Value of arthrography after supination trauma of the ankle.

OBJECTIVE: To investigate the merits of arthrography after supination trauma of the ankle. DESIGN AND PATIENTS: In a group of 160 consecutive patients operative exploration was performed in cases where arthrography and/or a delayed physical examination showed positive findings. In all patients arthrography was performed within 48 h after trauma. To determine interobserver agreement, all arthrograms were independently evaluated by two radiologists, both ignorant of the first assessment. RESULTS: The prevalence of an ankle ligament lesion was found to be 76%. Of the 122 patients with a rupture

Downhill skiing injuries in children.

The objective of our study was to compare injuries sustained by a large group of children to those of injured adults and a control population of skiers selected from the Sugarbush North ski area in northern Vermont. We reviewed all 3182 injuries which occurred over nine ski seasons (1972 to 1973 through 1980 to 1981) at Sugarbush North. Six hundred and ninety-six (22%) of these injuries occurred in children 16 years of age or younger. For all injuries combined, those under 11 years of age had the same rate of injury as adults. Adolescents had a higher injury rate. Foot and ankle injuries were

SURFING INJURIES.

Skurfboarding is currently enjoying a phase of popularity, but it is not without hazards. Among 75 consecutive cases of skurfboard injuries seen in an emergency department, 29 fractures were encountered, 16 of which were in the region of the ankle and foot. The majority of injuries, however, consisted of sprains, contusions and abrasions. Six patients required hospital admission, three for operative management of fractures and three because of head injuries. The concrete playground, the instability of the board, the lack of protective clothing and the exhibitionist instincts of the teen-age ent

The role of extrinsic ligaments in maintaining carpal stability - A prospective statistical analysis of 85 arthroscopic cases.

Several biomechanical studies have shown that the scapholunate (SL) and lunotriquetral (LT) ligaments are not the only stabilizers of the proximal carpal row. However, no study has yet analyzed the range of ligament lesions leading to instability in vivo. Arthroscopy has been used to assess the condition of the wrist's extrinsic ligaments by palpating and tensioning the various ligament and capsule structures. In this prospective study, this arthroscopic method was used in 85 cases of wrist sprain without static instability to evaluate the correlation between lesions of the intrinsic and extri

Rehabilitation for Chronic Ankle Instability With or Without Destabilization Devices: A Randomized Controlled Trial.

CONTEXT: Individuals with chronic ankle instability (CAI) have deficits in neuromuscular control and altered movement patterns. Ankle-destabilization devices have been shown to increase lower extremity muscle activity during functional tasks and may be useful tools for improving common deficits and self-reported function. OBJECTIVE: To determine whether a 4-week rehabilitation program that includes destabilization devices has greater effects on self-reported function, range of motion (ROM), strength, and balance than rehabilitation without devices in patients with CAI. DESIGN: Randomized con

Cohesive taping and short-leg casting in acute low-type ankle sprains in physically active patients.

BACKGROUND: Cohesive taping is commonly used for the prevention or treatment of ankle sprain injuries. Short-leg cast immobilization or splinting is another treatment option in such cases. To determine the clinical efficacy and antiedema effects of cohesive taping and short-leg cast immobilization in acute low-type ankle sprains of physically active patients, we performed a preliminary clinical study to assess objective evidence for edema and functional patient American Orthopaedic Foot and Ankle Society (AOFAS) scores with these alternative treatments. METHODS: Fifty-nine physically active p

Field-expedient screening and injury risk algorithm categories as predictors of noncontact lower extremity injury.

In athletics, efficient screening tools are sought to curb the rising number of noncontact injuries and associated health care costs. The authors hypothesized that an injury prediction algorithm that incorporates movement screening performance, demographic information, and injury history can accurately categorize risk of noncontact lower extremity (LE) injury. One hundred eighty-three collegiate athletes were screened during the preseason. The test scores and demographic information were entered into an injury prediction algorithm that weighted the evidence-based risk factors. Athletes were th

Value of ultrasonography for detecting ligament damage in athletes with chronic ankle instability compared to computed arthrotomography.

Ankle sprains may be followed by chronic pain and/or instability, which may induce substantial disability, most notably in athletes. Chronic ankle instability promotes the development of cartilage lesions in athletes. Therefore, accurate evaluation of the ankle ligaments is crucial to the optimal management of chronic ankle instability after a sprain. The objective of this study was to assess the performance of ultrasonography in assessing damage to the anterior talofibular ligament (ATFL) in athletes with chronic ankle instability after a sprain. Consecutive patients seen at the author's clin

Clinical results and functional evaluation of the Chrisman-Snook procedure for lateral ankle instability in athletes.

There is no consensus regarding optimum surgical treatment for chronic ankle instability. The purpose of this study is to describe a variation of the Chrisman-Snook lateral ligament reconstruction that the senior author uses in patients with chronic ankle instability recalcitrant to conservative management. All patients who underwent reconstruction from 1997 to 2006 were identified, and those with a minimum 2-year follow-up were included, representing a total of 44 ankles in 43 patients. All underwent clinical evaluation and completion of the Foot and Ankle Outcome Survey and the Kaikkonen Sca

Back pain in athletes.

The athlete with back pain presents a clinical challenge. Self-limited symptoms must be distinguished from persistent or recurrent symptoms associated with identifiable pathology. Athletes involved in impact sports appear to have risk factors for specific spinal pathologies that correlate with the loading and repetition demands of specific activities. For example, elite athletes who participate in longer and more intense training have higher incidence rates of degenerative disk disease and spondylolysis than athletes who do not. However, data suggest that the recreational athlete may be protec

A clinical and pharmacologic review of skeletal muscle relaxants for musculoskeletal conditions.

Muscle strains and other musculoskeletal disorders (MSDs) are a leading cause of work absenteeism. Muscle pain, spasm, swelling, and inflammation are symptomatic of strains. The precise relationship between musculoskeletal pain and spasm is not well understood. The dictum that pain induces spasm, which causes more pain, is not substantiated by critical analysis. The painful muscle may not show EMG activity, and when there is, the timing and intensity often do not correlate with the pain. Clinical and physiologic studies show that pain tends to inhibit rather than facilitate reflex contractile
