Gender differences in the electrophysiological characteristics of atrioventricular conduction system and their clinical implications.

OBJECTIVE: The underlying mechanisms of the differences in sex distribution of patients with atrioventricular (AV) nodal re-entrant tachycardia and Wolff-Parkinson-White syndrome are poorly understood. The objective of this study was to determine potential gender differences in the electrophysiological properties of the normal AV conduction system that may be attributable to differences in sex distribution. DESIGN: The AV conduction properties were studied in 96 patients (52 men and 44 women) who underwent electrophysiological testing, 32 patients with atrial tachycardia, 39 with idiopathic v

Heart block in patients with obstructive sleep apnoea: pathogenetic factors and effects of treatment.

Heart block during sleep has been described in up to 10% of patients with obstructive sleep apnoea. The aim of this study was to determine the relationship between sleep stage, oxygen desaturation and apnoea-associated bradyarrhythmias as well as the effect of nasal continuous positive airway pressure (nCPAP)/nasal bi-level positive airway pressure (nBiPAP) therapy on these arrhythmias in patients without electrophysiological abnormalities. Sixteen patients (14 males and two females, mean age 49.6+/-10.4 yrs) with sleep apnoea and nocturnal heart block underwent polysomnography after exclusion

Slow conduction in cardiac tissue, II: effects of branching tissue geometry.

In cardiac tissue, functional or structural current-to-load mismatches can induce local slow conduction or conduction block, which are important determinants of reentrant arrhythmias. This study tested whether spatially repetitive mismatches result in a steady-state slowing of conduction. Patterned growth of neonatal rat heart cells in culture was used to design unbranched cell strands or strands releasing branches from either a single point or multiple points at periodic intervals. Electrical activation was followed optically using voltage-sensitive dyes under control conditions and in elevat

P wave morphology during atrial pacing along the atrioventricular ring. ECG localization of the site of origin of retrograde atrial activation.

P wave morphology during atrial pacing along the atrioventricular (AV) ring was evaluated to develop electrocardiographic (ECG) criteria for identifying the site of origin of the atrial activation wave during reentrant supraventricular tachycardia. Because P wave morphology changes as the pattern of atrial activation changes, the P wave should show characteristic morphologies during reentrant supraventricular tachycardia with use of either accessory AV pathways or the AV node for retrograde atrial activation. In 14 patients, 12-lead ECGs were recorded during bipolar atrial pacing at sites in t

Bradycardia in Athletes: Prevalence, Mechanisms, and Risks.

BACKGROUND: Sinus bradycardia is a well-recognized physiological adaptation in endurance athletes, primarily attributed to sinus node remodeling or increased vagal modulation. Although genetic influences on resting heart rate (HR) have been observed, the genetic contribution to athletic bradycardia has not been elucidated. METHODS: We phenotyped current and former elite endurance athletes in the Pro@Heart cohort study using multimodal cardiac imaging, cardiopulmonary exercise testing, and Holter monitoring. Genetic susceptibility to bradycardia was assessed using a validated HR-associated pol

[Pacemaker lead implant via the persistent left superior vena cava].

BACKGROUND: Persistent left superior vena cava (PLSVC) is a structural, asymptomatic and infrequent anomaly, present in 0.5% of the general population. Typically the diagnosis reveals itself unexpectedly at the time of pacemaker implantation, when it acquires anatomic relevancy. Several techniques are used for the transvenous approach to enter the central venous circulation; the left subclavian vein has become a common access site for electrode implantation and, occasionally, one can find an anomalous venous structure such as a PLSVC. Placement of electrodes through this anomalous venous struc

Clinical and genetic investigation of atrial septal defect with atrioventricular conduction defect in a large consanguineous Tunisian family.

BACKGROUND: Atrial septal defect (ASD) is an autosomal dominant disease characterized by left-to-right shunting and increased right ventricular output. Approximately 5-10% of congenital heart diseases (CHD) are due to ASD, which is one of the most frequent CHD found in adults. The gene responsible for ASD was mapped to chromosome 5q35 encoding the transcription factor NKX2-5 that plays an important role for the regulation of septation during cardiac morphogenesis. METHODS: A Tunisian family including four affected members was investigated. Individuals were genotyped using the polymorphic micr

Is phenytoin administration safe in a hypothermic child?

A male neonate with a Chiari malformation and a leaking myelomeningocoele underwent ventriculoperitoneal shunt insertion followed by repair of myelomeningocoele. During anaesthesia and surgery, he inadvertently became moderately hypothermic. Intravenous phenytoin was administered during the later part of the surgery for seizure prophylaxis. Following phenytoin administration, the patient developed acute severe bradycardia, refractory to atropine and adrenaline. The cardiac depressant actions of phenytoin and hypothermia can be additive. Administration of phenytoin in the presence of hypothermi

Acute and long-term effects of atrioventricular junction ablation and VVIR pacemaker in symptomatic patients with chronic lone atrial fibrillation and normal ventricular response.

