Open joint injuries. Classification and treatment.

Experience with wounds involving the major joints of the lower extremity suggests that the extracapsular soft tissues and the intracapsular structures should be considered individually to assess accurately the magnitude of injury and to provide a prognosis. A classification scheme is proposed to characterize the areas and types of injury. Type I open joint injuries are single penetrations without extensive soft-tissue damage, permitting uncomplicated joint and wound closure. Type II injuries are single or multiple penetrations with extensive soft-tissue disruptions (flaps, avulsions, degloving

Sports injuries in the emergency department: an observational study with a gender perspective.

INTRODUCTION: Sports injuries are a significant concern in emergency departments and affect both amateur and professional athletes. With the increase in women's participation in sports, it is crucial to understand sex-specific injury characteristics, as patterns observed in men may not apply to women. This observational, descriptive and retrospective study aims to analyze sex differences in sports injuries to improve diagnosis, treatment, and prevention strategies. Our hypothesis is that sports injury patterns differ between sexes. METHODS: Patients treated for sports injuries between 2020 an

Clinical evaluation of two artificial intelligence algorithms in standard radiography for post-traumatic exploration of peripheral limbs in children.

BACKGROUND: Pediatric traumatology represents a significant portion of emergency department visits, and advances in artificial intelligence (AI) provide promising tools for fracture detection on radiographs. However, few studies have assessed these tools on the same pediatric population. OBJECTIVE: To evaluate the diagnostic performance in a pediatric population of two commercially available AI software programs for the detection of peripheral limb fractures, both alone and in comparison with radiology residents. MATERIALS AND METHODS: Consecutive recruitment of 366 children under 18 referre

A new phenotype of EVEN-PLUS syndrome in a Chinese family and literature review.

BACKGROUND: Epiphyseal, Vertebral, Ear, and Nose (EVEN)-PLUS syndrome is a rare condition characterized by the involvement of the Epiphyses, Vertebrae, Ears, and Nose, plus other associated findings, due to pathogenic variants in the HSPA9 gene. Due to the sparse number of patients, the clinical phenotypic spectrum is not clear. METHODS: We report two patients with pathogenic HSPA9 variants from a Chinese family. Besides the core clinical features of EVEN-PLUS syndrome, the two cases had seizures, developmental delay, and basal ganglia lesions in cerebral MRI. We also reviewed the previously

Long-term follow-up of a patient with autosomal dominant lower extremity-predominant spinal muscular atrophy-2 due to a BICD2 variant.

INTRODUCTION: Bicaudal D homolog 2 (BICD2) is a causative gene of autosomal-dominant lower extremity-predominant spinal muscular atrophy-2 (SMA-LED2). The severity of SMA-LED2 varies widely, ranging from cases in which patients are able to walk to cases in which severe joint contractures lead to respiratory failure. In this study, we report the long-term course of a case of SMA-LED2 in comparison with previous reports. CASE REPORT: The patient was a 19-year-old woman. She had knee and hip dislocations with contractures, femoral fracture, and talipes calcaneovalgus since birth, and was diagnos

Hospital Readmissions After Total Joint Arthroplasty: An Updated Analysis and Implications for Value-Based Care.

BACKGROUND: While risk factors have been published for readmissions following primary total joint arthroplasty, little is known about the etiology of those costly adverse events. In this study, we sought to identify the reasons for 30-day readmission following primary total joint arthroplasty in a contemporary national patient sample. METHODS: The American College of Surgeons National Surgical Quality Improvement Program was queried to identify 367,199 patients who underwent primary total knee (TKA) or hip arthroplasty (THA) between 2011 and 2018. The primary outcomes were the annual rates of

Analysis of Difficulties and Complications Following Implantation of Modular Prostheses in Patients with Lower Limb Bone Tumours.

BACKGROUND: The implantation of a modular prosthesis is the most popular method of reconstruction of bone defects in oncological patients. Along with clear-cut benefits associated with this procedure, there is also an increased risk of complications. Common complications include deep infections, aseptic loosening and mechanical implant damage. This study assessed the risk of complications following the implantation of a resection prosthesis in patients with lower limb bone tumours and to evaluate difficulties encountered during the treatment. MATERIAL AND METHODS: A total of 149 patients with

Kinematics of meniscal- and ACL-transected mouse knees during controlled tibial compressive loading captured using roentgen stereophotogrammetry.

Pre-clinical studies of post-traumatic OA have examined the pathways that lead to disease after injury by using surgical models such as the destabilization of the medial meniscus (DMM) and anterior cruciate ligament transection (ACLT). While the morphological, molecular, and genetic pathways leading to OA have been examined extensively; the effects of these injuries on joint kinematics, and thus disease progression, have yet to be fully characterized. To this end, we sought to understand the kinematics in the DMM and ACLT joints compared to intact joints subjected to controlled tibial compress

[Arthroscopic treatment of tibial plateau fractures].

