BACKGROUND: To identify risk factors related to the onset of secondary glaucoma in eyes that have undergone pars plana lensectomy for congenital cataract.
METHODS: We investigated intraocular pressure levels after pars plana lensectomy for congenital cataract in 80 eyes of 44 patients who were followed up for at least 36 months. Mean age at the time of pars plana lensectomy was 11+/-18 months. Mean follow-up period was 116+/-57 months.
RESULTS: An elevation in intraocular pressure (>20 mmHg) was found in 21 eyes of 12 patients (26%). In 7 of these 21 eyes surgical treatment was required. The

An aphakic patient with severe chronic hypotony had an alternative treatment of a traumatic cyclodialysis cleft: a 3-port pars plana vitrectomy, cryotherapy of the cleft, and fluid-gas exchange with subsequent supine positioning. The therapeutic principle was mechanical apposition of the detached ciliary muscle to the scleral spur by the gas bubble and scar induction by cryotherapy. Intraocular pressure increased to within normal ranges, and visual acuity improved over a 15 month follow-up.

To study the image of contact lenses and intraocular lens implants in vitro and in vivo by CT and MRI prospectively. Soft and rigid contact lenses can be seen in vitro in both methods but in vivo imaging was insensitive. Intraocular lens implants were detected in CT and MRI in vitro. In vivo, intra ocular lens implants were detected in all CT, but were harder to detect by the MRI.

PURPOSE: To compare the results of secondary implantation of angle-supported anterior chamber intraocular lenses (IOLs) and scleral-fixated posterior chamber lenses.

Forty one patients aged 29 to 82 years were subjected to cataract extraction in ambulatory conditions. Fifty eyes were operated, among them 33 with implantation of a posterior chamber artificial lens. We measured three aspects of vision in children treated for unilateral congenital cataract: visual resolution, the symmetry of optokinetic nystagmus (OKN), and peripheral vision. Argon laser trabeculoplasty (ALT) was used to treat 237 eyes of 156 presurgical patients with various open-angle glaucomas. Patients with primary open-angle glaucoma formed the largest group; intraocular pressure (IOP) control judged clinically to be adequate was achieved in 77.3% of eyes. 73 consecutive aphakic retinal detachments were operated on by microsurgery combined with intra-operative biomicroscopic control of the ocular fundus. Retinal reattachment was achieved in 60 cases (82%), 59 eyes had only one operation. The visual, refractive, and surgical results with 190 consecutive secondary implantations of an anterior chamber intraocular lens were studied. Best-corrected postoperative visual acuity was either better than or within 1 Snellen line of best-corrected preoperative vision in 87% of the cases. Thirty-three keratophakias were performed at Holy Cross Hospital in Mission Hills, California between August 1979 and October 1982. Fifteen consecutive eyes in 14 patients (six men and eight women, ranging in age from 43 to 75 years) underwent anterior vitrectomy for chronic cystoid macular edema documented by fluorescein angiography. A contact lens made from silicone rubber has been fitted successfully for extended wear in children. The lens has been used primarily with aphakic children, but it has also been tolerated by children with anisometropia, high myopia, and keratoconus. The lens can be handled like a hard contact lens and can be fitted by a variety of methods. The eye shows excellent tolerance of the lens and complications or failures to wear the lens have been due primarily to discomfort. After about a year of wear the lens surface can no longer be cleaned.

In 650 consecutive Binkhorst lens implants, secondary membrane surgery was required more than eight times as frequently after an extracapsular than after an intracapsular cataract extraction. The visual acuity results after discussion were uniformly good. Transpars plana vitrectomy was reserved for more severe cases. The pupils were cleared, but the visual acuity results were mixed.
