On the basis of a retrospective study concerning 63 cases of normal pressure hydrocephalus, the author examines the characteristics of patients with a traumatic aetiology who were not ameliorated by a ventricular derivation. Four of these patients suffering from a post-traumatic dementia are described. Reviewing the literature, several topics concerning post-traumatic dementia are considered: incidence, time delay before diagnosis, variation of pictures, aetiopathogenic explanations, medico-legal problems encountered with Alzheimer's of Pick's diseases. Finally the differential diagnostic char

This paper discusses the conditions which may be associated with diffuse periventricular hypodensity on CT. Of these conditions, subacute arteriosclerotic encephalopathy (Binswanger's disease) is a common differential diagnostic consideration. This condition results from ischemic demyelination with loss of axons and astrogliosis. CT usually shows diffuse bilateral confluent hypodensities in the cerebral white matter. This is most prominent and seen earliest in the periventricular region and may extend more diffusely into the centrum semiovale. It is important to differentiate this condition fr

Arachnoid intracranial cysts are benign lesions which until recently have been considered to be quite uncommon. They are benign congenital lesions usually discovered at an early age. We have recently treated two patients with advanced age and arachnoid cysts, unusual due to their location and extent. Both of them presented clinically with an organic dementia syndrome with some similarities to that seen in normal pressure hydrocephalus. Both recovered fully after surgery. We review the literature and try to emphasize the clinical differences between arachnoid and the more rare neuroepithelial c

A case of spontaneous thrombosis of a previously ruptured anterior communicating artery aneurysm is reported. The patient was referred by another hospital to our service 4 months after the bleeding episode. He had developed a symptomatic normal pressure hydrocephalus, successfully treated by ventriculoperitoneal shunt. Follow-up angiographies, the first performed 4 months and the second 1 year after the bleeding, did not show the aneurysm. Furthermore, the parent artery was not occluded. The literature relative to the subject is reviewed and the possible mechanisms of spontaneous thrombosis ar

Seven children with rapid enlargement of the head and other clinical features resembling hydrocephalus are described. All children remained with relatively large heads, developed slowly and all but one had various degrees of mental and motor handicaps. The EEG was disturbed in all cases; two children developed seizures. Increased intracranial pressure was present. In all patients the subarachnoid space was markedly widened while the ventricular system was normal or minimally dilated. The characteristics of this syndrome, the features distinguishing it from similar conditions as well as its pos

Sixty consecutive patients with progressive intellectual deterioration were evaluated by a specific protocol. Eighteen (30%) were found to have an underlying disease potentially reversible by medical or surgical therapy. Sixteen patients had a specific diagnosable but untreatable disease as a cause of dementia. The laboratory and radiologic tests of the protocol that uncovered treatable illnesses were the radioisotope brain scan (two cases of bilateral subdural hematoma), the pneumoencephalogram (seven patients with normal-pressure hydrocephalus), thyroid function screen, and liver function st

The authors report a case of intratumoral hemorrhage in a pineal region choriocarcinoma during neuroendoscopic third ventriculostomy. A 12-year-old boy who presented with headache and vomiting had precocious puberty. Neuroimagings revealed a pineal region tumor with obstructive hydrocephalus and his serum HCG level was 4,280 mIU/ml. He was diagnosed as having choriocarcinoma and underwent neuroendoscopic third ventriculostomy for obstructive hydrocephalus. There were many tumor vessels observed on the tumor surface, some of which bled subcapsularly. Postoperative CT scan showed the tumor incre

INTRODUCTION: Craniopharyngiomas are dysontogenic tumors with benign histology but aggressive behavior. The surgical challenges posed by the tumor are well recognized. Neuroendoscopy has recently contributed to its surgical management. This study focuses on our experience in managing craniopharyngiomas in recent years, highlighting the role of combined endoscopic trans-ventricular and endonasal approach.
CASE SERIES: Ninety-two patients have been treated for craniopharyngioma from 2000 to 2016 by the senior author. A total of 125 procedures, microsurgical (58) and endoscopic (67), were undert

Docetaxel constitutes a widely used chemotherapeutic agent as a first-line treatment for several neoplastic diseases. One of the most common side effects induced by this drug is polyneuropathy, which among other symptoms can cause gait disbalance. However, in exceptional cases gait disturbances could be related to docetaxel-induced hydrocephalus, a rare event that up to the present has been overseen throughout the medical literature and should be meticulously differentiated from polyneuropathy, since its clinical features, treatment, and prognosis differ drastically. We present the case of a w

Germinal matrix hemorrhages (GMHs) are typically seen in preterm neonates during the first 4 days of life. The authors encountered 2 children with late-onset GMH subsequent to ventriculoperitoneal (VP) shunt insertion for congenital hydrocephalus. Both children were delivered at full term with normal body weight, although they were compromised with the preceding hypoxic events prior to shunt insertion. The first case involved a female infant with severe craniofacial deformities. Because of aspiration pneumonia related to the comorbid upper airway stenosis, endotracheal intubation with intermit

BACKGROUND: Chronic adult hydrocephalus (CAH) is believed to have an incidence rate between 0.2 and 5.5 cases/100,000 individuals. Although the underlying mechanisms remain largely unknown, several potential contributing factors have been identified, such as meningitis, cranial injuries, intradural tumors, malformations of the posterior fossa, neurological conditions, and complications arising from intracranial surgeries. These factors may lead to a delayed onset of symptoms associated with hydrocephalus, which include cognitive decline, mobility difficulties, and urinary incontinence.

