Clinical implications of nonsarcomeric gene polymorphisms in hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is characterized by cardiomyocyte hypertrophy and fibrosis. Although is an autosomal dominant trait, a group of nonsarcomeric genes have been postulated as modifiers of the phenotypic heterogeneity. MATERIAL AND METHODS: We prospectively recruited 168 HCM patients and 136 healthy controls from three referral centres. Patients and controls were clinically stable at entry in the study. Nine polymorphisms previously associated with ventricular remodelling were determined: I/D ACE, AGTR1(A1666C), CYP11B2(C344T), PGC1-α(G482S), COLIA1(G2046T), ADRB1(R3

Cilostazol may prevent cardioembolic stroke in patients undergoing antiplatelet therapy.

OBJECTIVES: Randomised trials have shown the efficacy of antiplatelet therapy with cilostazol to prevent secondary ischaemic stroke. Recently, cilostazol has been reported to prevent the development and/or recurrence of atrial fibrillation (AF), which can potentially prevent cardioembolic stroke in patients undergoing antiplatelet therapy. Herein, we examined the impact of prior antiplatelet therapy with cilostazol on the incidence of cardioembolic stroke, which had not been fully investigated. METHODS: Using the multicenter retrospective study of stroke risk in antithrombotic therapy (RESTAT

Advances in the understanding of the pathogenesis, progression and diagnosis of myxomatous mitral valve disease in dogs.

A number of key questions remain unanswered in the pathogenesis of myxomatous mitral valve disease (MMVD). As MMVD typically afflicts small-breed dogs, a genetic basis has been implied. In addition, the fact that not all dogs within a risk group develop MMVDis still unexplained. Research into the pathogenesis of MMVD typically falls under three categorical divisions, namely genetic factors, mechanical factors of the valve and systemic factors. Genetic studies have implicated certain loci in the pathogenesis of MMVD. Of particular interest is the insulin-like growth factor (IGF)-1 locus, as IGF

Plasma 15-F2t-isoprostane in idiopathic pulmonary arterial hypertension.

BACKGROUND: 15-F2t-isoprostane (15-F2t-IsoP), a prostaglandin F2-like compound, is widely recognized as a biomarker of chronic heart failure. This study investigated the potential role and prognostic significance of plasma 15-F2t-IsoP in patients with idiopathic pulmonary arterial hypertension (IPAH). METHODS: Plasma 15-F2t-IsoP concentrations were determined in 80 consecutive IPAH patients at the time of their first right heart catheterization, and monitored for 30±12 months. The expression of 15-F2t-IsoP protein in autopsy lung samples was determined by immunohistochemical staining. RESULT

Biventricular / left ventricular pacing in hypertrophic obstructive cardiomyopathy: an overview.

Hypertrophic cardiomyopathy (HCM) is an autosomal dominant inherited genetic disease characterized by compensatory pathological left ventricle (LV) hypertrophy due to sarcomere dysfunction. In an important proportion of patients with HCM, the site and extent of cardiac hypertrophy results in severe obstruction to LV outflow tract (LVOT), contributing to disabling symptoms and increasing the risk of sudden cardiac death (SCD). In patients with progressive and/or refractory symptoms despite optimal pharmacological treatment, invasive therapies that diminish or abolish LVOT obstruction relieve he

The immediate effects of pacemaker-related electric remodelling on left ventricular function in patients with sick sinus syndrome.

AIMS: The immediate effects of electric remodelling on the left ventricular (LV) function by dual-chamber pacemakers remain unknown. The purpose of our study was to assess the interaction between heart rates and right ventricular pacing (VP) on LV contractility and diastolic function. METHOD AND RESULTS: Twenty-five patients with dual-chamber pacemakers and sick sinus syndrome were evaluated. Echocardiographic examinations included standard and tissue-Doppler echocardiography at bilateral mitral annulus margins under either the intrinsic atrio-ventricular sequential conduction (ventricular se

Intermittent hypoxia and sleep-disordered breathing: current concepts and perspectives.

There are three major types of sleep-disordered breathing (SDB) with respect to prevalence and health consequences, i.e. obstructive sleep apnoea syndrome (OSAS), Cheyne-Stokes respiration and central sleep apnoea (CSR-CSA) in chronic heart failure, and obesity hypoventilation syndrome (OHS). In all three conditions, hypoxia appears to affect body functioning in different ways. Most of the molecular and cellular mechanisms that occur in response to SDB-related hypoxia remain unknown. In OSAS, an inflammatory cascade mainly dependent upon intermittent hypoxia has been described. There is a stro

Cardiac resynchronization therapy: device-based medicine for heart failure.

Cardiac resynchronization therapy (CRT) or biventricular pacing is a novel adjunctive therapy for patients with advanced heart failure (HF). Many patients with severe HF have a left bundle branch block or an intraventricular conduction delay, with up to 25% of patients with a QRS > 120 ms, resulting in significant left ventricular (LV) dyssynchrony and a high mortality rate. The efficacy of CRT is based on the reduction in the conduction delay between the two ventricles and optimization of the ejection fraction, decrement in mitral regurgitation, LV remodeling, thus resulting in symptom improv

Impact of Physiologic Pacing Versus Right Ventricular Pacing Among Patients With Left Ventricular Ejection Fraction Greater Than 35%: A Systematic Review for the 2018 ACC/AHA/HRS Guideline on the Evaluation and Management of Patients With Bradycardia and Cardiac Conduction Delay: A Report of the American College of Cardiology/American Heart Association Task Force on Clinical Practice Guidelines and the Heart Rhythm Society.

