Experimental autoimmune labyrinthitis: assessment of molecular size of autoantigens in fractions of inner ear proteins eluted on the Mini Whole Gel Eluter.

In order to determine the molecular size of the causative autoantigens of experimental autoimmune labyrinthitis, crude inner ear antigen was separated into 14 fractions and the constituent molecules were identified by means of sodium dodecyl sulfate-polyacrylamide gel electrophoresis using the Mini Whole Gel Eluter. The mean total protein recovery was approximately 66%. Sensitization with 55-65 kDa proteins induced the highest number of infiltrating cells among the fractions and sensitization with 38-45 kDa proteins induced the second highest number of inflammatory cells. These results suggest

Clinical usefulness of glycerol vestibular-evoked myogenic potentials: preliminary report.

The detection of intense sound-induced vestibular-evoked myogenic potentials (VEMPs) on the sternocleidomastoid muscle comprises the basis of the saccular function test. In order to evaluate the endolymphatic hydrops (EH) of the saccule of the inner ear, a glycerol VEMP (GVEMP) test was performed in 15 patients with unilateral typical Ménière's disease (UMD) and 7 with delayed endolymphatic hydrops (DEH). Using the GVEMP test, 8 of the 15 patients (53%) with UMD were evaluated as being abnormal. In addition, a greater number of patients (67%) were judged to be abnormal when the results of the

[Technique for the assessment of vestibular compensation--clinical observation/quantitative neuro-otometry?].

PROBLEM: Reliable evaluation of vestibular compensation in indispensable to determine whether labyrinth surgery or vestibular neurectomy is indicated. It is also important for postoperative follow-up. Vestibular compensation can manifest itself differently in distinct frequency ranges. METHOD: Twenty volunteers were examined in five distinct situations of daily vestibular stimulation. Measurement of angular and linear head acceleration was performed using accelerometers fixed on the volunteers' head. A 200 Hz AD fed data to a PC database. FFT was used for data analysis. RESULTS: Stimulus fre

A new mouse insertional mutation that causes sensorineural deafness and vestibular defects.

This article describes a new recessive insertional mutation in the transgenic line TgN2742Rpw that causes deafness and circling behavior in mice. Histologic analysis revealed virtually complete loss of the cochlear neuroepithelium (the organ of Corti) in adult mutant mice. In association with the neuroepithelial changes, there is a dramatic reduction of the cochlear nerve supply. Adult mutants also show morphological defects of the vestibular apparatus, including degeneration of the saccular neuroepithelium and occasional malformation of utricular otoconia. Audiometric evaluations demonstrated

[Distortion product otoacoustic emissions for the assessment of auditory sensitivity].

Distortion product otoacoustic emissions (DPOAEs) were evaluated in 494 normal and 506 cochlear-impaired human ears, to determine whether DPOAEs depend on factors such as background noise, the shape of the pure tone audiogram, sex and aging, and whether a DPOAE test can perform well in distinguishing normal-hearing from hearing-impaired ears. The amplitudes of DPOAEs were measured at the frequency of 2f1-f2 (f1 < f2, f2/f1 = 1.22, f2 at 1, 2 and 4 kHz) using as stimuli two pure tones at level of 70 dB from an ILO92 Otoacoustic Emission Analyzer. The correlation coefficients between the DPOAE l

Characteristics of six otologic diseases involving vertigo.

To characterize otologic causes for vertigo, data on 564 patients with the six most common diseases involving vertigo were retrieved from the database of a computer-aided diagnostic system for neurotologic diseases. The diseases were Meniere's disease, vestibular schwannoma, benign paroxysmal positional vertigo, vestibular neuritis, sudden deafness, and traumatic vertigo. The prevalence of tinnitus in the study population was 76%. The most severe forms of vertigo and nausea were found in vestibular neuritis, whereas the most severe case of tinnitus appeared in Meniere's disease. Of the patient

Neurohumoral mechanism of space motion sickness.

This paper reviews existing hypotheses concerning the mechanisms of adaptation of the vestibular apparatus and related somatosensory systems to microgravity with reference to the flight data. Having in view theoretical concepts and experimental data accumulated in space flights, a conceptual model of the development of a funtional system responsible for the termination of vestibular dysfuntion and space motion sickness manifestations is presented. It is also shown that changes in the hormonal status during motion sickness induced by vestibular stimulation give evidence that endocrine regulatio

Retrolabyrinthine surgery: anatomy and pathology.