Idiopathic stenosis of the foramina of Magendie and Luschka is a rare cause of obstructive hydrocephalus involving the four ventricles. Like other causes of noncommunicating hydrocephalus, it can be treated with endoscopic third ventriculostomy (ETV). Three patients who were 21, 53, and 68 years of age presented with either headaches (isolated or associated with raised intracranial pressure) or vertigo, or a combination of gait disorders, sphincter disorders, and disorders of higher functions. In each case, magnetic resonance (MR) imaging demonstrated hydrocephalus involving the four ventricle

Hydrocephalus is a dynamic neurosurgical entity, with cerebrospinal fluid (CSF) diversion methods continually evolving to strike a balance between long-term efficacy and the risk of complications. We describe a 19-year-old man who was treated with endoscopic third ventriculostomy (ETV) for congenital aqueductal stenosis, with initial relief of symptoms. Two years later, he experienced recurring headaches and papilledema, but neuroimaging showed stable ventricles and patency of the ETV stoma. Magnetic resonance venography (MRV) ruled out venous sinus thrombosis, while lumbar CSF manometry revea

Transforming growth factor-beta 1 (TGF-beta 1) is a multi-functional polypeptide, which controls proliferation, differentiation of various cells, and regulates synthesis of extracellular matrix proteins. We injected human recombinant TGF-beta 1 into the subarachnoid space of 10-day-old C57BL/6 mice in order to study the role of TGF-beta 1, which is known to be released from platelets into the cerebrospinal fluid following subarachnoid hemorrhage. The ventricular system became dilated within 3 weeks following the injection and the body weights of injected mice stopped increasing 6 weeks after i

OBJECTIVE AND IMPORTANCE: We present a very unusual case of diffuse spread of Wegener's granulomatosis causing hydrocephalus.
CLINICAL PRESENTATION: A 53-year-old man presented in 1985 with bilateral middle ear infections requiring myringotomies. During the next 18 months, he went on to develop a left Bell's palsy. The patient then began to develop recurrent occipital headaches along with left sixth and seventh nerve palsies and a green nasal discharge requiring hospitalization. Workup included magnetic resonance imaging showing pronounced enhancement of the tentorium and meninges in the occi

Papillary glioneuronal tumors are a rare and typically benign entity with pathological and radiographic complexity. Presentation can mimic other neoplasms, making diagnosis more challenging. The literature to date describes the clinical understanding, diagnostic, therapeutic, and prognostic characteristics of this limited number of patients. In this article, we report an unusual case of a glioneuronal tumor with multifocal presentation, normal pressure hydrocephalus-like symptoms, and large peritumoral parenchymal cysts, which guided the surgical approach. This unusual presentation has not pre

A patient, aged 47 is described in whom changes in hormonal profiles were observed following surgical excision of the pineal. She presented with a clinical syndrome of increased intracranial pressure and postmenopausal symptoms. Computerized tomographic scans showed obstructive hydrocephalus from a cystic lesion in the pineal region. Surgical removal of the cystic mass revealed a benign ependymal cyst and normal pineal tissue. After the operation, she had no nocturnal increase in serum melatonin. The morning basal LH was further elevated and night-time PRL showed little fluctuation from the ba

The EEG in normal aging and in dementia due to a variety of diseases has been described. In dementia due to degenerative diseases, metabolic disorders, normal pressure hydrocephalus, and deficiency states, the diffuseness of the abnormalities observed has been emphasized; whereas with cerebrovascular disease and with intracranial massess, the focal nature of the changes has been stressed. Although a normal EEG does not rule out the diagnosis of dementia especially in its early stages, the EECG can nevertheless be a significant instrument in the identification or organic processes when the diag

A group of gerontopsychiatric patients suffering from mild to severe dementia was studied. Clinical instruments were a psychological assessment system and a battery of tests. Structural changes of the brain were detected by computerized tomography. A number of correlations between clinical and morphological variables was found. The statistical analysis of most of the variables correlating with the CT determinants of the frontal lobe (Huckman-number, width of the frontal interhemispheric fissure) revealed plausibility consistent with the classical frontal lobe syndrome. The relations of the rem

INTRODUCTION: Challenges in diagnosing post-traumatic hydrocephalus (PTH) have created a need for an accurate diagnostic tool. We aim to report CSF dynamics in PTH and atrophy, along with differences before and after cranioplasty.
METHODS: We retrospectively analyzed traumatic brain injury patients with ventriculomegaly who had infusion studies. We divided patients depending on CSF dynamics into two groups: 'likely PTH' (A) and 'likely atrophy' (B). A group of idiopathic normal pressure hydrocephalus shunt-responsive patients was used for comparison (C).
RESULTS: Group A consisted of 36 pati
