Discontinuation reactions to alprazolam in panic disorder.

Panic disorder is a chronic illness with only some degree of spontaneous recovery. It is not surprising therefore that discontinuation of an effective medical treatment may be followed by relapse. Therefore the timing and methodology of discontinuing that treatment are now recognized as essential facets of optimal clinical management. In addition to relapse, rebound and the withdrawal syndrome have been reported with many psychotropic agents, particularly with the benzodiazepines. This paper discusses data from three discontinuation studies with alprazolam i.e. the Phase I Cross-National Colla

Postpartum onset of panic disorder: a coincidental event?

BACKGROUND: This study investigated whether the onset of panic for the first time during the postpartum period represented a coincidental occurrence. METHOD: Sixty-four childbearing women diagnosed with panic disorder who had been treated at the Yale Anxiety Research Clinic were interviewed. Postpartum panic was defined as the first panic occurring within 12 weeks of the woman's first childbirth. RESULTS: Seven women (10.9%) met criteria for postpartum onset, which is significantly greater than the expected age-corrected percentage of 0.92%, Z = 2.29, p < .02. CONCLUSION: The onset of panic

Biological correlates of lactate sensitivity in panic disorder.

In this pilot study, 9/9 patients with panic disorder experienced a lactate-induced panic attack as compared with 0/9 controls. Baseline measurements were significant for higher anxiety self-ratings, higher heart and respiratory rates, elevated potassium, and lower lymphocyte 3H-dihydroalprenolol (DHA) binding in the patient group. Spielberger State anxiety scores correlated with baseline mean heart rate, and Spielberger Trait anxiety scores with lymphocyte DHA binding. The lactate infusion was not found to differentially affect any physiological or biochemical measures in the two groups, thou

Reliability of anxiety assessment. I. Diagnostic agreement.

Test-retest reliability of lifetime anxiety disorder diagnoses was determined using the Schedule for Affective Disorders and Schizophrenia-Lifetime Anxiety version. The subjects were 104 patients at an anxiety research clinic. Reliability ranged from good to excellent (kappa = +.60 to +.90) for generalized anxiety, social phobic, panic, agoraphobic, and obsessive-compulsive disorders. Simple phobia showed poor agreement. Current episodes showed better agreement than past episodes, particularly for social phobia and obsessive-compulsive disorder. Major sources of disagreement (variance in subje

The cerebellar-vestibular predisposition to anxiety disorders.

To test for a cerebellar-vestibular (CV) predisposition to anxiety disorder, 402 consecutively referred subjects with varying anxiety symptoms were separated into eight DSM-III--R diagnostic categories and evaluated for CV dysfunction, using neurological and electronystagmographic (ENG) examinations. Of the total sample, 94% evidenced CV-dysfunction on the basis of two or more abnormal neurological or ENG parameters per subject. All DSM-III--R diagnostic anxiety-disorder categories contained a high percentage of abnormal neurological and ENG parameters, regardless of the size of the subsample.

Mitral valve prolapse and thyroid abnormalities in patients with panic attacks.

Panic disorder has been associated with mitral valve prolapse and thyroid abnormalities. Mitral valve prolapse has also been associated with thyroid abnormalities. The authors studied a consecutive series of 65 patients self-referred for evaluation of panic attacks, who were examined for cardiac and thyroid abnormalities. Fifty percent of the patients had mitral valve prolapse according to both cardiac auscultation and echocardiography. Twenty-six percent of the women had some thyroid abnormality; 17% had thyroid microsomal antibodies. There was no apparent relationship between mitral valve pr

The spectrum of anxiety disorders in family practice.

The spectrum ranges from fearful concerns about illness to clinical anxiety disorders, such as phobias, panic attacks, generalized anxiety, post-traumatic stress disorder and adjustment disorder. Anxiety and fear are often associated with hospitalization, heart disease, insomnia, and somatopsychic syndromes. Determining the cause of the anxiety may help the physician plan the treatment approach. In selected cases, benzodiazepines are useful adjuncts to therapy.

Relationship between separation anxiety and panic and agoraphobic disorders.

