Vestibular evoked myogenic potentials and distortion-product otoacoustic emissions combined with glycerol testing in endolymphatic hydrops: their value in early diagnosis.

The present investigation was specifically designed to evaluate the ability of the glycerol test combined with pure tone audiometry, distortion-product otoacoustic emissions (DPOAEs), and vestibular evoked myogenic potentials (VEMPs) to diagnose endolymphatic hydrops early and to identify cases that may evolve toward Meniere's disease. This investigation consisted of 29 consecutive patients with mild dizziness or vertigo who received no treatment. Each patient underwent glycerol testing measured with conventional pure tone audiometry and with both DPOAEs and VEMPs. It is interesting to note th

Dizziness from whiplash and head injury: differences between whiplash and head injury.

OBJECTIVE: Large discrepancies exist in the literature regarding incidence and types of symptomatology in whiplash. This is because of the evolution of whiplash injury over the years with the advent of head rests and seat belts. Previous authors have regarded symptoms of dizziness as a result of brainstem or cerebellar injury or both. It has been difficult in those studies to ascribe a mechanism of injury, as patients with whiplash injury only have been grouped with those who have incurred mild traumatic brain injury as a result of a significant blow to the head. The authors saw the need to de

Visually induced eye movements in Wallenberg's syndrome.

Eighteen patients with Wallenberg's syndrome were investigated concerning visually induced eye movements. All results were analysed quantitatively using a computer. In 16 out of 18 patients, OKN slow-phase velocities were impaired, in the remaining 2 patients they were normal. Patients who showed impaired OKN were classified into two groups according to directional asymmetry of OKN impairment. In 7 out of the 16 patients, slow-phases toward the lesion side were predominantly impaired in contrast to the slow-phases toward the intact side (group A). In the remaining 9 patients, slow-phases towar

[Neurootological findings in multiple sclerosis (author's transl)].

After clinical examination of 104 patients with multiple sclerosis sensory-motor dysfunctions of the extremities turned out to be the predominant symptoms. Hypacusis, tinnitus and dizziness were mentioned by less than 15% of the patients. The investigation of the vestibular system showed often an enhanced caloric reaction and dysfunctions of the optokinetic mystagmus as well as the eye tracking test. Moreover the threshold for temperature and gustatory sensations was often enhanced. Hypacusis was found only in a few patients, in less than 30% the SISI and the tone decay test indicated neuronal

Clinical Profiles of DFNA11 at Diverse Stages of Development and Aging in a Large Family Identified by Linkage Analysis.

HYPOTHESIS: The phenotype of DFNA11 consists of specific features at diverse developmental and age stages. BACKGROUND: Only eight mutations have been identified for autosomal dominant non-syndromic hearing loss related to MYO7A (DFNA11), and the onset and progression of DFNA11 are poorly understood. METHODS: After linkage analysis and following Sanger sequencing in a family suspected to have autosomal dominant hereditary hearing loss, we analyzed the audiometric and vestibular functions and their long-term changes in the subjects carrying the variant. RESULTS: A reported variant of uncertai

Effects of olivocochlear bundle section on otoacoustic emissions in humans: efferent effects in comparison with control subjects.

The effects of contralateral acoustic stimulation on evoked otoacoustic emissions (OAE) were examined in three subject groups in order that the impact of efferent olivocochlear bundle section (as a consequence of vestibular neurectomy) could be compared with normal findings, and with a control surgical population. Results demonstrated that the inhibitory effect of contralateral noise on OAE amplitude was absent from the cochlea with severed efferent fibers. These findings appear to be independent of acoustic reflex activity, as suppression was absent despite normal reflexes. Inter-aural suppre

Observations on recurrent syncope and presyncope in 641 patients.

BACKGROUND: Syncope is a common disorder that is potentially disabling and affects both young and old. Once neurological, cardiological, and metabolic causes have been excluded, there remains a group in which diagnosis is unclear; some may have an autonomic basis. We therefore did a retrospective study on consecutive patients referred to our tertiary referral autonomic centres between 1992 and 1998 with recurrent syncope and presyncope, in whom non-autonomic causes, before referral, had been sought and excluded. The object was to find out whether autonomic investigation helped diagnosis. METH

Simple methods for determining the affected side and performing otolith repositioning in cases of lateral semicircular canal-benign paroxysmal positional vertigo-canalolithiasis: Healthy-ear-down 135° maneuver and affected-ear-down 135° maneuver.

