Open Reduction and Internal Screw Fixation of Transitional Ankle Fractures in Adolescents.

UNLABELLED: A triplane fracture is an example of a transitional fracture of adolescence that occurs because the distal tibial physis closes in a predictably asymmetric way from central to medial and then lateral. The triplane fracture is so named because the fracture lines propagate in 3 planes (axial, sagittal, and coronal) and thus appear on radiographs as a Salter-Harris III pattern on anteroposterior images and Salter-Harris II or IV on lateral images. The fracture occurs via a twisting mechanism (usually supination and external rotation) through the relatively weak open portion of the phy

Association Between Cognitive Function and Health Care Costs 3 Months and 6 Months After Initiating Antidepressant Medication for Depressive Disorders.

BACKGROUND: Major depressive disorder is one of the most common and disabling mental health disorders and is associated with substantial costs in terms of direct health care utilization and workplace productivity. Cognitive dysfunction, which alone substantially increases health care costs, is commonly associated with major depressive disorder. However, the health care costs of cognitive dysfunction in the context of depressive disorder are unknown. Recovery from mood symptoms is not always associated with resolution of cognitive dysfunction. Thus, cognitive dysfunction may contribute to healt

Melatonin for preoperative and postoperative anxiety in adults.

BACKGROUND: Anxiety in relation to surgery is a well-known problem. Melatonin offers an alternative treatment to benzodiazepines for amelior … lly relevant. The effect of melatonin on postoperative anxiety compared to placebo (measured in the recovery room and six hours after surgery) was also evident but was much smaller, and the clinical relevance of this finding is uncertain. There was little or no difference in anxiety when melatonin was compared with benzodiazepines. Thus, melatonin may have a si …

Diagnosis of very long chain acyl-dehydrogenase deficiency from an infant's newborn screening card.

Very long chain fatty acid dehydrogenase (VLCAD) deficiency is a rare but treatable cause of cardiomyopathy, fatty liver, skeletal myopathy, pericardial effusions, ventricular arrhythmias, and sudden death. Unrecognized, VLCAD deficiency may be rapidly progressive and fatal, secondary to its cardiac involvement. Because early diagnosis improves outcome, we present a neonate with VLCAD deficiency in whom retrospective analysis of the newborn screening card revealed that a correct diagnosis could have been made by newborn screening using tandem mass spectrometry. Our patient demonstrated a class

Preoperative Shoulder Stiffness Was Accompanied by Distinct Supraspinatus Tendon Inflammatory Signatures but Not Inferior Long-term Outcomes After Rotator Cuff Repair.

BACKGROUND: Rotator cuff tears with concomitant preoperative shoulder stiffness present a clinical challenge because the rehabilitation needed to restore motion may conflict with the protection typically used to support tendon healing after rotator cuff repair. Although recent studies suggest that preoperative stiffness may not be associated with inferior long-term outcomes, whether it is associated with a different pattern of postoperative improvement and distinct inflammatory features in the torn tendon remains unclear. QUESTIONS/PURPOSES: (1) Do a greater proportion of patients with preope

The effectiveness of strategies for the management and/or prevention of hypothermia within the adult perioperative environment: systematic review.

BACKGROUND: Inadvertent hypothermia is common in patient's undergoing surgical procedures. Hypothermia within the perioperative environment may have many undesired physiological effects that are associated with significant postoperative morbidity. Patient's temperature drops to below 35°C during the first hour of anaesthesia because of impaired thermoregulatory mechanism and patient getting cold in the operating theatre. For this reason, health care professionals working in the perioperative environment need to know what are the most effective strategies for treating or preventing hypothermia

Bispectral index monitor: an evidence-based analysis.

The objective of this analysis was to evaluate the effectiveness and cost-effectiveness of the bispectral index (BIS) monitor, a commercial device to assess the depth of anesthesia. Conventional methods to assess depth of consciousness, such as cardiovascular and pulmonary measures (e.g., heart rate, systolic/diastolic blood pressure, mean arterial pressure, respiratory rate, and level of oxygen in the blood), and clinical signs (e.g., perspiration, shedding of tears, and limb movement) are not reliable methods to evaluate the brain status of anesthetized patients. Recent progress in understan

[A protein-specific quantitative detection method based on polyacrylamide gel electrophoresis and online fluorescence imaging].

Specific protein detection plays a crucial role in biological analysis and clinical diagnostics, serving as an essential tool for disease diagnosis, therapeutic monitoring, and biological research. However, conventional methods such as immunofixation electrophoresis (IFE) and western blotting (WB) suffer from complex workflows, time-consuming operations, and limited quantification capabilities owing to intricate staining and de-staining procedures. In addition, these traditional immunological detection methods require extensive manual handling and specialized expertise, while low levels of aut

[Preparation and application of graphene oxide functionalized melamine-formaldehyde aerogel coated solid-phase microextraction tube].

Many solid-phase microextraction (SPME) sorbents have been developed from aerogels because of their low densities, large surface areas, and high porosities. Melamine-formaldehyde (MF) aerogel, made from melamine and formaldehyde by a sol-gel reaction, is one of the typical organic aerogels. MF aerogel has better mechanical strength, chemical stability and extraction performance than inorganic aerogels. The performance of the aerogel is limited in some fields, while composite aerogels can meet different requirements such as good mechanical strength and strong adsorption performance. Graphene ox

Bridging Haemato-Oncology and Critical Care: Survival and Functional Outcomes From a Portuguese Central Hospital.

Background Despite substantial therapeutic progress in haemato-oncology (HO), referring exclusively to patients with haematological malignancies, ICU admission decisions for these patients are often hindered by the perception of poor prognosis. This study aimed to evaluate the one-year survival and functional outcomes of critically ill HO patients following hospital discharge. Methods A retrospective, single-centre observational study was conducted in a Portuguese tertiary care centre, including 54 consecutive non-elective ICU admissions of adult HO patients between January 2022 and October 20

Navigating isolation- barriers and multidimensional support needs in symptom self-management for HSCT patients within laminar airflow rooms: a descriptive qualitative study.

BACKGROUND: Hematopoietic stem cell transplantation (HSCT) is a crucial therapy for hematological malignancies and other conditions. However, the intensive therapy and required isolation place significant physical and psychological burdens on patients. Although effective symptom self-management during isolation is pivotal, the current efforts are often challenged, leading to distress and feelings of loss of control. Nevertheless, research specifically addressing the self-management needs and barriers faced by patients with HSCT confined to laminar airflow rooms is limited. This study aimed to
