Radiologic manifestations of pulmonary tuberculosis in patients of intensive care units.

BACKGROUND: Tuberculosis (TB) is a serpent disease with various pulmonary manifestations, and timely diagnosis of the disease is paramount, since delayed treatment is associated with severe morbidity, particularly in intensive care units (ICU). Therefore, it is imperative that intensivists understand the typical distribution, patterns, and imaging manifestations of TB. AIM: To describe different manifestations of pulmonary TB in patients in the ICU. METHODS: In a retrospective study, all patients with a clinical and a laboratory-confirmed diagnosis of TB who were admitted to the ICU were ent

Pulmonary changes of pleural TB: up-to-date CT imaging.

BACKGROUND: The objective of this study was to evaluate pulmonary abnormalities of pleural TB by CT scanning and to determine CT scan findings for the development of the paradoxical response (PR). METHODS: CT scans were performed for 349 patients with pleural TB (between 2008 and 2013). We excluded 34 patients with coexisting pulmonary disease (n 5 13) or a totally collapsed lung (n 5 21). We analyzed CT scans focusing on pulmonary abnormalities such as the presence of consolidation, cavitation, interlobular septal thickening, and micronodules and their distribution. In addition, we recorded

Rigid Mini-Thoracoscopy Versus Semirigid Thoracoscopy in Undiagnosed Exudative Pleural Effusion: The MINT Randomized Controlled Trial.

BACKGROUND: There is debate regarding the ideal instrument for medical thoracoscopy. The authors compared rigid mini-thoracoscopy with semirigid thoracoscopy for thoracoscopic pleural biopsy. METHODS: Consecutive subjects with undiagnosed exudative pleural effusion were randomized (1:1 ratio) to mini-thoracoscopy or semirigid thoracoscopy groups. The primary objective was a comparison of the diagnostic yield of pleural biopsy. Key secondary outcomes were the comparison of sedative/analgesic dose, operator-rated and patient-rated pain on visual analog scale (VAS), operator-rated overall proced

Enzyme-linked immunospot assay for interferon-gamma in the diagnosis of tuberculous pleurisy.

Patients presenting with pleural effusion of undetermined aetiology were prospectively enrolled, and an enzyme-linked immunospot (ELISPOT) assay on pleural fluid and peripheral blood was performed. Forty patients were studied, including 19 with culture- or biopsy-confirmed (n = 15) or clinically compatible (n = 4) tuberculous pleurisy, and 21 with pleural effusions due to non-tuberculous causes. The sensitivity, specificity and positive and negative predictive values of the assay were 94.7%, 85.7%, 85.7% and 94.7%, respectively, on pleural fluid, and 77.8%, 90.5%, 87.5% and 82.6%, respectively

Omentoplasty in thoracic surgery.

Plombage methods were designed for treating pulmonary tuberculosis surgically around 1945 when pulmonary tuberculosis was regarded as a "traitor disease" in Japan. Nagaishi had reported extrapleural synthetic resin ball plombage in 1948 as the world's first case, but it was complicated by empyema due to perforation of cavities and rupture of lesions because the resin ball was hard. Later, fillers were improved by using soft material. At the present time, only air plombage and omentoplasty remain. Especially the latter has gained widespread use in the thoracic surgery field and become a standar

Mechanisms of T-lymphocyte accumulation during experimental pleural infection induced by Mycobacterium bovis BCG.

Tuberculous pleurisy is a frequent extrapulmonary manifestation characterized by accumulation of fluid and inflammatory cells in the pleural space. Here, we investigated the mechanisms of T-lymphocyte accumulation in the pleural space by using a murine model of pleurisy induced by Mycobacterium bovis BCG. Intrathoracic (i.t.) injection of BCG (4.5 x 10(5) bacteria/cavity) induced accumulation of T lymphocytes in the pleural cavities of C57BL/6 mice. We observed the presence of CFU in pleural washes conducted 1, 2, 3, 7, and 15 days after pleurisy induction. Pretreatment with fucoidan inhibited

Carcinomatous and tuberculous pleural effusions. Comparison of tumor markers.