The paper addresses several issues pertinent to the characteristics and treatment of separation anxiety in children and adults. The rationale for separating separation anxiety from other childhood anxiety conditions rests on clinical observations of its relationship to panic disorder and its response to drug treatment. The psychopharmacological treatment of separation anxiety is reviewed, with special emphasis on the use of tricyclic antidepressants. The literature relevant to a possible association between separation anxiety and adult panic disorder is presented. The prevalence of separation

Managing desperate emotional behaviour with hypnosis.

When no other psychotherapy or pharmacotherapy could prevent debilitation or institutionalization consequent to longstanding frequent repeated maladaptive behaviour, hypnosis has been observed to be effective in suitable patients. A documentation, three cases of such chronic severe behaviour which have been encountered by the author are described; each had been regarded as hopelessly unmanageable by the primary physician at the time of referral for hypnotherapy. Relevant to their suitability for hypnosis, all three patients were able to understand normal speech with difficulty, were able to co

Incongruence Between Implicit Attachment Schemes and Unconscious Attachment Representations.

Assessments based on reaction time and language-based interviews postulate that unconscious attachment processes be measured. Nevertheless, a possible empirical equivalence of these two approaches has not yet been investigated. To fill this void, the Adult Attachment Interview and the Implicit Association Test were implemented with a group of patients with panic disorder (n = 157, mean age = 29, SD = 2.47) based on the Structured Clinical Interview for DSM-IV, axis I and II disorders and a group of healthy individuals (n = 138). In total, the securely attached individuals showed significantly

Social anxiety and emotional suppression: the mediating role of beliefs.

There is mounting evidence to suggest that social anxiety is associated with the suppression of emotional expression. The current study examined self-reported emotional suppression and beliefs about expressing emotions among undergraduate students (n=95). Socially anxious undergraduates reported greater use of emotional suppression compared to their non-socially anxious peers. They also reported greater ambivalence about emotional expression, more difficulties in emotional responding, more fears of emotional experiences, and more negative beliefs about emotional expression. Believing that emot

Reactivity to imagery and nocturnal panic attacks.

We assessed psychophysiological responses to imagery of traumatic events, panic attacks, and loss of vigilance (i.e., being hypnotized) in participants diagnosed with panic disorder. Participants were either currently experiencing nocturnal panic or daytime panics only. It was hypothesized that nocturnal panickers would exhibit heightened responsivity to imagery of their worst panic and worst trauma, as well as being hypnotized, compared to day panickers. Using established imagery procedures, heart rate, skin conductance, and frontalis electromyogram were recorded as participants imagined each

[Ambulatory treatment of anxiety disorder. Behavior therapy and drug therapy approaches].

Phobic disorders and other anxiety disorders belong to the most frequent psychiatric illnesses. Since they can be overlapped by other mental illnesses, they are often overlooked in practice. For example, comorbidity between anxiety and depression is very high. Also, personality disorders and a person's biography can lay the ground to anxiety. A typical complication of anxiety disorders is dependence of substances, predominantly of tranquilizers and alcohol. By means of a clinical case example, the topic of anxiety disorders is discussed practically and the treatment concept is detailed. Alread

Rate of improvement during cognitive-behavioral group treatment for panic disorder.

This study examined the rate of symptom improvement in patients receiving cognitive-behavioral group treatment for panic disorder in an outpatient clinic setting. Treatment was a standard program of 12 sessions that emphasized information, interoceptive and situational exposure, and cognitive restructuring, but also included diaphragmatic breathing and relaxation training as elements of treatment. Subjects were 37 patients selected from sequential admissions into an outpatient treatment program; all data were derived from ongoing quality assurance measures that are a standard part of clinical

Comorbidity in social anxiety disorder: impact on disease burden and management.

Social anxiety disorder is a chronic, disabling disorder in which patients suffer with considerable morbidity that, more often than not, precedes the development of other psychiatric disorders. The development of comorbidity adds to the severity of the disorder, increases the risk of suicide attempts, and increases the overall burden of the disease for both the patient (greater disability) and the health care service (greater use of medical services). Comorbidity in social anxiety disorder may result in one good thing: increased recognition and treatment, because in the absence of comorbidity
