BACKGROUND: The long-term success of primary goniotrephining with scleral flap was evaluated in the treatment of primary congenital glaucoma.
PATIENTS AND METHODS: Retrospective analysis was carried out in 31 eyes of 18 patients with primary congenital glaucoma (mean follow-up 63 months, range 10-161). PURPOSE: To describe 2 techniques of combined viscocanalostomy and phacoemulsification and report the results.
SETTING: Gimbel Eye Centre, Calgary, Alberta, Canada.

Failure of glaucoma surgery is mostly due to fibrocellular scar formation, derived from Tenon's capsule fibroblasts. In high-risk cases, postoperative Tenon's capsule fibroblast proliferation is inhibited by mitomycin C or 5-fluorouracil. Toxicity to other ocular cell types and the risk of ocular hypotony limits the use of these agents. BACKGROUND: Three children, ranging in age from 2 to 6 months, had diffuse and homogeneously opaque corneas, clinically consistent with congenital hereditary endothelial dystrophy. Bilateral elevated intraocular pressure (IOP) was a feature in all three children.
METHODS: Initially, all patients underwent glaucoma surgery to reduce IOP. Subsequently, a penetrating keratoplasty was performed in one eye of each patient to clear the visual axis. The excised corneal button was examined by light microscopy and by transmission and scanning electron microscopy.

Two patients with spontaneous, angiographically proven fistulas showed progressive ocular symptoms which precluded waiting for spontaneous occlusion of the fistulas. On the other hand, endovascular therapy of the carotid-cavernous fistulas was not possible because the arterial supply of the fistulas came from both internal carotids. Therefore filtering surgery was performed in the affected eyes. Postoperatively, intraocular pressure decreased to normal values in both patients and the symptoms resolved.
