Clinical Characteristics and Clinical Course of Body Lateropulsion in 47 Patients with Brainstem Infarctions.

BACKGROUND: In patients with lower lateral medullary infarction (LMI) located under the vestibular nucleus, proprioceptive impairment due to dorsal spinocerebellar tract (DSCT) is considered a pathological condition for body lateropulsion. In patients with brainstem infarction located at or above the level of the vestibular nucleus, other pathways, such as the crossed vestibulothalamic tract (CVTT), are considered responsible. RESEARCH QUESTION: The clinical course of body lateropulsion between each anatomical level of infarction remains unclear. Further, whether body lateropulsion refers to

Laryngeal vibration as a non-invasive neuromodulation therapy for spasmodic dysphonia.

Spasmodic dysphonia (SD) is an incurable focal dystonia of the larynx that impairs speech and communication. Vibro-tactile stimulation (VTS) alters afferent proprioceptive input to sensorimotor cortex that controls speech. This proof-of-concept study examined the effect of laryngeal VTS on speech quality and cortical activity in 13 SD participants who vocalized the vowel /a/ while receiving VTS for 29 minutes. In response to VTS, 9 participants (69%) exhibited a reduction of voice breaks and/or a meaningful increase in smoothed cepstral peak prominence, an acoustic measure of voice/speech qual

The Chiropractic Vertebral Subluxation Part 4: New Perspectives and Theorists From 1916 to 1927.

OBJECTIVE: The purpose of this paper is to review and discuss the history of chiropractic vertebral subluxation (CVS) between the years 1916 and 1927. DISCUSSION: Theories during this period were shaped by many chiropractic school leaders and instructors. Unique contributions to theories during this period come primarily from 4 authors, John Craven, Jim Drain, Shelby Riley, and Ralph Stephenson. This period included the first thermographic instrumentation in chiropractic, which led to one of Craven's modifications of CVS theory. He also added to the literature about spinal cord pressure and d

Memory-Guided Stumbling Correction in the Hindlimb of Quadrupeds Relies on Parietal Area 5.

In complex environments, tripping over an unexpected obstacle evokes the stumbling corrective reaction, eliciting rapid limb hyperflexion to lift the leg over the obstruction. While stumbling correction has been characterized within a single limb in the cat, this response must extend to both forelegs and hindlegs for successful avoidance in naturalistic settings. Furthermore, the ability to remember an obstacle over which the forelegs have tripped is necessary for hindleg clearance if locomotion is delayed. Therefore, memory-guided stumbling correction was studied in walking cats after the for

Motor imagery in spinal cord injured people is modulated by somatotopic coding, perspective taking, and post-lesional chronic pain.

Motor imagery (MI) allows one to mentally represent an action without necessarily performing it. Importantly, however, MI is profoundly influenced by the ability to actually execute actions, as demonstrated by the impairment of this ability as a consequence of lesions in motor cortices, limb amputations, movement limiting chronic pain, and spinal cord injury. Understanding MI and its deficits in patients with motor limitations is fundamentally important as development of some brain-computer interfaces and daily life strategies for coping with motor disorders are based on this ability. We explo

Posturographic destabilization in eating disorders in female patients exposed to body image related phobic stimuli.

Human postural control is dependent on the central integration of vestibular, visual and proprioceptive inputs. Psychological states can affect balance control: anxiety, in particular, has been shown to influence balance mediated by visual stimuli. We hypothesized that patients with eating disorders would show postural destabilization when exposed to their image in a mirror and to the image of a fashion model representing their body ideal in comparison to body neutral stimuli. Seventeen females patients attending a day centre for the treatment of eating disorders were administered psychometric

The right temporoparietal junction plays a causal role in maintaining the internal representation of verticality.

Perception of the visual vertical is strongly based on our ability to match visual inflow with vestibular, proprioceptive, tactile, and even visceral information that contributes to maintaining an internal representation of the vertical. An important cortical region implicated in multisensory integration is the right temporoparietal junction (rTPJ), which also is involved in higher order forms of body- and space-related cognition. To test whether this region integrates body-related multisensory information necessary for establishing the subjective visual vertical, we combined a psychophysical

Unilateral posterior cervical spinal cord infarction due to spontaneous vertebral artery dissection.

CONTEXT: Ischemia of the cervical spinal cord is a rare complication of spontaneous vertebral artery dissection (VAD) and usually involves the ventral portion. We describe a less evocative clinical presentation and images of unilateral posterior spinal cord infarction due to spontaneous VAD in order to facilitate early diagnosis. FINDINGS: A previously fit 30-year-old man presented with persistent headaches and proximal motor deficit of the right arm. He was diagnosed with spontaneous dissection of both vertebral arteries, with occlusion of the right one, and the right carotid artery. Neurolo

Modulation of spinal reflex by assisted locomotion in humans with chronic complete spinal cord injury.

