remain so until much more is learned about the fundamental physiological mechanisms whereby man maintains a correct head/trunk, head/ey

An electrophysiological investigation of propriospinal inspiratory neurons in the upper cervical cord of the cat.

This study was performed in order to describe the location, axonal projection and possible synaptic action of the inspiratory neurons recently described in the upper cervical cord. In 26 cats anaesthetized with Nembutal, extracellular recordings were made from 224 cervical inspiratory units which were found near the lateral border of lamina VII and formed a column extending from the caudal end of the nucleus retroambigualis at the C1 segment to the rostral half of the C3 segment. Most of the units (approximately 85%) could be excited antidromically from the thoracic cord. Antidromic mapping sh

Cervico-vestibular interaction in eye movements.

The cervico-vestibular interaction was investigated in normal subjects and patients with unilateral labyrinthine dysfunction using neck vibratory stimulation and caloric stimulation. There is a functional interaction between sensory inputs from the neck proprioceptors and the peripheral labyrinth in the patients with unilateral labyrinthine dysfunction. However, this interaction was not observed in normal subjects. This result indicates that the role of the cervical sensory inputs in normal subjects and the patients is different. That is, inputs from the neck proprioceptors have little effect
