Clinical Response of Paracetamol versus Indomethacin in the Medical Treatment of Patent Ductus Arteriosus in Newborn.

Patent Ductus Arteriosus (PDA) is a common acyanotic congenital heart disease in newborn which has a lot of serious consequences if left untreated. By giving proper medication timely, we can limit its complications. Already established drug 'Indomethacin' has demonstrated good effects but with many contraindications and potential adverse effects. Hence, the search for alternative drug for PDA is now a need of time. So, the objective of the study is to compare the clinical response of paracetamol and indomethacin in the medical treatment of PDA in newborn. This randomized controlled trial was c

Mortality risk factors in actively resuscitated 22-week preterm infants: a case-control study focusing on NEC and maternal hospitalization.

BACKGROUND: Infants born at 22 weeks’ gestation represent one of the most vulnerable populations in neonatal care. Despite increasing survival rates with active resuscitation, variability in outcomes remains significant. This study aimed to identify risk factors associated with in-hospital mortality in actively resuscitated infants born at 22 weeks’ gestation. METHODS: A retrospective case-control study was conducted at Kagoshima City Hospital between January 2006 and December 2023. Fifty-seven live-born infants at 22 weeks’ gestation who received active resuscitation were included. Clinical c

A Prediction Nomogram for Moderate-to-Severe Bronchopulmonary Dysplasia in Chinese Very Preterm Infants With Respiratory Distress: A Multi‑Center Prospective Study.

Moderate-to-severe bronchopulmonary dysplasia (msBPD) is a prevalent and severe condition in very preterm infants, contributing to long-term respiratory issues. Developing a predictive model specific to the Chinese population could support early intervention. This research focused on determining risk factors and developing a predictive model for msBPD in very preterm infants with respiratory distress, following the 2018 National Institute of Child Health and Human Development criteria. A prospective multicenter study was conducted across 26 tertiary neonatal intensive care units (NICUs) in Chi

Role of Human Milk Microbiota in Infant Neurodevelopment: Mechanisms and Clinical Implications.

BACKGROUND: Human milk (HM) is recognized as an ideal source of nutrition for newborns; as a result, its multiple bioactive molecules can support the growth of healthy newborns and reduce the risk of mortality and diseases such as asthma, respiratory infections, diabetes (type 1 and 2), and gastrointestinal disorders such as ulcerative colitis and Crohn's disease. Furthermore, it can reduce the severity of necrotizing enterocolitis (NEC) in preterm infants. Moreover, human milk oligosaccharides (HMOs) present in breast milk show an immunomodulatory, prebiotic, and neurodevelopmental effect tha

Prevention of Inflammatory Disorders in the Preterm Neonate: An Update with a Special Focus on Bronchopulmonary Dysplasia.

BACKGROUND: The rates of major neonatal morbidities, such as bronchopulmonary dysplasia, necrotizing enterocolitis, preterm white matter disease, and retinopathy of prematurity, remain high among surviving preterm infants. Exposure to inflammatory stimuli and the subsequent host innate immune response contribute to the risk of developing these complications of prematurity. Notably, the burden of inflammation and associated neonatal morbidity is inversely related to gestational age - leaving primarily but not exclusively the tiniest babies at highest risk. SUMMARY: Avoidance, prevention, and t

A rare case of extensive pneumatosis cystoides intestinalis with intestinal malrotation: Case report.

Intramural bowel gas (Pneumatosis intestinalis) refers to the radiological or clinical evidence of gas within the wall of the bowel lumen. While intramural gas could be secondary to life-threatening pathologies such as mesenteric ischemia in adults and necrotizing enterocolitis in neonates, it could also occur as a rare benign sub-type called Pneumatosis cystoides intestinalis, which is characterized by multiple gas-filled cysts in the submucosa and/or subserosal of the gastrointestinal tract. Distinguishing between life-threatening Pneumatosis intestinalis and its benign subtypes requires car

Gene expression in the intestine of newborn piglets after hypoxia-reoxygenation.

BACKGROUND: In preterm infants, intestinal hypoxia may partly contribute to the pathophysiology of necrotizing enterocolitis through changes in gene expression. Splanchnic hypoxia can be detected with monitoring of regional splanchnic oxygen saturation (rsSO2). Using a piglet model of asphyxia, we aimed to correlate changes in rsSO2 to gene expression. METHODS: Forty-two newborn piglets were randomized to control or intervention groups. Intervention groups were subjected to hypoxia until they were acidotic and hypotensive. Next, they were reoxygenated for 30 min according to randomization, i.

What is Known About Cow's Milk Protein Allergy in Preterm Infants?

Background: Cow's milk protein allergy (CMPA) is well described in term infants, as opposed to preterm infants. In preterm infants, CMPA shares many gastrointestinal symptoms with necrotizing enterocolitis (NEC). Objectives: To evaluate the presentation of CMPA in preterm infants and to investigate the different diagnostic and therapeutic options. Materials and Methods: We searched for the relevant literature using the medical databases PubMed, Web of Science, and the Cochrane Library. We performed a post hoc analysis on the 25 case reports included in this study. Results: Literature was scarc

Declining incidence of cerebral palsy in South Korea.

Presuming that the incidence of cerebral palsy (CP) in Korea is decreasing due to medical advances, we analyzed the trends and risk factors of CP in changing circumstances. We identified all women who delivered a singleton between 2007 and 2015 using the Korea National Health Insurance (KNHI). Information on pregnancy and birth was obtained by linking the KNHI claims database and data from the national health-screening program for infants and children. The 4-years incidence of CP decreased significantly from 4.77 to 2.52 per 1000 babies during the study period. The multivariate analysis reveal

Nonsynchronized nasal intermittent positive pressure ventilation versus continuous positive airway pressure as a primary mode of respiratory support in neonates (26-40 weeks) admitted in a tertiary care center: A randomized controlled trial.

INTRODUCTION: Continuous positive airway pressure (CPAP) is a standard respiratory care for neonates for last few decades but it too has a high failure rate. Nasal intermittent positive pressure ventilation (NIPPV) is proven to be superior to CPAP in maintaining higher mean airway pressure in neonates with Respiratory Distress Syndrome. The main objective of this study was to compare failure within 72 h of initiation of primary respiratory support between nonsynchronized NIPPV and CPAP in all causes of respiratory distress in newborn infants. Patterns of Infections among Extremely Preterm Infants.

Infections remain a leading cause of neonatal death, especially among the extremely preterm infants. To evaluate the incidence, pathogenesis, and in-hospital outcomes associated with sepsis among hospitalized extremely preterm infants born at 24-0/7 to 27-6/7 weeks of gestation, we designed a post hoc analysis of data collected prospectively during the Preterm Epo Neuroprotection (PENUT) Trial, NCT #01378273. We analyzed culture positive infection data, as well as type and duration of antibiotic course and described their association with in-hospital morbidities and mortality. Of 936 included

Anti-inflammatory actions of acetate, propionate, and butyrate in fetal mouse jejunum cultures ex vivo and immature small intestinal cells in vitro.

Necrotizing enterocolitis (NEC) is an intestinal disease that frequently occurs in premature infants. Presently, there is no effective therapy for NEC. Therefore, the key to reduce the incidence rate of NEC is to take effective intervention measures as early as possible. Short-chain fatty acids (SCFAs) (acetate, propionate, and butyrate), the principal terminal products of enterobacteria fermentation, play anti-inflammatory actions in mature intestinal cells. However, few studies focus on their roles in immature intestine. Here, we evaluated the anti-inflammatory actions of SCFAs ex vivo with
