A comparative analysis of fatty infiltration and muscle atrophy in patients with chronic rotator cuff tears and suprascapular neuropathy.

BACKGROUND: Little is known of the mechanisms that lead to the muscle changes associated with rotator cuff disorders. We have observed that the magnetic resonance imaging (MRI) appearance of fatty infiltration (FI) and muscle atrophy (MA) differ between chronic cuff tears and suprascapular neuropathy, suggesting different pathophysiology. This study compares the different MRI changes that occur in chronic cuff tears and suprascapular neuropathy. METHODS: Two groups were retrospectively identified: (1) RCT group (20 shoulders): patients with chronic tears of the supraspinatus and/or infraspina

Revision arthroplasty with a hip-inspired computer-assisted design/computer-assisted manufacturing implant for glenoid-deficient shoulders.

BACKGROUND: Revision arthroplasty for failed post-traumatic humeral head replacement associated with rotator cuff and glenoid deficiency is challenging. Current surgical solutions are fraught with complications, and no best-practice strategy has been established. We hypothesized that the computer-assisted design/computer-assisted manufacturing (CAD/CAM) shoulder (Stanmore Implants, Elstree, UK), a total shoulder design resembling a total hip prosthesis, can offer a reliable alternative in this surgically challenging subset of patients with rotator cuff deficiency and advanced glenoid bone loss

MR imaging of the shoulder: first dislocation versus chronic instability.

Traumatic dislocation of the shoulder is a frequent injury in the young and active population. An acute shoulder dislocation often denotes a onetime traumatic episode, whereas chronic shoulder instability indicates multiple recurrent dislocations. Imaging, in particular MRI, is a useful tool that can accurately demonstrate the typical soft tissue and osseous markers of shoulder dislocation. However, the ability to differentiate between first time versus recurrent dislocation based on imaging remains in question. In this article, we describe the underlying biomechanics of glenohumeral stability

Comparison of 2- and 3-dimensional shoulder ultrasound to magnetic resonance imaging in a community hospital for the detection of supraspinatus rotator cuff tears with improved worktime room efficiency.

PURPOSE: The purpose of this study was to evaluate whether 3-dimensional (3D) volumetric acquisition of shoulder ultrasound (US) data for supraspinatus rotator cuff tears is as sensitive when compared with conventional 2-dimensional (2D) US and routine magnetic resonance imaging (MRI), and whether there is improved workroom time efficiency when using the 3D technique compared with the 2D technique. METHODS: In this prospective study, 39 shoulders underwent US and MRI examination of their rotator cuff to confirm the accuracy of both the 2D and 3D techniques. The difference in sensitivities was

A new nerve block procedure for the suprascapular nerve based on a cadaveric study.

HYPOTHESIS: Shoulder pain after rotator cuff repair surgery is sometimes very severe. Suprascapular nerve block (SSNB) is a method used to relieve this shoulder pain, but is not always completely effective. The purpose of this study is to develop the new effective suprascapular nerve block procedure. METHODS: To obtain an effective SSNB, we investigated an anatomic lesion of the sensory branches of the SSN in 8 cadavers and developed a new procedure. We evaluated its effectiveness by recording visual analog scale (VAS) scores in 8 patients experiencing severe pain after rotator cuff repair su

MR shoulder arthrography in patients younger than 40 years of age: frequency of rotator cuff tear versus labroligamentous pathology.

The purpose of this study was to compare the frequency of rotator cuff pathology versus labroligamentous pathology in patients younger than 40 years and to determine whether routine MR arthrography is justified in all patients in this age group, regardless of the clinical symptoms. The MR arthrography was carried out on 332 patients 40 years of age and younger. Two hundred and forty-three patients had clinical history of instability and possible labroligamentous pathology. Eighty-nine patients had no history or physical signs of instability and were referred for reasons other than instability,

[Postoperative imaging of the shoulder].