OBJECTIVE: In healthy subjects, spinal reflexes (SR) evoked by non-noxious tibial nerve stimulation consist of an early (60-120ms latency) and an occasional late-appearing (120-450ms latency) component in the ipsilateral tibialis anterior. In chronic (>1year) complete spinal cord injured (cSCI) subjects early components are small or lacking while late components are dominant. Here we report on the modulation of SR by assisted locomotion in healthy and chronic motor cSCI subjects. METHODS: SR was evoked by tibial nerve stimulation at the terminal stance phase during assisted locomotion and was

Visual targeting of forelimbs in ladder-walking locusts.

Accurate limb placement helps animals and robots to walk on substrates that are uneven or contain gaps. Visual information is important in controlling limb placement in walking mammals but has received little attention in insects. We investigated whether desert locusts walking along a horizontal ladder use vision to control limb placement. High-speed video analysis showed that locusts targeted their front legs to specific rungs in the absence of any previous contact, suggesting that visual information alone is sufficient for targeting single steps. Comparison between the proportions of missed

Learning a novel myoelectric-controlled interface task.

Control of myoelectric prostheses and brain-machine interfaces requires learning abstract neuromotor transformations. To investigate the mechanisms underlying this ability, we trained subjects to move a two-dimensional cursor using a myoelectric-controlled interface. With the upper limb immobilized, an electromyogram from multiple hand and arm muscles moved the cursor in directions that were either intuitive or nonintuitive and with high or low variability. We found that subjects could learn even nonintuitive arrangements to a high level of performance. Muscle-tuning functions were cosine shap

[Reversal of vision metamorphopsia as a manifestation of cerebellar infarct].

INTRODUCTION: Reversal of vision metamorphopsia is a disorder affecting the visuospatial perception of objects, without any changes in their shape, size or colour. It generally involves a full 180 degrees rotation of the visual field in the coronal plane. Its chief causation is vertebrobasilar ischaemia, although the phenomenon has also been linked to many other conditions, such as multiple sclerosis, epilepsy, migraine or traumatic head and neck injuries. Some notable features of reversal of vision metamorphopsia are the wide topographic variety of the lesions responsible for the condition, t

Development of involuntary movements after ventriculoperitoneal shunting for normal pressure hydrocephalus in a patient with chronic-phase thalamic haemorrhage.

BACKGROUND: Delayed-onset involuntary movements have been described after thalamic stroke. METHODS: We treated a patient with involuntary movements that increased after ventriculoperitoneal shunting (VPS) for normal pressure hydrocephalus (NPH) following thalamic haemorrage. One and one-half years after right thalamic and intraventricular haemorrhage, NPH suggested clinical evaluation and neuroimaging studies in a 56-year-old man. RESULTS: Hemidystonia and pseudochoreoathetosis were evident in the left arm, leg and trunk. Proprioceptive impairment and mild cerebellar dysfunction affected the

Motor cortex activation is preserved in patients with chronic hemiplegic stroke.

Many central nervous system conditions that cause weakness, including many strokes, injure corticospinal tract but leave motor cortex intact. Little is known about the functional properties of surviving cortical regions in this setting, in part because many studies have used probes reliant on the corticospinal tract. We hypothesized that many features of motor cortex function would be preserved when assessed independent of the stroke-affected corticospinal tract. Functional MRI was used to study 11 patients with chronic hemiplegia after unilateral stroke that spared regions of motor cortex. Ac

Pain, distress and joint hyperlaxity.

Pain dominates the lives of many patients with hyperlaxity syndromes, most commonly the Benign Joint Hypermobility Syndrome (BJHS/EDS). As a result they may experience psychosocial problems, which in many cases severely affects their healthy functioning. Above all is the overriding chronic pain in joints, muscles and ligaments, which arises from an inherent predisposition to the effects of everyday trauma, but other factors such as associated osteoarthritis or fibromyalgia are also important. There may also be neurophysiological factors at play producing nociceptive enhancement. Pain and distr

Thoracic and prothoracic leg neuromuscular system of the praying mantid, Sphodromantis lineola (Burmeister).

Historically, praying mantids have attracted attention because of their dramatic prey capture behavior, loosely termed the strike. However, little is known about the neuromuscular organization that underpins the behavior. Although once thought to be quite stereotyped, recent data indicate that the strike is quite plastic and can be aimed accurately within a relatively large three-dimensional space. Hence, successful prey capture requires the integration of (1) visual information, indicating prey has been recognized; and (2) proprioceptive information, indicating head and prothorax (i.e., visua
