A reconstruction of the anterior cruciate ligament of the knee has been devised on the anatomical principle of an intra-articular flap taken from the middle third of the patellar tendon and based on the sub-patellar fat pad. Fifty knees have been treated and assessed after a follow-up of two to four years to provide a critical analysis of the method. The technique of the operation is described in detail in relation to the isometric reconstruction of the course of the anterior cruciate ligament. With a mean follow-up of 33 months (minimum 24 and maximum 48 months), 98 per cent of the patients h

The results of treatment in 77 clubfeet were assessed after an average follow-up of 8 years. In those patients with surgical treatment it was found that the Turco posteromedial release within the first 3 months of life resulted in 13 of 16 satisfactory feet. In comparison, only five of 21 feet had satisfactory results when the average age of operation was delayed to 3.7 years and a less extensive release performed. At follow-up, abnormal radiological findings were not found in the early operated cases. The average talocalcaneal index was 47 degrees. The late operated feet showed major abnormal

In this article, an overview is provided of pediatric spinal cord injury, organized by effects of this injury on various organ systems. Specific management differences between children and adults with spinal cord injury are highlighted. A detailed management approach is offered for particularly complex topics, such as spasticity and upper extremity reconstruction.

PURPOSE: The reconstruction of total lower lip defects is very challenging, whether from trauma or after oncologic treatment. The challenge faced by surgeons is to re-create not only an esthetically pleasing lower lip but also, more importantly, a lip that is functional.
MATERIALS AND METHODS: The authors reviewed their experience at the Division of Oral and Maxillofacial Surgery, University of Florida College of Medicine, Jacksonville in reconstructing these challenging defects over a 3 year period. The timeline extended from 2007 through 2010. All surgeries were performed by the senior auth

INTRODUCTION: Vascularized toe proximal interphalangeal (PIP) joint transfer restores motion after destructive finger injuries, yet outcomes are limited by a persistent extensor lag. We observed subtendinous connective tissue attachment beneath the extensor digitorum longus (EDL), from the proximal phalangeal neck to the PIP joint, that may tether the tendon to bone. We assessed whether releasing this tissue improves surgical outcomes without compromising perfusion.
METHODS: We performed a retrospective matched cohort study with a 1:1 propensity score matching on sex, age at surgery, native t

This retrospective non-randomized study aimed to compare the clinical and radiological outcomes of oblique versus transverse tunnels in ligament reconstruction and tendon interposition arthroplasty. This study included 15 patients with oblique tunnels (Group 1) and 13 patients with transverse tunnels (Group 2), all of whom had been diagnosed with Eaton stage 3 or 4 carpometacarpal arthritis and had undergone trapeziectomy with the ligament reconstruction and tendon interposition surgery. The visual analogue scale was used to assess pain levels, while the Quick Disabilities of the Arm, Shoulder

Young’s procedure contains an action mechanism that works better than other techniques on the pathophysiology of FFD. It respects the anatomy and biomechanics of the foot to reach the necessary muscular balance. The benefits of this technique include that the ATT is not detached, so its function mechanism is still active; the new trajectory of the ATT provides a powerful sling function at the level of the navicular; and the horizontal trajectory of the ATT and the osteoperiosteal flaps constitute a powerful inner capsular–tendinous–ligamentous support. What is more, an insufficiency of the ATT

Long-standing peripheral monolateral facial paralysis in the adult has challenged otolaryngologists, neurologists and plastic surgeons for centuries. Notwithstanding, the ultimate goal of normality of the paralyzed hemi-face with symmetry at rest, and the achievement of a spontaneous symmetrical smile with corneal protection, has not been fully reached. At the beginning of the 20(th) century, the main options were neural reconstructions including accessory to facial nerve transfer and hypoglossal to facial nerve crossover. In the first half of the 20(th) century, various techniques for static

Avian reovirus (ARV) infection causes viral arthritis, growth retardation, and immunosuppression in chickens. Current immunoprophylaxis, which relies mainly on vaccination, offers limited protection. This study evaluated the efficacy of transfer factor (TF) against ARV infection. One-day-old specific-pathogen-free (SPF) chicks were randomly divided into 4 groups (n = 35 each): a prophylaxis group (TF administered at 3 to 5 days of age), a treatment group (TF administered at 9 to 11 days of age), an infected group, and a blank control group. All chicks except the blank controls were infected wi

We compared the construct stability of traditional knotted techniques with modern knotless anchor systems used in arthroscopically assisted latissimus dorsi transfers (aLDTs) for irreparable posterosuperior rotator cuff tears. Eighteen cadaveric shoulders were age- and gender-matched to the following three groups: knotted group (two knotted anchors); knotless group (two knotless anchors); and triple anchor group (two knotless anchors; one all-suture anchor at the apex of the humeral head). All tendons were cyclically loaded in line of the aLDT over 400 cycles followed by a load to failure test

BACKGROUND: The Bridle procedure restores active ankle dorsiflexion through a tri-tendon anastomosis of the tibialis posterior, transferred to the dorsum of the foot, with the peroneus longus and tibialis anterior tendon. Inter-segmental foot motion after the Bridle procedure has not been measured. The purpose of this study is to report kinetic and kinematic variables during walking and heel rise in patients after the Bridle procedure.
METHODS: 18 Bridle and 10 control participants were studied. Walking and heel rise kinetic and kinematic variables were collected and compared using an ANOVA.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is a well-established treatment for cuff tear arthropathy and consistently restores active forward elevation. However, when there is a deficiency of the posterior rotator cuff (infraspinatus and teres minor), patients can exhibit lag signs at the side and a "Hornblower's sign." This loss of active external rotation (ER) in abduction can be functionally disabling, and rTSA alone may not reliably restore active ER ability in this clinical scenario. We present a series of rTSA with lower trapezius transfer (LTT) (utilizing tibialis anterior a

BACKGROUND: Cavovarus foot deformity is common in patients with Charcot-Marie-Tooth disease. Multiple surgical reconstructive procedures have been described, but few authors have reported long-term results. The purpose of this study was to evaluate the long-term results of an algorithmic approach to reconstruction for the treatment of a cavovarus foot in these patients.
METHODS: We evaluated twenty-five consecutive patients with Charcot-Marie-Tooth disease and cavovarus foot deformity (forty-one feet) who had undergone, between 1970 and 1994, a reconstruction consisting of dorsiflexion osteot

For the treatment of irreparable posterosuperior rotator cuff tears of the supraspinatus and infraspinatus, superior capsular reconstruction (SCR) has continued to supplant other less successful and/or reproducible techniques, including partial rotator cuff repair and latissimus tendon transfer. After its initial description utilizing a folded fascia lata autograft with a thickness of up to 8 mm, many in North America adapted their surgical technique to use commercially available human dermal allograft to mitigate donor site morbidity. Early series have revealed the importance of graft thickne…