BACKGROUND: It is unclear whether physiologic pacing by either cardiac biventricular pacing (BiVP) or His bundle pacing (HisBP) may prevent adverse structural and functional consequences known to occur among some patients who receive right ventricular pacing (RVP). AIM: Our analysis sought to review existing literature to determine if BiVP and/or HisBP might prevent adverse remodeling and be associated with structural, functional, and clinical advantages compared with RVP among patients without severe left ventricular dysfunction (>35%) who required permanent pacing because of heart block. Certain role of extra-domain A containing fibronectin in the development of pulmonary hypertension in the sugen/hypoxia mouse model.

BACKGROUND: Pulmonary hypertension (PH) is a heterogeneous condition with diverse aetiologies and clinical presentations, driven by remodelling of the pulmonary vasculature and right ventricular (RV) myocardium, progressing to right heart failure (RHF) with poor prognosis. Cardiovascular remodelling is characterized by re-expression of extra-domain A containing fibronectin (ED-A+ Fn), which our prior studies identified as a key pathogenetic mediator in monocrotaline-induced PH. This study investigated the role of ED-A+ Fn in the progression of hypoxia-associated PH and subsequent RHF, correspo

The Rationale of Neprilysin Inhibition in Prevention of Myocardial Ischemia-Reperfusion Injury during ST-Elevation Myocardial Infarction.

During the last three decades, timely myocardial reperfusion using either thrombolytic therapy or primary percutaneous intervention (pPCI) has allowed amazing improvements in outcomes with a more than halving in 1-year ST-elevation myocardial infarction (STEMI) mortality. However, mortality and left ventricle (LV) remodeling remain substantial in these patients. As such, novel therapeutic interventions are required to reduce myocardial infarction size, preserve LV systolic function, and improve survival in reperfused-STEMI patients. Myocardial ischemia-reperfusion injury (MIRI) prevention repr

Review article: current status of myocardial regeneration: new cell sources and new strategies.

Clinical trials of stem cell therapy in cardiology are based upon a reasonably solid foundation in animal laboratory research. The most widely used cell source in clinical trials has been the patient's own reconstituted bone marrow cell (BMC) aspirate. Cell sources in human bone marrow include hematopoietic stem cells, mesenchymal progenitor cells, and other cell types with many desirable characteristics. In vitro, they can be induced to become typical sarcomeres with centrally positioned nuclei and abundant mitochondria and to express atrial natriuretic peptide (ANP), brain natriuretic peptid

Clinical significance of fragmented QRS in isolated hypertension: Associations with ventricular arrhythmia burden and autonomic dysfunction.

BACKGROUND: Fragmented QRS (fQRS) complexes have been associated with myocardial conduction abnormalities and arrhythmic risk in various cardiac conditions. However, their clinical significance in patients with isolated hypertension, particularly in relation to ventricular arrhythmia burden and autonomic dysfunction, remains incompletely understood. This study aimed to investigate the association between fQRS, ventricular arrhythmia burden, left ventricular geometry, and cardiac autonomic function in these patients. METHODS: This retrospective observational study included 229 patients with is

Partial Anomalous Pulmonary Venous Return in Adults: Beyond Anatomy: A Cardiologist's Guide.

Partial anomalous pulmonary venous return (PAPVR) is a congenital cardiovascular malformation in which one or more pulmonary veins drain abnormally into the right atrium or systemic venous circulation, producing a pretricuspid left-to-right shunt. In anatomic terms, this condition is more precisely described as a partial anomalous pulmonary venous connection, whereas PAPVR refers to the physiological consequence of anomalous venous drainage. Historically, these anomalies were most often diagnosed during childhood; however, with increasing use of cross-sectional imaging modalities such as cardi

Dynamic Trends of Secondary Tricuspid Regurgitation in Acute Heart Failure and Association With Outcomes.

In acute heart failure (HF), the response of secondary tricuspid regurgitation (STR) to therapeutic management may indicate reverse right ventricular remodeling and response to treatment. This study sought to explore the dynamic trends of STR during acute HF hospitalization and their association with outcomes at follow-up. Among 463 acute hospitalized HF patients, 285 had significant STR on admission and were considered for this analysis. All patients underwent echocardiographic assessment on admission and prior to discharge and were divided into 2 groups: (1) significant STR on admission that

Novel Prognostic Insights in Multivalvular Disease: The Crucial Role of Right Ventricular-Pulmonary Artery Coupling.

BACKGROUND: To our knowledge, the prognostic role of right ventricular-pulmonary artery coupling (RV-PAc) in the context of multiple valvular heart disease (MVD) has so far not been evaluated. Therefore, in this study we assessed the prognostic role of surrogate of RV-PAc in patients affected by one of the most common MVDs: moderate mitral regurgitation (MR) with concomitant moderate tricuspid regurgitation (TR). METHODS: We collected comprehensive data of patients with coexisting moderate MR and moderate TR at 2 centers between 2014 and 2021. The outcome of interest was all-cause death. RV-P