Retrolabyrinthine surgery is done to expose the cerebellopontine angle directly through the ear. It is indicated when the hearing in the ear to be operated upon is useful. The surgical technique entails four steps: complete mastoidectomy, extended posterior exposure of the sigmoid sinus, exposure of the posterior fossa dura, and exposure of the cerebellopontine angle. Retrolabyrinthine exposure of the cerebellopontine angle is indicated in tic douloureux, atypical facial pain, and hemifacial spasm. Recently, this approach has been used for sectioning the vestibular nerve, exploration of the po

Antibodies against inner-ear proteins in the sera of patients with inner-ear diseases.

Sera from patients with various inner-ear diseases, especially Ménière's disease, were investigated by Western blot against guinea pig inner-ear proteins. Of 45 patients, 24 (53%) with various inner-ear diseases had antibodies against inner-ear proteins, compared with 0 of 10 (0%) in control subjects without inner-ear diseases. Of the 10 proteins that showed a positive reaction with patient sera, the 28-kD band was unique in that it appeared only in the membranous fraction of the inner ear and was highly positive (28%) in reaction with Ménière's disease patient sera. The results in the present

Retrolabyrinthine vestibular nerve section: efficacy in disorders other than Menière's disease.

The retrolabyrinthine vestibular nerve section has evolved as an effective treatment for intractable vertigo of peripheral vestibular origin when hearing preservation is desired. This report studies the efficacy of retrolabyrinthine vestibular nerve section for control of vertigo due to causes other than Meniere's disease. This report details our experience with 42 patients with a wide variety of diagnoses. The reduced success rate of retrolabyrinthine vestibular nerve section in these patients is difficult to evaluate, as very few patients have been analyzed with respect to their specific dia

Translabyrinthine Fistula Closure in Inner Ear Malformations with Otogenic Cerebrospinal Fluid Leak.

Inner ear malformations can cause abnormal communication between the inner ear and subarachnoid space, leading to otogenic cerebrospinal fluid (CSF) leakage and subsequent bacterial meningitis. Persistent leakage may result in recurrent meningitis, which is associated with neurological sequelae and poor prognosis; thus, early diagnosis and surgical intervention are essential. Various surgical strategies have been reported, commonly termed middle ear obliteration and inner ear obliteration. However, their definitions remain ambiguous, and techniques vary widely in approach and effectiveness. Th

Cisplatin Differentially Damages Mammalian Vestibular End Organs.

OBJECTIVES: Cisplatin is an effective anticancer drug that can induce cochleotoxicity in up to 50 to 60% of treated patients. However, the vestibulotoxic potential of cisplatin remains unclear. Our lab previously developed a multicycle model of cisplatin administration in mice that results in hearing loss similar to that observed clinically. The purpose of this study was to comprehensively characterize the functional and morphological consequences of cisplatin on the vestibular periphery in this mouse model. DESIGN: Twenty-nine adult CBA/CaJ mice were assigned to control or cisplatin-treated

Rare otologic presentation of cat eye syndrome.

We encountered an extremely rare case where a patient with cat eye syndrome (CES) who presented with symptoms of posterior semicircular canal dehiscence (PSCD). CES is a rare genetic disorder, resulting from duplication of chromosome 22. Patients may present with variable phenotypes, including characteristic of coloboma, heart defect, periauricular skin pit/tag, microtia, anal atresia and mildly retarded mental development in some cases. PSCD is also a disease of the inner ear, where patients present with third window signs and symptoms due to lack of bony coverage. PSCD is usually associated

Usher syndrome and non-syndromic deafness: Functions of different whirlin isoforms in the cochlea, vestibular organs, and retina.

Usher syndrome (USH) is the leading cause of inherited combined vision and hearing loss. However, mutations in most USH causative genes lead to other diseases, such as hearing loss only or vision loss only. The molecular mechanisms underlying the variable disease manifestations associated with USH gene mutations are unclear. This review focuses on an USH type 2 (USH2) gene encoding whirlin (WHRN; previously known as DFNB31), mutations in which have been found to cause either USH2 subtype USH2D or autosomal recessive non-syndromic deafness type 31 (DFNB31). This review summarizes the current kn