Correct interpretation of imaging findings in the postoperative shoulder is impaired by surgical distortion of normal anatomy and possible artifacts. Advanced postoperative imaging of the shoulder in addition to the selection of the best suited modality necessitates familiarity with the surgical procedure that has been performed and its consecutive morphological changes. This article reviews the most common arthroscopic and open techniques used for treatment of shoulder instability, lesions of the superior labral-bicipital complex, primary impingement, and rotator cuff tears, their typical pos

What Are Practical Surgical Anatomic Landmarks and Distances from Relevant Neurologic Landmarks in Cadavers for the Posterior Approach in Shoulder Arthroplasty?

BACKGROUND: Traditional total shoulder arthroplasty is performed through the deltopectoral approach and includes subscapularis release and repair. Subscapularis nonhealing or dysfunction may leave patients with persistent pain, impairment, and instability. Alternative approaches that spare the subscapularis include rotator interval and posterior shoulder approaches; however, to our knowledge, a cadaveric study describing pertinent surgical anatomy for a posterior shoulder approach regarding shoulder arthroplasty has not been performed. QUESTIONS/PURPOSES: (1) What are the distances from impor

The role of scapular kinematics in patients with different shoulder musculoskeletal disorders: A systematic review approach.

This study aimed to identify the role of the scapula in shoulder musculoskeletal disorders (SMDs) and provided a systematic review of available studies in the field of scapular three-dimensional kinematics. We systematically searched 5 international databases, including Scopus, EMBASE, PubMed, CINAHL, PEDro, and Cochrane Library from June to September 2015. Twenty studies met the inclusion criteria and were retrieved in full paper. The selected studies were critically appraised independently by two researchers. The patients with shoulder impingement syndrome (SIS) and shoulder instability had

LIPUS combined with mild hyperthermia promotes rotator cuff healing via TRPV1/Ca2+/CaMKII-driven BACH2 nuclear translocation and inhibition of M1 polarization.

BACKGROUND: Rotator cuff injuries often lead to impaired shoulder function, and tendon-bone healing remains challenging due to the complex structure of the enthesis and persistent remodeling-associated pro-inflammatory signaling. Low-intensity pulsed ultrasound (LIPUS) and mild hyperthermia (MH) have shown potential in tissue repair, but the molecular mechanisms underlying their combined effects are unclear. OBJECTIVE: This study aimed to investigate whether LIPUS and MH synergistically promote tendon-bone healing by regulating macrophage polarization through the TRPV1/Ca2+/CaMKII/BACH2 signa…

Efficacy of Diagnostic In-Office Shoulder Ultrasound in the Surgical Treatment of Full-Thickness Rotator Cuff Tears.

BACKGROUND: Early surgical management of full-thickness traumatic rotator cuff tears (RCTs) may optimize functional outcomes, prioritizing timely diagnoses. Ultrasound and magnetic resonance imaging (MRI) are highly sensitive and specific modalities for RCT diagnosis, yet MRI remains the gold standard diagnostic tool despite increased costs and potential delays in care. Ultrasound can provide same-day diagnosis, thus possibly expediting care. HYPOTHESIS: The use of diagnostic shoulder ultrasound alone in the orthopaedic surgeon's office could increase the efficiency of diagnosis and care of t

[Magnetic resonance imaging criteria for the differentiation of traumatic and non-traumatic rotator cuff tears].

It is often not easy to classify a rotator cuff tear into traumatic or non-traumatic. For individual medical examinations the classification depends basically on the following four criteria: Case history, analysis of the accident, analysis of the complaints after the accident (or even before) and the pathomorphological findings. The aim of this study was to identify findings in the magnetic resonance imaging of the shoulder joints of patients with rotator cuff tears that could help in the differentiation of traumatic versus non-traumatic. As a result it could be shown that infraspinatus and su

Correlation of age, acromial morphology, and rotator cuff tear pathology diagnosed by ultrasound in asymptomatic patients.

