Non-culprit coronary lesions in young patients have higher rates of atherosclerotic progression.

The present study aimed to investigate whether non-culprit coronary lesions (NCCLs) in young patients (<45 years) who underwent percutaneous coronary intervention (PCI) with stents have higher rates of atherosclerotic progression than older patients. Eight hundred and forty-eight consecutive patients who underwent successful PCI with stents and second coronary angiography in a single center from January 7, 2008 to May 7, 2013 were enrolled. NCCL progression was assessed using three-dimensional quantitative coronary angiography and was defined as ≥ 10% diameter reduction of preexisting stenoses

Image-Derived Modeling to Assess Coronary Proximity in Patients Undergoing Transcatheter Pulmonary Valve Replacement With Self-Expanding Valves.

BACKGROUND: The potential for coronary artery compression (CC) during transcatheter pulmonary valve replacement (TPVR) using self-expanding valves (SEV) is not fully understood, yet anecdotal reports suggest that this risk exists. AIMS AND METHODS: We performed a retrospective cohort study of patients evaluated for SEV-TPVR to evaluate the relationship between the right ventricular outflow tract (RVOT) and coronary arteries (CA). CT-derived segmentations of the RVOT and CA were created using machine learning. A 2D map of the distance between the RVOT surface and CA, in systole and diastole, w

Quantitative Prediction of Paravalvular Leak in Transcatheter Aortic Valve Replacement Based on Tissue-Mimicking 3D Printing.

OBJECTIVES: This study aimed to develop a procedure simulation platform for in vitro transcatheter aortic valve replacement (TAVR) using patient-specific 3-dimensional (3D) printed tissue-mimicking phantoms. We investigated the feasibility of using these 3D printed phantoms to quantitatively predict the occurrence, severity, and location of any degree of post-TAVR paravalvular leaks (PVL). BACKGROUND: We have previously shown that metamaterial 3D printing technique can be used to create patient-specific phantoms that mimic the mechanical properties of biological tissue. This may have applicat

Applicability of next generation balloon-expandable transcatheter heart valves in aortic annuli exceeding formally approved dimensions.

OBJECTIVES: Physicians are frequently confronted with patients suffering from aortic stenosis with annular diameters exceeding dimensions in which currently available transcatheter heart valves (THV) are formally approved. Experience in patients receiving significantly undersized Sapien 3 (S3) THV (Edwards Lifesciences, Inc., Irvine, CA, USA) in aortic annuli up to 32 mm has not been reported so far. METHODS: Patients with aortic annuli exceeding the formally determined upper size limit and who received a 29 mm S3, were identified from our database. Calcification pattern and annulus dimension

Adjunctive stenting of endograft limbs during endovascular treatment of infrarenal aortic and iliac aneurysms according to 3-projection completion angiography.

PURPOSE: To evaluate the role of 3-projection completion angiography in defining endograft limb stenosis after endovascular aneurysm repair (EVAR) and to determine the role of adjunctive stenting in reducing the risk of endograft limb occlusion. METHODS: In our center, stent-graft limb dilation was routinely done after endograft deployment in patients with preoperatively identified severe iliac axis angulation to reduce the incidence of limb stenosis. Completion angiography was then routinely performed in anteroposterior (AP) and 45° right and left oblique projections to identify perioperativ

Time-domain optical mammography SoftScan: initial results.

RATIONALE AND OBJECTIVES: Near-infrared (NIR) technology appears promising as a noninvasive technique for breast cancer screening and diagnosis. The technology capitalizes on the relative transparency of human tissue in this spectral range and its sensitivity to the main components of the breast: water, lipid, and hemoglobin. In this study, the authors report quantitative measurements of these components and the functional contrast between healthy and diseased tissue. MATERIALS AND METHODS: A four-wavelength time domain optical imaging system was used to perform noninvasive NIR measurements i

Visual classification of three computed tomography lung patterns to predict prognosis of COVID-19: a retrospective study.

BACKGROUND: Quantitative evaluation of radiographic images has been developed and suggested for the diagnosis of coronavirus disease 2019 (COVID-19). However, there are limited opportunities to use these image-based diagnostic indices in clinical practice. Our aim in this study was to evaluate the utility of a novel visually-based classification of pulmonary findings from computed tomography (CT) images of COVID-19 patients with the following three patterns defined: peripheral, multifocal, and diffuse findings of pneumonia. We also evaluated the prognostic value of this classification to predi

Pre-procedural fit-testing of TAVR valves using parametric modeling and 3D printing.

BACKGROUND: Successful transcatheter aortic valve replacement (TAVR) requires an understanding of how a prosthetic valve will interact with a patient's anatomy in advance of surgical deployment. To improve this understanding, we developed a benchtop workflow that allows for testing of physical interactions between prosthetic valves and patient-specific aortic root anatomy, including calcified leaflets, prior to actual prosthetic valve placement. METHODS: This was a retrospective study of 30 patients who underwent TAVR at a single high volume center. By design, the dataset contained 15 patient

No Benefit After THA Performed With Computer-assisted Cup Placement: 10-year Results of a Randomized Controlled Study.

BACKGROUND: Computer-assisted surgery (CAS) for cup placement has been developed to improve the functional results and to reduce the dislocation rate and wear after total hip arthroplasty (THA). Previously published studies demonstrated radiographic benefits of CAS in terms of implant position, but whether these improvements result in clinically important differences that patients might perceive remains largely unknown. QUESTIONS/PURPOSES: We hypothesized that THA performed with CAS would improve 10-year patient-reported outcomes measured by validated scoring tools, reduce acetabular polyethy

YOLOv10-PDTFN: An integrated ROI segmentation and transformer fusion framework for osteoporosis classification in knee radiographs.

INTRODUCTION: Reduced bone mineral density and degeneration of bone microarchitecture are hallmarks of Osteoporosis, a degenerative skeletal condition that raises the risk of fractures. Due to the subjective nature of conventional radiography assessment and the potential for inter-observer variability, early diagnosis and osteoporosis grading remain clinical challenges. Although recent developments in deep learning have demonstrated promise in automating diagnostic tasks, it is still challenging to identify contextual structural patterns and minor trabecular alterations in radiographs.
