Effects of ethanol ingestion on the severity and outcome of trauma.

Review of 1,198 patients with regard to outcome and the presence or absence of detectable ethanol in the blood as determined in the emergency room demonstrated no difference in the severity of injury in those who had been drinking and those who had not. Mortality was significantly lower in those who had been drinking. There were no other significant differences in the two groups. The mechanism by which this occurs is not well understood but may be related to an augmentation of the catecholamine response normally seen after injury.

Factors influencing morbidity and mortality after liver injury.

The factors influencing morbidiy and mortality after liver injury were monitored prospectively in 637 patients. Death correlated with the number of associated injuries, severity of injury, presence of great vessel injury, active bleeding from liver at laparotomy, and severe shock on admission. Morbidity correlated most with presence of a colon injury, and the use of choledochostomy drainage. The first priority in treating liver injury is to stop bleeding. The simplest technique for this is multiple liver suture. Drainage is most safely achieved with soft rubber drains placed to the site of inj

Mindfulness Is Associated with Lower Depression, Anxiety, and Post-Traumatic Stress Disorder Symptoms and Higher Quality of Life in Patients with an Implantable Cardioverter-Defibrillator-A Cross-Sectional Study.

Several studies suggested the efficacy of dispositional mindfulness and mindfulness-based interventions in reducing anxiety and depression in cardiovascular diseases. However, data on the impact of mindfulness on the psychological well-being of patients with an implantable cardioverter-defibrillator (ICD) are scarce. In this study, 422 patients with an ICD were prospectively recruited. Logistic regression was applied to determine associations between dispositional mindfulness (Freiburg Mindfulness Inventory), depression (Patient Health Questionnaire-8), anxiety (Generalized Anxiety Disorder-7

Conditioned Lick Suppression: Assessing Contextual, Cued, and Context-cue Compound Fear Responses Independently of Locomotor Activity in Mice.

Pavlovian fear conditioning is a widely used procedure to assess learning and memory processes that has also been extensively used as a model of post-traumatic stress disorder (PTSD). Freezing, the absence of movement except for respiratory-related movements, is commonly used as a measure of fear response in non-human animals. However, this measure of fear responses can be affected by a different baseline of locomotor activity between groups and/or conditions. Moreover, fear conditioning procedures are usually restricted to a single conditioned stimulus (e.g., a tone cue, the context, etc.) an

Epidemiology and predictors of traumatic spine injury in severely injured patients: implications for emergency procedures.

PURPOSE: This study aimed to identify the prevalence and predictors of spinal injuries that are suitable for immobilization. METHODS: Retrospective cohort study drawing from the multi-center database of the TraumaRegister DGU®, spinal injury patients ≥ 16 years of age who scored ≥ 3 on the Abbreviated Injury Scale (AIS) between 2009 and 2016 were enrolled. RESULTS: The mean age of the 145,833 patients enrolled was 52.7 ± 21.1 years. The hospital mortality rate was 13.9%, and the mean injury severity score (ISS) was 21.8 ± 11.8. Seventy percent of patients had no spine injury, 25.9% scored 2-

Treatment of liver injuries at the Trauma Centre of the University Hospital in Pilsen.

INTRODUCTION: Traumatic liver injury is the most frequent cause of death due to traumatic bleeding in the peritoneal cavity. The progress of pre-hospital emergency medical service, improved surgical techniques, and advanced post-operative care decreased the morbidity from 65% to 10% over the last 40 years. The management is either surgical or non-surgical. Operation is indicated in case of haemodynamic instability and failure of non-surgical treatment. The aim of the paper is to evaluate the aetiology of liver injury, the type of treatment, and therapeutic results in patients hospitalized betw

[Accidental hypothermia in multiple trauma patients].

BACKGROUND: Hypothermia, defined as a body core temperature below 35 °C, could be divided into an endogeneous, therapeutic and accidental hypothermia. At admission in the emergency room multiple trauma patients show a hypothermic core temperature in up to 66 %. A core temperature below 34 °C seems to be critical in these patients as this temperature limit has been demonstrated to be associated with an increased risk for post-traumatic complications and a decreased survival. In polytraumatised patients with a core temperature below 32 °C a mortality rate of 100 % has been described. MATERIAL A

[The prophylaxis of the thromboembolic disease in the head trauma].

BACKGROUND: The prophylaxis of the thromboembolic disease in the severe head trauma remains a controversy. AIM: In this study, we are interested to the determination of under groups of patients for whom the advantages of the prophylaxis of the thromboembolic disease (TED) are higher than its disadvantages. METHODS: We proceeded to a retrospective study based on patient medical records ranging from March 2003 until March 2004, enrolling 56 consecutive patients. The data collected related to the age, the gender, past medical history, the type of trauma, results of the initial CT scan, the trea

Video-assisted thoracoscopic surgery repair of an idiopathic internal mammary artery aneurysm.

A discussion of an internal mammary artery aneurysm is a rare finding in the literature. This condition can cause serious complications, including haemothorax and pneumo-haemothorax, and can lead to haemorrhagic shock; it can also be fatal. These effects can be explained by the rate of flow of the blood in the internal mammary artery, that is, 150 ml/minute, which leads to a blood loss of one litre in minutes. As reported in the literature, it has many different clinical presentations. Because an internal mammary artery aneurysm occurs so rarely, agreement regarding the best way to manage its

Point of Care Ultrasonography for the Septic Patient in the Emergency Department: A Literature Review.

The point-of-care ultrasound (POCUS) has been effectively used in intensive care units for the management of septic patients. Since it is a time- and cost-effective non-invasive imaging modality, its use in the emergency department (ED) has been advocated for by medical experts. This review summarizes the existing literature regarding the breadth of POCUS as a supplementary tool to the holistic approach of septic patients in the ED setting. A literature search was conducted via PubMed (MEDLINE), Cochrane Library, and Scopus databases, analyzing studies which examined the use of POCUS in the ED

Damage control surgery: old concepts and new indications.

PURPOSE OF REVIEW: While the principles of damage control surgery - rapid hemorrhage and contamination control with correction of physiologic derangements followed by delayed definitive reconstruction - have remained consistent, forms of damage control intervention have evolved and proliferated dramatically. This review aims to provide a historic perspective of the early trends of damage control surgery as well as an updated understanding of its current state and future trends. RECENT FINDINGS: Physiologically depleted patients in shock due to both traumatic and nontraumatic causes are often

First Isolation of Yarrowia lipolytica in a Granulomatous Pneumonia of a Spectacled Caiman, Caiman crocodilus Linnaeus, 1758.

Contrary to humans, candidiasis is a rare infection in animals. However, in reptiles, candidiasis can cause gastrointestinal, cutaneous, or rarely systemic infections in stressed animals. The infections due to Yarrowia lipolytica have been increasingly described in human medicine, and hundreds of cases are reported, comprised of granulomatous lung lesions. Herein, granulomatous pneumonia of a spectacled caiman, Caiman crocodilus, was described, and the presence of Y. lipolytica in the lesion was confirmed through histopathology, microbiologic cultures, and molecular methods. The cause of death