As an aid in the differential diagnosis of exudative pleural effusions, tumor markers were investigated. We measured immunosuppressive acidic protein (IAP), carbohydrate antigen 19-9 (CA 19-9), tissue polypeptide antigen (TPA), carcinoembryonic antigen (CEA), adenosine deaminase (ADA), and alpha 1-acid glycoprotein (AGP) in the pleural fluid of 36 patients with carcinomatous pleural effusions and of 35 patients with tuberculous pleurisy because we have frequently found these diseases to be associated with exudative pleuritis. Tuberculous pleural effusions had significantly higher levels of IAP

T-helper type 1/T-helper type 2 balance in malignant pleural effusions compared to tuberculous pleural effusions.

STUDY OBJECTIVES: Malignant and tuberculous pleurisies are two major causes of lymphocyte-dominant pleurisy. Several studies have already reported that tuberculous pleurisy is a T-helper type 1(Th1)-dominant disease. In this study, we sought to examine the Th1/T-helper type 2 (Th2) balance, especially focusing on the polarizing status of T-cells to Th1/Th2 in malignant pleural effusions by measuring cytokines in pleural effusions and by evaluating the polarizing status of T-cells on the point of stimulation with interleukin (IL)-12 and/or IL-18. Furthermore, we evaluated inhibitors of interfer

Diagnosis of tuberculosis from smear-negative presumptive TB cases using Xpert MTB/Rif assay: a cross-sectional study from Nepal.

BACKGROUND: In most developing countries, smear-negative pulmonary TB (SNPT) often gets missed from the diagnosis of consideration, though it accounts 30-65% of total PTB cases, due to deficient or inaccessible molecular diagnostic modalities. METHODS: The cross-sectional study enrolled 360 patients with clinical-radiological suspicion of SNPT in Tribhuvan University Teaching Hospital (TUTH). The patient selection was done as per the algorithm of Nepal's National Tuberculosis Program (NTP) for Xpert MTB/RIF testing. Participants' demographic and clinical information were collected using a pre

Identification of Potential Exosomal miRNA-mRNA Regulatory Network Relevant to Tuberculous-Associated Lung Cancer.

Background/Objectives: Tuberculosis is a risk factor for lung cancer. Previous studies have demonstrated that miRNAs derived from tuberculous pleural effusion (TPE) exosomes are involved in lung cancer progression. This study aimed to identify potential miRNA-mRNA regulatory networks associated with TPE-derived exosome exposure in the context of lung cancer. Methods: Exploratory differential expression analysis was performed using RNA-seq data from TPE-derived exosomes to select candidate miRNAs showing differential expression patterns. In an in vivo mouse xenograft tumor model, TPE-derived ex

Accuracy of extended point-of-care lung ultrasound (EPLUS) for aetiological differentiation of lower respiratory tract infections: a prospective cohort study from India.

BACKGROUND: Infectious respiratory diseases significantly cause morbidity and mortality worldwide, particularly in low- and middle-income countries (LMICs) with limited diagnostic resources. This study explored the utility of lung ultrasound (LUS) paired with extra-pulmonary point-of-care ultrasound (POCUS) for differentiating infectious aetiologies in lower respiratory tract infections (LRTI). METHODS: This prospective cohort study was conducted at a tertiary care centre in India. We recruited consenting adults with suspected LRTI who underwent extended point-of-care lung ultrasound (EPLUS).

Tuberculosis Diagnostics and Localization in Chest X-Rays via Deep Learning Models.

For decades, tuberculosis (TB), a potentially serious infectious lung disease, continues to be a leading cause of worldwide death. Proven to be conveniently efficient and cost-effective, chest X-ray (CXR) has become the preliminary medical imaging tool for detecting TB. Arguably, the quality of TB diagnosis will improve vastly with automated CXRs for TB detection and the localization of suspected areas, which may manifest TB. The current line of research aims to develop an efficient computer-aided detection system that will support doctors (and radiologists) to become well-informed when making
