Epidemiology of collegiate baseball injuries.

OBJECTIVE: We sought to establish injury incidence, onset, location, type, and severity for a collegiate baseball team. The second objective was to compare the number of musculoskeletal problems for which baseball players sought treatment with those that resulted in time lost or modified participation. METHODS: This was a prospective, epidemiologic study. A complaint was defined as any evaluation by a player to the medical staff that required either evaluation or treatment. An injury was defined as any complaint that resulted in altered participation or time lost from practice or game partici

Short-Term Clinical Outcomes of Platelet-Rich Plasma Injections in Patients With Rotator Cuff Tendinopathy and Partial-Thickness Rotator Cuff Tears.

INTRODUCTION: Platelet-rich plasma (PRP) has been increasingly applied for rotator cuff pathology; however, clinical evidence regarding its short-term efficacy remains limited. This study evaluated the effectiveness of a single PRP injection for rotator cuff injury. METHODS: This single-center retrospective study analyzed 25 cases managed between August 2024 and August 2025 to evaluate the effectiveness of a single PRP injection for rotator cuff injury. Pain was assessed using the visual analog scale (VAS), and shoulder function was assessed using Shoulder 36 (Version 1.3) at baseline and at

Pressed Long Head of the Biceps Tendon Autograft Augmentation Enhances Tendon-to-Bone Healing in a Rat Model of Rotator Cuff Repair.

PURPOSE: To investigate the underlying regeneration mechanisms of biological augmentation using pressed long head biceps tendon (LHBT) in a rat model. METHODS: Bilateral infraspinatus tendons were torn and repaired 1 week later in 32 Sprague-Dawley rats. In the biologic augmentation (BA) group, the LHBT was extracted, pressed, and augmented on the repair site. Histological evaluation was performed at 3 and 6 weeks to measure the thickness of the repaired tendon, number of chondrocytes and non-chondrocytes, percentage of aligned chondrocytes, areas of fibrocartilage and collagen fiber, and COL

Survivorship of implanted bone marrow-derived mesenchymal stem cells in acute rotator cuff tear.

BACKGROUND: This study examined whether a mesenchymal stem cells (MSCs)-seeded 3-dimensional construct into a tendon defect would promote cellular differentiation and matrix healing. MATERIALS AND METHODS: Bone marrow was harvested from the iliac crests of 2 male New Zealand White rabbits. The MSCs were cultured, and an open-cell polylactic acid (OPLA) scaffold was encapsulated with these cells. The injury model was a 5-mm × 5-mm-sized full-thickness window defect in the central part of each rotator cuff tendon. The defects on the right side were grafted with the autologous MSCs-seeded OPLA s

Tenocytes of chronic rotator cuff tendon tears can be stimulated by platelet-released growth factors.

BACKGROUND: Bone-to-tendon healing after rotator cuff repairs is mainly impaired by poor tissue quality. The tenocytes of chronic rotator cuff tendon tears are not able to synthesize normal fibrocartilaginous extracellular matrix (ECM). We hypothesized that in the presence of platelet-released growth factors (PRGF), tenocytes from chronically retracted rotator cuff tendons proliferate and synthesize the appropriate ECM proteins. MATERIALS AND METHODS: Tenocytes from 8 patients with chronic rotator cuff tears were cultured for 4 weeks in 2 different media: standard medium (Iscove's Modified Du

Multimodality imaging of the postoperative shoulder.

Multimodality imaging of the postoperative shoulder includes radiography, magnetic resonance (MR) imaging, MR arthrography, computed tomography (CT), CT arthrography, and ultrasound. Target-oriented evaluation of the postoperative shoulder necessitates familiarity with surgical techniques, their typical complications and sources of failure, knowledge of normal and abnormal postoperative findings, awareness of the advantages and weaknesses with the different radiologic techniques, and clinical information on current symptoms and function. This article reviews the most commonly used surgical pro

Posterosuperior and anterosuperior impingement of the shoulder in overhead athletes-evolving concepts.

During throwing motion the athlete puts enormous stress on both the dynamic and the static stabilisers of the shoulder. Repetitive forces cause adaptive soft tissue and bone changes that initially improve performance but ultimately may lead to shoulder pathologies. Although a broad range of theories have been suggested for the pathophysiology of internal impingement, the reasons are obviously multifactorial. This review aims to critically analyse the current literature and to summarise clinically important information. The cardinal lesions of internal impingement, articular-sided rotator cuff

Current concept for the use of PRP in arthroscopic surgery.

PRP is commonly used in sports medicine and because it is supposed to increase healing capacities of damaged tissues, it is expected to be increase efficiency or god clinical outcomes when added to arthroscopic surgical procedure. The current review of literature explores the evidence-based medicine supporting the use of PRP in three arthroscopic related disorders. Regarding cartilage lesions of the knee, many studies are exploring several aspect of cartilage lesion treatment. It appears that PRP, in some protocols with specific concentration, should be more efficient than current therapies in

[Superior labrum-biceps anchor complex].