The importance of acromial morphology in the pathogenesis of rotator cuff tears remains controversial. Some surgeons place great emphasis on acromial morphology and others feel that acromial shape is a result of cuff tear pathology rather than the cause. The purpose of this study was to determine if there was an association between acromial morphology, age, and rotator cuff tears found by ultrasound in asymptomatic volunteer subjects with no past history of shoulder symptoms. One hundred eighteen outlet x-rays and ultrasound scans were performed in 59 asymptomatic patients in various age group

Biologics in shoulder and elbow pathology.

In recent years, orthobiologics have been of increasing clinical interest in the treatment of shoulder and elbow pathology. In some conditions, such as rotator cuff injury and lateral epicondylitis, there have been high-quality trials that support the use of platelet-rich plasma in reducing pain, restoring functionality, and improving clinical outcomes. However, as the numbers of both cellular-based biologics and the conditions being augmented by biologics continue to expand, there is a substantial need for high-quality investigations to support their routine use in most shoulder and elbow con

Repair of partial tears of the rotator cuff.

Partial-thickness rotator cuff tears present partial disruption of tendon fibers with no communication between the subacromial bursa and the glenohumeral joint. The clinical presentation is surprisingly variable, ranging from mild discomfort to decreased throwing speed, chronic pain, and shoulder inability. The first approach to partial-thickness rotator cuff tears is usually conservative, but the hypovascularity of the critical zone and mechanical factors often result in poor spontaneous tendon healing. Surgical options include arthroscopic cuff "debridement" or "repair," performed arthroscop

Shoulder tendon transfer options for adult patients with brachial plexus injury.

Enhancement of upper-extremity function, specifically shoulder function, after brachial plexus injury requires a good understanding of nerve repair and transfer, with their expected outcome, as well as shoulder anatomy and biomechanics enabling the treating surgeon to use available functioning muscles around the shoulder for transfer, to improve shoulder function. Surgical treatment should address painful shoulder subluxation in addition to improvement of function. The literature focuses on improving shoulder abduction, but improving shoulder external rotation should take priority because this

Arthroscopic acromioplasty. Current status.

Impingment is a chronic syndrome characterized by microtrauma, which causes progressive injury to the rotator cuff tendon. In recent years, arthro- scopic subacromial decompression/acromioplasty has been frequently used for the treatment of impingement syndrome and is quickly becoming the preferred surgical treatment when conservative modalities fail. Arthroscopic acromioplasty offers many benefits over open acromioplasty, including better cosmesis, lessened preoperative morbidity, a more complete intraoperative examination, and a hastened, early rehabilitation program.

A Hypothalamic Oxytocinergic-Sympathetic Axis Couples Anxiety Dysregulation With Impaired Tendon-Bone Repair.

Rotator cuff injury (RCI) is frequently accompanied by psychological comorbidities, particularly anxiety-related symptoms, which predict poor functional recovery, yet the central mechanisms linking affective disturbance to impaired tendon-bone healing remain unclear. Here, by integrating clinical neuroimaging, imaging transcriptomics, and mechanistic animal experiments, we identify hypothalamic paraventricular nucleus (PVN) oxytocinergic (OXT) neurons as a critical central node coordinating emotional regulation and bone-tendon interface (BTI) repair. In patients with RCI, resting-state fMRI (r

Autologous peroneus longus tendon graft for superior fulcrum reconstruction: Maintained prospective 1-year outcomes at short-term final follow-up.

BACKGROUND: Massive irreparable rotator cuff tears (MIRCTs) present challenges in terms of traditional treatments and can result in pain and functional impairment. Due to the limitations of traditional treatments for MIRCTs, alternative options such as superior fulcrum reconstruction (SFR) using the autologous peroneus longus tendon (PLT) have been explored. This study aims to evaluate the clinical and radiographic outcomes of SFR with autologous PLT grafts for the treatment of MIRCTs after a minimum follow-up period of 1 year. METHODS: This was a prospective cohort study. Thirty-six patients
