The Stener Lesion and Complete Ulnar Collateral Ligament Injuries of the Thumb A Review.

A Stener lesion is a complete tear of the ulnar collateral ligament (UCL) from the thumb proximal phalanx at the level of the metacarpophala … uture, suture anchor, or pull-out suture techniques. Chronic injuries are treated with dynamic (via tendon transfer) or static (via grafting) reconstruction methods, while MCP arthrodesis or arthroplasty can be reserved for cases where MCP joint osteoarthritis is present. Overall, patient outcomes are generally good with operative treatment of Stener lesions. Thi …

Lateral condylar retrograde humeral nail for management of high-energy distal humeral fractures.

PURPOSE: Distal metaphyseo-diaphyseal humeral fractures are challenging particularly if open, comminuted, or associated with nerve injury. I … he arm, shoulder, and hand score was 11.2 ± 6.4. Only five patients underwent subsequent successful tendon transfer. CONCLUSIONS: Lateral condylar retrograde humeral nail with early radial nerve exploration in cases of high-energy distal metaphyseo-diaphyseal humeral fractures yielded good results regarding union and spontaneous radial nerve recovery or later on …

3D morphometric analysis of the human incudomallear complex using clinical cone-beam CT.

Human middle ears show large morphological variations. This could affect our perception of hearing and explain large variation in experimentally obtained transfer functions. Most morphological studies focus on capturing variation by using landmarks on cadaveric temporal bones. We present statistical shape analysis based on clinical cone beam CT (CBCT) scans of 100 patients. This allowed us to include surface information on the incudomallear (IM) complex (joint, ligaments and tendon not included) of 123 healthy ears with a scanning resolution of 150 μm and without a priori assumptions. Good results after Ponseti treatment for neglected congenital clubfoot in Ethiopia. A prospective study of 22 children (32 feet) from 2 to 10 years of age.

BACKGROUND AND PURPOSE: Neglected clubfoot deformity is a major cause of disability in low-income countries. Most children with clubfoot have little access to treatment in these countries, and they are often inadequately treated. We evaluated the effectiveness of Ponseti's technique in neglected clubfoot in children in a rural setting in Ethiopia. PATIENTS AND METHODS: A prospective study was conducted from June 2007 through July 2010. 22 consecutive children aged 2-10 years (32 feet) with neglected clubfoot were treated by the Ponseti method. The deformity was assessed using the Pirani scori

[Hypokalemic thyrotoxic periodic paralysis (HTPP). Rare differential diagnosis in case of acute tetraparesis in Europe].

BACKGROUND: Differential diagnosis of acute tetraparesis includes paraplegia caused by trauma, ischemia, inflammation or tumor, Guillain-Barré syndrome, periodic paralysis, myasthenia gravis, and dissociative paralysis. CASE REPORT: The case of a 45-year-old man is reported who noticed weakness of both legs after heavy labor. In the evening he was unable to climb stairs, at night he noticed additional weakness in both arms and dyspnea. After transfer to the authors' department profound tetraparesis, tachycardia and slight dyspnea were found on examination. Cranial nerve examination was unrema

Interphalangeal joint arthrodesis of the lesser toes.

Fusion of the PIP joint of the lesser toes provides sound correction of deformity of that joint. Fusion is achieved reliably in most cases and fibrous ankylosis is well tolerated in those that fail. Patients at the extremes of the age scale are perhaps less tolerant of the procedure; complaints include poor cosmesis and subjective lack of control of the toes [18]. Although fusion of the DIPJ is a sound primary procedure, it would seem prudent to reserve PIP fusion as a revision procedure [7] or for the treatment of severe, fixed hammer toe deformities. For flexible or mild, fixed hammer toe de

Results of suture of the radial, median, and ulnar nerves after missile injury below the axilla.

OBJECTIVE: To evaluate the functional outcome and return to work after suture of upper extremity nerves after low-velocity missile injury. METHODS: Forty-one patients had suture of the radial, median, and/or ulnar nerves between the wrist and axilla, 3 to 17 weeks after low-velocity missile injury. Postoperative functional outcome was good if no supplemental techniques (permanent splints, tendon or muscle transfer, fusion) were required, fair if supplemental techniques were required to restore a functional hand, and poor if the hand was severely deformed. The follow-up period was 14 to 38 mon

[A case of thrombocytopenia due to heparin therapy for a progressing ischemic stroke].