OBJECTIVE: The aim of arthroscopically assisted treatment of tibial plateau fractures is to achieve minimally invasive reduction and internal fixation of the joint fracture of the tibial plateau. Using the arthroscopic procedure, both the approach morbidity and the control of the articular reduction can be optimized. INDICATIONS: Displaced tibia plateau fractures of AO type A1 and B1/2/3 or Tscherne P2. CONTRAINDICATIONS: Strongly displaced tibial plateau fractures, which require an open surgical approach and stabilization with plate fixation (e.g., AO type C fractures or Moore type 5 fractu

Proposed digital arthroplasty critical study of the preliminary results.

In view of the results of Swanson implants, particularly the long-term course of silicone implants, the authors studied and developed semi-constrained sliding arthroplasty designed for metacarpo-phalangeal and proximal interphalangeal joints. The preliminary study on bones and then on cadaver hands and finally on the knee of the rabbit allowed the design and experimentation of a prototype which led to the creation of a semi-constrained arthroplasty for which the elastic diaphyseal anchoring is ensured without cement and which possesses sliding surfaces composed of a metallic (proximal piece)--

[Treatment of limb deformities by the Ilizarov method].

INTRODUCTION: The numerous possibilities for adapting the Ilizarov apparatus allows the progressive correction of complex angular deviations, for which flat apparatus are sometimes difficult to adapt and this report describes our experience using the Ilizarov apparatus to treat axial limb deformities. MATERIAL AND METHODS: A total of 48 patients (22 girls and 26 boys aged between 2 and 18 years-old) suffering from 58 angular deformities were treated with an Ilizarov device. 40 of the deformities involved bones: 22 tibias, 13 femurs and 6 radius. The remaining 18 deformities involved joints, (

Impact of joint laxity and hypermobility on the musculoskeletal system.

Excessive joint laxity, or hypermobility, is a common finding of clinical importance in the management of musculoskeletal conditions. Hypermobility is common in young patients and in general is associated with an increased incidence of musculoskeletal injury. Hypermobility has been implicated in ankle sprains, anterior cruciate ligament injury, shoulder instability, and osteoarthritis of the hand. Patients with hypermobility and musculoskeletal injuries often seek care for diffuse musculoskeletal pain and injuries with no specific inciting event. Orthopaedic surgeons and other healthcare provi

Trochlear Morphological Parameters Based on Measurements at the Trochlear Groove Entry Plane Demonstrate Excellent Predictive Efficacy for Identifying the High-Grade J-Sign.

OBJECTIVE: The correction of high-grade J-sign is critical for improving clinical outcomes in patients with trochlear dysplasia, but insufficient attention has been given to anatomical abnormalities at the trochlear groove entry plane, the established gold standard for describing trochlear morphology. We aim to compare the diagnostic accuracy and reliability of axial computed tomography (CT) measurements at the trochlear groove entry plane versus the Roman arch plane in predicting the J-sign among patients with patellar instability. METHODS: This retrospective study enrolled 120 knees diagnos

Factors Associated With Pain and Function Before Medial Patellofemoral Ligament Reconstruction.

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is performed to treat recurrent patellar instability. Measurement of joint pain and function at the time of surgery has been demonstrated to be a predictor of the final outcomes in many surgical procedures. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate the relationship between baseline patient characteristics, mental health, and intraoperative findings and patient-reported knee pain and function at the time of MPFL reconstruction. We hypothesized that patient characteristics and associated pathology would be asso

No Growth Disturbance After Trochleoplasty for Recurrent Patellar Dislocation in Adolescents With Open Growth Plates.

BACKGROUND: Trochlear dysplasia is the most important risk factor for patellofemoral instability among adolescents; therefore, trochleoplasty to reshape the trochlear groove is the treatment of choice for patients with high-grade trochlear dysplasia. However, in the presence of open growth plates, there is a potential risk of injury to the distal femoral growth plate and subsequent growth disturbance. As such, most authors do not recommend trochleoplasty for skeletally immature patients. The effect of trochleoplasty on femoral growth when performed before closure of the distal femoral physis r

The ability of medial patellofemoral ligament reconstruction to correct patellar kinematics and contact mechanics in the presence of a lateralized tibial tubercle.

BACKGROUND: Tibial tubercle (TT) transfer and medial patellofemoral ligament (MPFL) reconstruction are used after patellar dislocations. However, there is no objective evidence to guide surgical decision making, such as the ability of MPFL reconstruction to restore normal behavior in the presence of a lateralized TT. HYPOTHESIS: MPFL reconstruction will only restore joint contact mechanics and patellar kinematics for TT-trochlear groove (TG) distances up to an identifiable limit. STUDY DESIGN: Controlled laboratory study. METHODS: Eight fresh-frozen cadaveric knees (mean TT-TG distance, 10.