INTRODUCTION: The precise role of irregular ventricular response in atrial fibrillation (AF) has not been fully elucidated. This study examined the independent effects of rhythm regularity in patients with chronic AF. METHODS AND RESULTS: This study included 50 patients who had chronic lone AF and a normal ventricular rate. Among these patients, 21 who underwent AV junction ablation and implantation of a VVIR pacemaker constituted the ablation group; the other 29 patients were the medical group. Acute hemodynamic findings were measured in 21 ablation patients before ablation (during AF, basel

Characterization of atrioventricular nodal response to electrical left vagal stimulation.

The dynamic effect of left vagal stimulation on atrioventricular conduction was studied in six isofluorane-anesthetized dogs ranging in weight from 22 to 29 kg. The cervical vagus nerve trunks were left intact and no beta-adrenergic blockade was produced so that any influences of the sympathetic nervous system and autonomic reflex activity could be observed. Atrial pacing was used to control the heart rate while single, short trains of left vagal stimulation were delivered and timed to occur at different instances during the cardiac cycle. The magnitude of the A-V delay depended on the instant

Radiofrequency catheter ablation of a nodoventricular Mahaim tract.

We report the case of a patient with both a left bundle branch block-preexcited tachycardia using a Mahaim bundle and a tachycardia exhibiting a left bundle branch pattern without preexcited complexes related to an AV nodal reentrant tachycardia. Successful ablation of the nodoventricular tract was obtained in the posteroseptal location in the AV node region. The pattern of Wolff-Parkinson-White (WPW) syndrome and both forms of tachycardia disappeared.

Apparent extension of the atrioventricular interval due to sensor-based algorithm against supraventricular tachyarrhythmias.

Rapid ventricular tracking response to supraventricular tachyarrhythmia is one major limitation to DDD pacing. In a DDDR pacemaker, sensor-based algorithms have been used to control these arrhythmias. These include the use of an interim rate limit (conditional ventricular tracking limit) or a separate maximum tracking and sensor rate limits (discrepant upper rate). These algorithms limit inappropriate ventricular pacing rate during tracking of pathological supraventricular tachyarrhythmia and atrial flutter by Wenckebach-like prolongation of the AV interval. We observed that this may cause an

Cardiac segmentation for ventricular tachycardia radioablation and radiotherapy patients with cardiac implants.

Deep-learning (DL) segmentation enables cardiac substructure (CS) delineation for advanced cardiac-sparing radiation therapy (RT). Patients presenting for RT may have implantable devices (cardioverter-defibrillators (ICDs)) and metal artifacts. Stereotactic body RT (SBRT) is used to treat ventricular tachycardia (VT), a population frequently with ICDs. We apply a DL model for RT patients with ICDs or undergoing VT-SBRT, enabling segmentation and CS-level dose analysis. CT simulation (CT-SIM) of 19 patients with ICDs and received VT-SBRT were evaluated. A validated segmentation model was retrai

Long-Term Outcomes of AV Node Ablation Plus CRT Compared to AF Catheter Ablation in Patients With AF and HFrEF.

BACKGROUND: Catheter ablation is an established rhythm-control strategy for atrial fibrillation (AF) and heart failure with reduced ejection fraction (HFrEF), while atrioventricular node ablation with cardiac resynchronization therapy (AVNA+CRT) is often used in older patients with advanced disease when durable rhythm control is less feasible. Comparative long-term outcomes remain unclear. METHODS: Using the TriNetX Research Network, we identified adults with HFrEF and AF from 2016 who underwent AF ablation or AVNA+CRT from 2016-2023 and 2016-2020 for the 2- and 5-year analysis, respectively.

Pacemaker therapy through a sex-specific lens: a narrative review of clinical and procedural disparities.

Pacemaker therapy is a cornerstone in the management of bradyarrhythmias, yet accumulating data highlight important sex-based disparities across indications, procedural care and long-term outcomes. Women are consistently under-represented among pacemaker recipients, tend to be older at implantation, and are more frequently admitted via emergency pathways despite substantial symptom burden from sinus node dysfunction. Men more often receive pacemakers for atrioventricular conduction disease, at younger ages and with greater cumulative exposure to long-term pacing and device-related procedures.

International Survey on Vein of Marshall Retrograde Ethanol Infusion.

BACKGROUND: Retrograde ethanolization of the vein of Marshall (VOM) has been identified as an adjunct technique in the treatment of persistent atrial fibrillation (AF) and left atrial tachycardia, as stated in the last consensus statement on ablation of AF. However, there is a lack of high-volume data on the technique. METHODS: Through the collection of data from worldwide centers, we performed this international survey that aims to analyze the safety and procedural characteristics of VOM ethanolization in patients referred for treatment of AF or left atrial tachycardia. RESULTS: We included
