Histoplasmosis in India: a systematic review of reported cases.

BACKGROUND: Histoplasmosis is a systemic fungal infection caused by Histoplasma capsulatum and is increasingly recognised in India, but its epidemiology, clinical spectrum, diagnosis, treatment, and outcomes remain poorly characterised. We conducted a systematic review of reported cases from India. METHODS: A PROSPERO-registered review (CRD42024556630) was conducted using PubMed, Embase, and Web of Science for studies published up to February 2025, without language restrictions. Two reviewers independently screened records and extracted individual patient-level data. Organ involvement was def…

Emerging Human Health Problems Caused by Pathogenic and Immuno-activating Fungi.

In recent years, several types of novel fungal health problems have been emerging in parallel. Antifungal-drug-resistant opportunistic patho … ge extension in Coccidioides immitis. Decimation of some North American bat species associated with Histoplasma hot spots has not had a perceptible effect on this fungus so far. Historical reading suggests that it may have survived a previous loss of conditioned habitat when the passenger pigeon became extinct. Emergence of Emergomyces and new Blastomyces spe …

Hemophagocytic Syndrome in a Patient with HIV and Histoplasmosis: A not so Rare Correlation.

Hemophagocytic lymphohistiocytosis (HLH) is a disorder that occurs due to unsuitable monocyte activation in a variety of infections. In huma … postmortem examination, histopathology revealed countless rounded fungal structures compatible with Histoplasma sp., which were observed in the peripancreatic lymph node, liver, and spleen, in addition to hemophagocytosis in the splenic parenchyma. We thus conclude that when the patient met criteria for HLH, such as fever, hepatosplenomegaly, hiperferritinemi …

Burden of HIV-associated histoplasmosis compared with tuberculosis in Latin America: a modelling study.

BACKGROUND: Fungal infections remain a major contributor to the opportunistic infections that affect people living with HIV. Among them, histoplasmosis is considered neglected, often being misdiagnosed as tuberculosis, and is responsible for numerous deaths in Latin America. The objective of this study was to estimate the burden of HIV-associated histoplasmosis compared with tuberculosis in Latin American countries. METHODS: For this modelling study, we estimated prevalence of previous exposure to Histoplasma capsulatum, HIV-associated histoplasmosis annual incidence, and number of deaths in …

Fibrosing mediastinitis: clinical presentation, therapeutic outcomes, and adaptive immune response.

Fibrosing mediastinitis (FM) is a rare disorder characterized by the invasive proliferation of fibrous tissue within the mediastinum. FM frequently results in the compression of vital mediastinal structures and has been associated with substantial morbidity and mortality. Its pathogenesis remains unknown. However, in North America most cases are thought to represent an immune-mediated hypersensitivity response to Histoplasma capsulatum infection. To characterize the clinical disease spectrum, natural disease progression, responses to therapy, and overall survival, we retrospectively analyzed a…

Genomic DNA microarray comparison of gene expression patterns in Paracoccidioides brasiliensis mycelia and yeasts in vitro.

Paracoccidioides brasiliensis is a thermally dimorphic fungus, and causes the most prevalent systemic mycosis in Latin America. Infection is … s and toxin and sugar transport showed differential expression. Sequenced clones were compared with Histoplasma capsulatum and Coccidioides posadasii genome sequences to assess potentially common pathways across species, such as sulfur and lipid metabolism, amino acid transporters, transcription factors and genes possibly related to virulence. We also analyse …

Immunoregulation in experimental disseminated histoplasmosis: flow microfluorometry (FMF) studies of the Thy and Lyt phenotypes of T lymphocytes from infected mice.

Previous studies have shown that mice infected i.v. with 6 X 10(5) yeast phase Histoplasma capsulatum (Hc) develop suppressed immune responses during weeks 1 to 4 of infection but that by weeks 8 to 12 of infection these responses return to normal. In this study total and differential cell counts showed that as early as the third day of infection there was a marked reduction in the number of lymphocytes recovered from the peripheral blood, bone marrow, and thymus of infected animals. Concomitantly, there was an increase in the number of splenic lymphocytes. By day 28 both the total and differe…

A 30-year delayed presentation of disseminated histoplasmosis in a heart transplant recipient: diagnostic challenges in a non-endemic area.

A 70-year-old man with history of heart transplant performed in 1986, presented with altered mental status. CT scan of brain showed ring-enhancing lesions, raising suspicion for metastatic malignancy. Work-up revealed bilateral adrenal masses, biopsy showed granulomatous changes consistent with histoplasmosis. The possibility of histoplasmosis was less likely as the patient had no prior history of symptomatic disease and had lived in the endemic area 30 years prior to presentation. Brain biopsy confirmed central nervous system involvement. Amphotericin B was initiated for disseminated disease

Investigation of the cause of death in a gene-therapy trial.

We present a case of disseminated histoplasmosis, complicated by retroperitoneal bleeding and leading to death, in a patient who was receiving systemic immunosuppressive therapy for rheumatoid arthritis and who was enrolled in a gene-therapy trial. This trial was designed to evaluate intraarticular delivery of a tumor necrosis factor alpha (TNF-alpha) antagonist, through an adeno-associated virus (AAV) type 2 delivery system, for inflammatory arthritis. The patient's receipt of concurrent anti-TNF-alpha therapy and other immunosuppressive therapy while she was living in an area where histoplas

Salvage treatment of histoplasmosis with posaconazole.

Six patients received salvage treatment with posaconazole oral suspension (800 mg/day in divided doses) for severe forms of histoplasmosis. One patient had pulmonary disease and 5 had disseminated disease. Previous antifungal therapy consisted of amphotericin B, itraconazole, fluconazole, or voriconazole. Posaconazole treatment duration for individual patients ranged from 6 weeks to 34 weeks. All patients had successful clinical outcomes with significant clinical improvements noted during the first month of therapy. Although the number of patients evaluated in this case series is small, the fi

Deep mycotic infections in cats.

A review of deep mycotic infections assessed incidence, signalment, clinical presentation, and outcome in 571 feline cases. Pertinent findings included a predisposition in young, male cats for Sporothrix schenckii and Blastomyces dermatitidis. There was an increased incidence of feline leukemia virus infection in cases with histoplasmosis and of feline panleukopenia virus infection in cases with either aspergillosis or mucoralosis. Few other predisposing conditions were identified. Cryptococcosis, coccidioidomycosis, and sporotrichosis had better prognoses for recovery. Blastomycosis, histopla

Histoplasmosis. A review, and account of three patients diagnosed in Great Britain.

Active histoplasmosis is rarely diagnosed in Great Britain. We present the illness records of three patients with disseminated histoplasmosis. In two patients the infection was discovered at necropsy and resulted in caseous destruction of the adrenal glands, and in one of these there was also fungal invasion of the liver and wall of the aorta. The third patient had Addison's disease and also a chronic tongue ulcer from which the fungus was cultured by biopsy. The eight other patients with disseminated histoplasmosis and the six with active localized disease previously reported in Great Britain

Histoplasmosis in Europe: report on an epidemiological survey from the European Confederation of Medical Mycology Working Group.

The purpose of this survey was to systematically collect data on individuals with histoplasmosis in Europe over a 5-year period (from January 1995 to December 1999). This included information on where and how the infection was acquired, the patient's risk factors, the causative organism, how the infection was diagnosed and what therapy the patients received. Data were sent on a standardized survey form via a national convenor to the coordinator. During the survey, 118 cases were reported, with 62 patients having disseminated disease, 31 acute pulmonary infection, chronic pulmonary infection in
