Health care issues affecting transgender people living with HIV: an opportunity for improvement.

The high vulnerability of transgender (TG) persons to HIV infection and the difficulties associated with access to health services can lead to delays in the diagnosis and treatment of HIV infection, thus increasing the risk of transmission of HIV by this population. We performed a retrospective study to analyze the main characteristics of TG living with HIV infection in a hospital in Madrid, Spain and to identify issues related to lack of access to the health care system and combination antiretroviral therapy (cART). We analyzed 28 TG, of whom 22 (78.6%) were TG women. Median age was 28 years

Examining risk and protective factors for alcohol use in lesbian, gay, bisexual, and transgender youth: a longitudinal multilevel analysis.

OBJECTIVE: Lesbian, gay, bisexual, and transgender (LGBT) youth are at increased risk for alcohol misuse, but little is known about the psychosocial and demographic factors that are associated with these differences over time. The purpose of this study was to investigate change in alcohol use across development. We aimed to describe group/demographic differences in alcohol use, the effects of psychosocial variables on drinking within persons (i.e., psychological distress, sexual orientation-based victimization, and perceived family support), and the interactions between demographic differences

"I'm not alone here": A qualitative study of social media as a source of support for transgender and gender diverse youth of color.

BACKGROUND: Social media has come under scrutiny, leading to recommendations that adolescents reduce or eliminate use. However, social media may offer benefits, particularly to marginalized groups; understanding ways in which this social context can be positive and supportive is expected to lead to more targeted recommendations for use. METHODS: This qualitative study investigates ways in which social media is a source of support for transgender and gender diverse (TGD) youth of color. Participants were recruited through youth serving organizations, community events, a Youth Advisory Board, p

A Matter of Conscience: Examining the Law and Policy of Conscientious Objection in Health Care.

Conscientious objection refers to refusal by a health care provider (HCP) to provide certain treatments, including the standard of care, to a patient based upon the provider's personal, ethical, or religious beliefs. Federal and state rules regarding conscientious objection have expanded the scope of legal protections that HCPs and institutions can invoke in support of refusal. Opponents of these rules argue that allowing refusal of care deprives patients of care that conforms to professionally established guidelines, contradicts long-standing principles related to informed consent, interferes

Scaffolded interviewing with lesbian, gay, bisexual, transgender, queer, and questioning youth: a developmental approach to HIV education and prevention.

The field of education has long recognized that adolescent development and learning are made possible by the structural supports or "scaffolds" that adults create with young people. Although the work of Lev Vygotsky (1978, 1987) has inspired developmentally-supportive approaches to education in the United States and internationally, his work has been largely overlooked in the field of HIV education and prevention. This article introduces an approach to scaffolded interviewing that builds narrative and relational "platforms" for young people's self-development and facilitates health communicati

"Being dealt with as a whole person." Care seeking and adherence: the benefits of culturally competent care.

The purpose of this study is to characterize the relationship between identity and health care experiences (including antiretroviral therapy utilization) among HIV-positive sexual minority males. This qualitative study used grounded theory with data collection occurring through focus groups and interviews. A questionnaire was used to complete a demographic profile. The study included 47 HIV positive participants from three minorities: gay men, bisexual men and transgendered persons, gender identifying as female and or living as women. Sessions elicited information on: (1) general experiences w

Long-Acting PrEP for People With High Vulnerability to HIV Acquisition in Brazil: A Cost-Effectiveness Analysis.

INTRODUCTION: In Brazil, men who have sex with men (MSM) and transgender women (TGW) remain heavily affected by HIV. Long-acting pre-exposure prophylaxis (LA PrEP) with injectable cabotegravir (CAB-LA) or lenacapavir (LEN-LA) is more effective at preventing HIV acquisition than oral PrEP. Our objective was to assess the potential clinical and economic impact of offering CAB-LA or LEN-LA to MSM and TGW with high vulnerability to HIV acquisition in Brazil and determine the maximum cost at which they would be cost-effective. METHODS: We used the CEPAC microsimulation model of HIV prevention and

The Lifecycle of Electronic Health Record Data in HIV-Related Big Data Studies: Qualitative Study of Bias Instances and Potential Opportunities for Minimization.

BACKGROUND: Electronic health record (EHR) data are widely used in public health research, including in HIV-related studies, but are limited by potential bias due to incomplete and inaccurate information, lack of generalizability, and lack of representativeness. OBJECTIVE: This study explores how workflow processes among HIV health care providers (HCPs), data scientists, and state health department professionals may potentially introduce or minimize bias within EHR data. METHODS: One focus group with 3 health department professionals working in HIV surveillance and 16 in-depth interviews (ie

Determinants of knowledge and positive perception of HIV pre-exposure prophylaxis: the role of age and sexual orientation in patients of specialized STI and transgender health services.

Pre-exposure prophylaxis (PrEP) is one of strategies for preventing human immunodeficiency virus infection. However, limited knowledge and perception of PrEP may impact the access to this prophylaxis. This study aimed to assess knowledge and perceptions of PrEP and to identify determinants, both sociodemographic and structural, associated with them among users of specialized public health services. This is a cross-sectional study based on data obtained through interviews conducted with patients who sought specialized services in STI on a spontaneous basis, as well as with routine patients at a
