Caloric tests on platform: "paradoxical responsiveness".

This paper deals with the abnormal responsiveness of the paradoxical stabilizing type (SR) occurring after caloric tests (CALT) performed on a passive force platform. The normal response after CALT (cold/warm, monaurally performed) consists of transient ataxia and body sway towards the side of the nystagmic slow phase induced by a similar CALT. Quantitatively the vestibular responsiveness to CALT is defined as the quotient of lengths QL calculated with measures of recordings after CALT (cold and warm responses averaged) versus recordings at rest. A normal vestibular responsiveness is defined a

Stability of the head: studies in normal subjects and in patients with labyrinthine disease, head tremor, and dystonia.

The dynamics of postural control of the head were investigated in normal human subjects and patients with neurological disease. The technique adopted was to measure the head movements provoked by passive, unpredictable oscillations of the trunk in the frequency range 0-6-7 Hz when subjects were required (a) to try to stabilise their head "in space" and (b) to try to make their heads move "en bloc" with the trunk. Head movement responses were characterised by the gain and phase with respect to trunk movement (transfer function) and degree of linear relationship between head and trunk (coherence

Bilaterality of Meniere's disease.

The audiometric configurations of a randomly selected group of 360 patients with clinical Meniere's disease were analysed in conjunction with their clinical manifestations. Although 78.6% of the patients had an abnormal pure-tone audiogram in the opposite ear from that which was initially diagnosed as Meniere's disease, based on the entire clinical picture, the disease was found to be definitely bilateral in 32% of the patients. A peaking audiogram type was found to be most common, occurring in about half of the involved ears. In approximately half of those with bilateral disease the second ea

The spectrum of audiologic and vestibular findings in patients with Meniere's disease.

A retrospective study of 55 patients with Meniere's disease suggested a subgrouping based on presentation of auditory symptoms. Patients were defined as "definite" (documented fluctuating hearing loss and episodic vertigo), "probable" (history of fluctuating hearing loss and episodic vertigo) and "possible" (history of fluctuating tinnitus and episodic vertigo). An in-depth discussion of the natural history, audiometric and vestibular findings is presented.

[Human visual functions during mild vestibular exposures].

Human experiments have shown that after mild stimulation of semi-circular receptors, which induces no motion sickness symptoms, the time of recovery of the central vision acuity shortens and after stimulation of otolith receptors tends to increase. Thresholds of color discrimination as applied to the red, green and violet colours as well as the course of back adaptation after mild stimulation of otolith receptors remain unchanged. The time of appearance of the Purkinje phenomenon after stimulation of receptors of semi-circular canals often remains unaltered and sometimes increases. It is assum

Comparison of EOG and VOG obtained eye movements during horizontal head impulse testing.

INTRODUCTION: Video head impulse testing is frequently used to evaluate the vestibular function. During this test, eye movement responses are recorded with video-oculography (VOG). However, the use of VOG can sometimes be challenging, especially due to pupil detection problems (e.g., blinking, droopy eyelids, etc.). Therefore, this study investigated whether electro-oculography (EOG), a technique that does not depend on pupil tracking but on the orientation of the corneoretinal potential, might be an alternative to VOG for quantifying eye movement responses during head impulse testing. A Tool to Quantify the Functional Impact of Oscillopsia.

BACKGROUND: Individuals with bilateral vestibular hypofunction (BVH) often report symptoms of oscillopsia during walking. Existing assessments of oscillopsia are limited to descriptions of severity and symptom frequency, neither of which provides a description of functional limitations attributed to oscillopsia. A novel questionnaire, the Oscillopsia Functional Impact scale (OFI) was developed to describe the impact of oscillopsia on daily life activities. Questions on the OFI ask how often individuals are able to execute specific activities considered to depend on gaze stability in an effort

A Study of Dizziness or Vertigo Cases Associated with Inflammatory Bowel Disease (Crohn's Disease and Ulcerative Colitis) in a Vertigo Outpatient Clinic.

Background/Objectives: Dizziness and vertigo are reported in about half of patients with inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC). Orthostatic dysregulation (OD) is recognized as one of the comorbidities that causes dizziness or vertigo with IBD. Our hospital is affiliated with the Inflammatory Bowel Disease Center, which specializes in diagnosing and treating IBD, so cases with dizziness or vertigo symptoms associated with IBD are sometimes referred to our department, a type of department which is rare in other facilities. The objective of t

Anisotropy of Human Horizontal and Vertical Navigation in Real Space: Behavioral and PET Correlates.

Spatial orientation was tested during a horizontal and vertical real navigation task in humans. Video tracking of eye movements was used to analyse the behavioral strategy and combined with simultaneous measurements of brain activation and metabolism ([18F]-FDG-PET). Spatial navigation performance was significantly better during horizontal navigation. Horizontal navigation was predominantly visually and landmark-guided. PET measurements indicated that glucose metabolism increased in the right hippocampus, bilateral retrosplenial cortex, and pontine tegmentum during horizontal navigation. In co

Functional vision head impulse testing (fvHIT) in children: Feasibility and normative values.

ObjectivesCurrent pediatric vestibular care mainly focuses on motor development and postural control whereas the consequences of impaired gaze stabilization are less frequently addressed. The Functional Vision Head Impulse Test (fvHIT) offers a method to assess functional gaze stabilization during rapid head movements. This study aimed to examine the feasibility of the fvHIT in children and establish normative values.MethodsNinety-four healthy children (4-12 years) underwent fvHIT testing. Feasibility, the percentage of correctly identified optotypes in total scores and per acceleration band w

Therapeutic Hybrid Intelligence with Neural and Knowledge-based Expert Reasoning for SRS (THINKERS): a mixture-of-experts AI model for vestibular schwannoma.

BACKGROUND: Outcome prediction after Gamma Knife radiosurgery (GKRS) for vestibular schwannoma remains largely guided by tumor size, Koos grade, baseline symptoms, cochlear dose constraints, and institutional experience rather than individualized estimates of tumor progression and functional outcomes. We developed THINKERS-VS, a mixture-of-experts (MoE) artificial intelligence framework for progression and symptom-transition prediction after GKRS. METHODS: We performed a retrospective single-center study of vestibular schwannomas treated with GKRS. Variables available before the treatment wer