A 44-year-old man was admitted, complaining of weakness of his left limbs. On admission, he was oriented and cooperative. He showed positive Barré sign only on the left leg. Deep tendon reflexes were unremarkable; pathological reflexes were negative. There was no cerebellar ataxia or sensory deficit. Immediately after admission, monoplegia of the left leg progressed. He was started on heparin infusion therapy. His paralysis had been resolved by the 5th day. MRI of the brain demonstrated an infarction involving the body of the corpus callosum, the right cingulate gyrus, and the right paracentra

Salvage of the failed Darrach procedure.

Six patients (5 post-traumatic, 1 rheumatoid) underwent a three-component reconstruction for correction of dorsal instability and radioulnar impingement following failure of a Darrach resection of the entire distal end of the ulna. The technique was devised to prevent simultaneous coronal and sagittal instability. The procedure used longitudinal intramedullary tenodesis of the extensor carpi ulnaris tendon, dorsal transfer of the pronator quadratus through the interosseous space, and temporary percutaneous pinning to maintain corrected distal radioulnar relationship. The were evaluated for 11

Recurrent Motor Branch Injury During Open Carpal Tunnel Release: A Case Report and Recommendations for Prevention of This Surgical Hazard.

BACKGROUND: The purpose of this article is to present a case reconstruction of iatrogenic recurrent motor branch of the median nerve (RMN) injury following open carpal tunnel release (OCTR). Additionally, we review the literature on safe incision lines to minimize risk of iatrogenic injury to the RMN. METHODS: A 48-year-old right-handed woman developed progressive thenar atrophy and thumb dysfunction after OCTR. Surgical reexploration 7 months post-OCTR revealed a transected RMN embedded in scar tissue. Microsurgical epineural nerve repair was performed, combined with tendon transfer and stru…

Editorial Commentary: Superior Capsular Reconstruction Employing Allograft Heals and Functions Well if the Graft Is Sufficiently Thick and Stiff.

Superior capsular reconstruction (SCR) has fallen into disrepute, and the numbers performed appear to be on the decline because it is technically demanding and time-consuming, requires a long postoperative recovery, and does not always heal or function as expected. In addition, two "new kids on the block," the subacromial balloon spacer and the lower trapezius tendon transfer, have emerged as viable alternatives for low-demand patients who cannot tolerate a lengthy recovery and for high-demand patients who lack external rotation strength, respectively. However, carefully selected patients cont…

Isolated musculocutaneous neuropathy: a case report.

OBJECTIVE: To present the diagnostic and clinical features of musculocutaneous neuropathy, propose possible conservative management strategies, and create awareness of this rare condition. CASE PRESENTATION: We present the case of a 24-year old competitive soccer athlete, who sought care for an unrelated lower extremity complaint. Upon examination, significant wasting of the right biceps was noted. The patient reported right arm pain and weakness that began six months prior, following a long sleep with his arm beneath him. Neurological examination revealed an absent deep tendon reflex of C5 o

The anatomic location and importance of the tibialis posterior fascicular bundle at the sciatic nerve bifurcation: report of 3 cases.

The authors present the cases of 3 patients with severe injuries affecting the peroneal nerve combined with loss of tibialis posterior function (inversion) despite preservation of other tibial nerve function. Loss of tibialis posterior function is problematic, since transfer of the tibialis posterior tendon is arguably the best reconstructive option for foot drop, when available. Analysis of preoperative imaging studies correlated with operative findings and showed that the injuries, while predominantly to the common peroneal nerve, also affected the lateral portion of the tibial nerve/divisio

Effectiveness of the Cobb-Stainsby excision arthroplasty.

BACKGROUND: Dislocated metatarsophalangeal joints from clawed or hammer toes can be a disabling consequence of several conditions. The Cobb-Stainsby forefoot arthroplasty combines partial phalangectomy (Stainsby) with extensor tendon transfer to the metatarsal head (Cobb). We present a retrospective, three surgeon case series of 215 toes in 126 patients. METHODS: Early results and complications were gathered from the medical charts of 126 patients who met the inclusion criteria. Seventy-five patients were contactable by phone with a follow up range of 12-82 months (median follow up 45 months)…