OBJECTIVE: To develop simple and effective methods for determining the affected side and promoting early symptom resolution in lateral semicircular canal-benign paroxysmal positional vertigo (L-BPPV) due to canalolithiasis, including its geotropic (L-BPPV-can-geo) and apogeotropic (L-BPPV-can-apo) variants. METHODS: A total of 146 cases of paroxysmal geotropic positioning nystagmus (PGPN, L-BPPV-can-geo) and 285 cases of persistent apogeotropic positional nystagmus (PAPN) due to peripheral vestibular disorders were analyzed. Positional and positioning nystagmus were observed and recorded usin

Optimizing Testing for BPPV - The Loaded Dix-Hallpike.

OBJECTIVES: Although the Dix-Hallpike testing is generally considered as the gold standard for the identification of the posterior canal benign paroxysmal positional vertigo (BPPV), we investigated a modification of the maneuver termed the "loaded Dix-Hallpike." STUDY DESIGN: Prospective randomized controlled trial. MATERIALS AND METHODS: Twenty-eight patients participated in this prospective study comparing the standard Dix-Hallpike (S-DH) to the loaded Dix-Hallpike (L-DH) test. Each patient underwent repeated testing with the S-DH and the L-DH. The patients were placed into two groups. Fou

Correlations between postural control and psychological factors in vestibular schwannoma patients.

BACKGROUND: Various individual factors have been described to influence postural performances related to vestibular schwannoma. However, psychological factors may also be involved in postural variations. OBJECTIVE: To identify relationships between postural performances, illness perceptions, coping, anxiety-depression and quality of life of patients with vestibular schwannoma. METHODS: Twenty-six patients who were scheduled for a surgical removal of a vestibular schwannoma underwent posturography tests and were asked to complete psychological questionnaires three days prior to surgery. The Se

Nucleus prepositus hypoglossi lesions produce a unique ocular motor syndrome.

OBJECTIVE: To describe the ocular motor abnormalities in 9 patients with a lesion involving the nucleus prepositus hypoglossi (NPH), a key constituent of a vestibular-cerebellar-brainstem neural network that ensures that the eyes are held steady in all positions of gaze. METHODS: We recorded eye movements, including the vestibulo-ocular reflex during head impulses, in patients with vertigo and a lesion involving the NPH. RESULTS: Our patients showed an ipsilesional-beating spontaneous nystagmus, horizontal gaze-evoked nystagmus more intense on looking toward the ipsilesional side, impaired p

Are signs of ocular tilt reaction in patients with cerebellar lesions mediated by the dentate nucleus?

A sensitive clinical sign of a vestibular tone imbalance in the roll plane is the ocular tilt reaction (OTR), a combination of skew deviation, ocular torsion and head and perceptual tilts such as tilts of the subjective visual vertical (SVV). Of these OTR components tilts of SVV are the most frequent. While these signs are regularly seen in patients with unilateral brainstem lesions, only a few case studies are available on their occurrence in patients with cerebellar lesions. Thus, the question arises whether cerebellar structures may be involved in contra- and/or ipsiversive tilts of the per

Operative management of a posterior semicircular canal dehiscence.

OBJECTIVE: The objective of this study was to describe the operative management of posterior canal dehiscence. METHODS: A transmastoid approach to and plugging of the posterior canal was performed for posterior semicircular canal dehiscence (PSCD). RESULTS: Postoperatively, the patient exhibited improvement in conductive hearing loss and vestibular symptoms. CONCLUSIONS: PSCD can cause symptoms identical to that of superior semicircular canal dehiscence. Successful PSCD plugging can be performed without visualization of the actual area of dehiscence.

Abnormal interaction between vestibular and voluntary head control in patients with spasmodic torticollis.

The functional status of vestibulo-collic reflexes in the sternocleidomastoid (SCM) muscles was investigated in 24 patients with spasmodic torticollis using small, abrupt 'drops' of the head. None had been treated with botulinum toxin injections during at least 4 months preceding the study. Eight of the patients, four of whom had been studied before surgery, were also studied after selective peripheral denervation of neck muscles. The reflex was of normal latency and duration in the 'passive drop' condition, in which subjects were instructed not to oppose the fall of the head. To study volunta

Surgical treatment of vertigo--the Karolinska Hospital policy.

Results are reported for 40 patients with disabling vestibular symptoms, treated with endolymphatic sac surgery (ELS), gentamicin injections in the middle ear, or vestibular neurectomy by the middle fossa approach. If ELS failed, an additional treatment with a second ELS procedure or gentamicin instillations was performed. If the gentamicin treatment failed, the patients were treated with a second series of gentamicin instillations or vestibular neurectomy or both. The results in relieving the patients of their vertigo were found to be almost the same with each of the treatment modalities: app