The focus in overhead sport injuries of the shoulder has been directed on lesions of the superior labrum-biceps complex during the last few years. This is due to improved imaging modalities as well as a better understanding of the anatomy and biomechanics. Arthroscopy is capable of diagnosing and treating these lesions. The anatomy, biomechanics, and operative treatment strategies and techniques are described in detail. The superior labrum anterior-posterior (SLAP) lesion is found in young active overhead athletes. Based on their classification and concomitant lesions, the unstable SLAP lesion

Functional internal rotation is associated with subscapularis tendon healing and increased scapular tilt after Grammont style bony increased offset reverse shoulder arthroplasty with 155° humeral implant.

BACKGROUND: While forward flexion consistently improves after reverse shoulder arthroplasty (RSA), restoration of internal rotation behind the back (IR1) is much less predictable. This study aims to evaluate the role of the subscapularis tendon in restoration of IR and identify other factors that may influence IR such as anterior scapular tilt and postoperative passive internal rotation at 90° of abduction (IR2). The hypothesis was that IR1 is positively associated with both subscapularis healing, postoperative passive IR2, and anterior scapular tilt. METHODS: A retrospective review was perfo

Humeral resurfacing arthroplasty in combination with latissimus dorsi tendon transfer in patients with rotator cuff tear arthropathy and preserved subscapularis muscle function: preliminary report and short-term results.

PURPOSE: Humeral resurfacing arthroplasty represents an alternative option to hemiarthroplasty for treatment of cuff tear arthropathy (CTA), with the advantages as follows: suitability for relatively young and high-demand patients because of preservation of bone stock and no loss of length, less invasive surgery, shorter operation time, no risk of periprosthetic stem fractures, and revision surgery can be undertaken easily. In the current study, resurfacing arthroplasty in combination with latissimus dorsi tendon transfer for CTA was performed. Three hypotheses: first, humeral resurfacing arth

Prosthetic replacement of the shoulder for the treatment of defects in the rotator cuff and the surface of the glenohumeral joint.

We operatively treated, between 1978 and 1987, twenty-one shoulders in nineteen patients, fifty-four to eighty-four years old, who had disabling pain attributable to a massive tear of the rotator cuff, accompanied by loss of the surface of the glenohumeral joint. These patients were not candidates for total shoulder replacement because of the massive deficiency in the cuff and the fixed upward displacement of the humeral head. A prerequisite for hemiarthroplasty was a functionally intact coracoacromial arch to provide superior secondary stability for the prosthesis. One important aspect of the

[Double oblique MR images of the shoulder: comparison with conventional images].

PURPOSE: Because the scapula is not only slanted on transverse sections but also inclines on sagittal section, we now perform shoulder MR imaging using double oblique images (DOI), which are planes perpendicular or parallel to the long axis of the scapula obtained with oblique sagittal scout imaging. The purpose of this study was to evaluate the usefulness of double oblique shoulder MR imaging. MATERIALS AND METHODS: MR images of shoulders with operatively or arthroscopically proven lesions (20 cases) that had been examined on both conventional images (CI) and DOI were retrospectively reviewe

The clinical presentation of shoulder instability including on field management.

In the athlete, shoulder instability may result from external high-energy trauma or repetitive overuse. Our understanding of how shoulder instability may contribute to and result from shoulder injury has improved considerably during the past decade. In addition, the delicate balance between mobility and stability and the interrelationship between instability and rotator cuff disease make the clinical management of the unstable shoulder and its field treatment a challenge for physicians, therapists, trainers, and others responsible for the care of injured athletes.

Leveraging Electronic Health Record Data to Evidence Collider Stratification Bias and Inform Clinical Epidemiology: Obesity, Diabetes and Rotator Cuff Tears.

Electronic health record (EHR) data are increasingly used for case-control investigations. Using multiple control groups in de-identified EHR-data we evidence how conditioning on imaging (descendent of a collider: symptomology) can perturb exposure estimations enough to reverse conclusions. Because imaging is often required for rotator cuff tear diagnosis, some argue imaging should be required for control selection. We constructed two control groups (with vs. without imaging) to evaluate selection bias through collider stratification involving metabolic exposures-body mass index (BMI), type 1

The use of a patch to augment rotator cuff surgery - A survey of UK shoulder and elbow surgeons.

BACKGROUND: Rotator cuff tears are a common cause of shoulder pain and can result in prolonged periods of pain, disability and absence from work. Rotator cuff repair surgery is increasingly used in an attempt to resolve symptoms but has failure rates of around 40%. There is a pressing need to improve the outcome of rotator cuff repairs. Patch augmentation increasingly being used within the NHS in an attempt to reduce repair failures. The aim of this survey was to determine current UK practice and opinion relating to the factors that influence choice of patch, current patient selection and will
