Adjuvant use of self-expanding stents in acute atherothrombotic vertebrobasilar occlusions.

Recanalization of acute atherothrombotic vertebrobasilar occlusions is a challenging neuroendovascular procedure. A long angiographic occlusion frequently overestimates the true extent of occlusive plaque. We propose the novel use of the Solitaire stent (ev3 Endovascular, Plymouth, MN, USA) as an adjuvant device in the endovascular management of acute atherothrombotic vertebrobasilar occlusions and present our experience. A self-expanding Solitaire stent is fully deployed, but not detached, along the entire occluded segment, from normal-to-normal arterial segment, to obtain a transient endovas

Clinical analysis of STA-SCA bypass for vertebrobasilar occlusive disease.

In order to clarify the effectiveness of extracranial- intracranial bypass in cases of vertebro-basilar occlusive disease, we investigated the operative complication, clinical course and follow-up study of 30 cases undergoing superficial temporal artery-superior cerebellar artery (STA-SCA) bypass surgery. Postoperative angiogram showed the patency of the anastomoses in all cases. No serious surgical complications were observed. The outcome on discharge was excellent, with no morbidity and one mortality which was due to cardiac infarction. In the follow-up study, there were four cases with isch

Surgical management of refractory ischemic-side aneurysms in chronic internal carotid artery occlusion: insights from cases unfit for endovascular treatment.

BACKGROUND: Managing ipsilateral intracranial aneurysms (IAs) in chronic internal carotid artery occlusion (ICAO) is surgically challenging, particularly when endovascular treatment is not viable. This study delineates patient characteristics, individualized microsurgical strategies, and outcomes. METHODS: A retrospective review was conducted of patients with chronic ICAO and ipsilateral IAs treated between 2015 and 2023. Based on surgical stroke risk, patients were stratified into low- and high-risk groups and received individualized treatments, including aneurysm clipping, external carotid

Association of Bypass Surgery and Mortality in Moyamoya Disease.

BACKGROUND: Patients with moyamoya disease (MMD) have a high risk of stroke or death. We investigated whether extracranial to intracranial bypass surgery can reduce mortality by preventing strokes in patients with MMD. METHODS AND RESULTS: This nationwide retrospective cohort study encompassed patients with MMD registered under the Rare Intractable Diseases program via the Relieved Co-Payment Policy between 2006 and 2019, using the Korean National Health Insurance Service database. Following a 4-year washout period, landmark analyses were employed to assess mortality and stroke occurrence bet

Blood Pressure Goals and Clinical Outcomes after Successful Endovascular Therapy: A Multicenter Study.

OBJECTIVE: Elevated systolic blood pressure (SBP) after successful revascularization (SR) via endovascular therapy (EVT) is a known predictor of poor outcome. However, the optimal SBP goal following EVT is still unknown. Our objective was to compare functional and safety outcomes between different SBP goals after EVT with SR. METHODS: This international multicenter study included 8 comprehensive stroke centers and patients with anterior circulation large vessel occlusion who were treated with EVT and achieved SR. SR was defined as modified thrombolysis in cerebral ischemia 2b to 3. Patients w

The evaluation of intracranial bypass patency in neurosurgical practice.

BACKGROUND: The patency of cranial bypasses must be carefully evaluated during and after the microsurgical procedure. Although, several imaging techniques are used to evaluate the patency of bypasses, their findings are sometimes difficult to interpret. PURPOSE: The goal of this study was to assess the consistency of different diagnostic modalities for evaluating intracranial bypass patency. PATIENTS AND METHOD: This prospective study included 19 consecutive patients treated with EC-IC or IC-IC bypass for MoyaMoya disease (MMD) or complex/giant aneurysms between June 2016 and June 2018. Protective STA-MCA bypass to prevent brain ischemia during high-flow bypass surgery: case series of 10 patients.

BACKGROUND: High-flow extracranial-intracranial bypass is associated with a significant risk of ischemic stroke. The goal of this study is to evaluate the effectiveness of STA-MCA bypass preceding a high-flow bypass as a means of protecting the brain from ischemia during the high-flow bypass anastomosis in patients with otherwise untreatable aneurysms. MATERIALS AND METHOD: This prospective study included 10 consecutive patients treated for complex/giant aneurysm using a previous combined STA-MCA bypass and high-flow EC-IC bypass between June 2016 and January 2018 when classical endovascular

[Stenting in acute stroke].

Hemodynamic impairment caused by pseudo-occlusion or occlusion of the internal carotid artery (ICA) is a rare etiology of stroke. Acute revascularization of the vessel can restore the flow to the brain. This retrospective study shows that ICA stenting with proximal and distal protection devices in patients with acute stroke and pseudo-occlusion or occlusion of the ICA is technically feasible and can prevent further hemodynamic strokes. In contrast, selected patients have a high risk of reperfusion syndrome with intracranial hemorrhage and peri-interventional monitoring of blood pressure in an

Intracerebral steal phenomenon associated with global hyperemia in moyamoya disease during revascularization surgery.

OBJECT: The collateral vessels in moyamoya disease appear to retain their ability to constrict during hypocapnia but not to dilate during hypercapnia. It has been claimed that hypercapnia, as well as hypocapnia, decreases the blood flow in regions perfused by collateral vessels, presumably because of intracerebral steal. If this holds true, the decrease in blood flow may be proportional to the global hyperemia in the brain. To establish appropriate hemodynamic control during revascularization surgery, the authors monitored the jugular bulb oxygen saturation (SjO2) intraoperatively, a method th

The efficacy of bypass surgery for the patients with hemorrhagic moyamoya disease.

The efficacy of bypass surgery for the patients with hemorrhagic Moyamoya disease were studied by multi-center retrospective questionnaire study. The rebleeding rate was 28.3% (39/138 patients) in conservative treatment group and 19.1% (29/152 patients) in bypass surgery group with no significant statistical difference. It is still difficult to clarify the efficacy of bypass surgery for prevention of hemorrhagic attack.

Intracranial microvascular decompression for "cryptogenic" hemifacial spasm, trigeminal and glossopharyngeal neuralgia, paroxysmal vertigo and tinnitus: II. Clinical study and long-term follow up.

20 patients who had undergone microvascular decompression for the treatment of "idiopathic" trigeminal neuralgia (9 cases), hemifacial spasm (7 cases), glossopharyngeal neuralgia (3 cases) and paroxysmal vertigo and tinnitus (1 case) were followed up for 25 months on average. Permanent relief of symptoms was observed in 19 (95%), with sparing of cranial nerve function. Analysis of the clinical data shows that the patients described in the present series did not differ from those considered to suffer from "idiopathic" cranial nerve dysfunction syndromes. The importance of vascular cross compres

[Revascularization of the distal vertebral artery (3d segment). Indications in the treatment of vertebrobasilar insufficiency].

This article reviews 11 cases of distal revascularization of the extracranial vertebral artery above C2. Revascularization was indicated in 7 cases of extensive atheromatous stenotic or occlusive lesions, 3 cases of dissecting aneurysm, and one case of cervical malformation of the vertebral artery. The procedure of choice was a venous graft - in the C1/C2 space in 10 cases and above C1 in the eleventh. Doppler tests, ultrasound and arteriography demonstrated the patency of all the bypasses, and this was confirmed, 6 months to 3 years postoperatively, by Doppler and ultrasound. No neurological

Neutrophil Targeting Platform Reduces Neutrophil Extracellular Traps for Improved Traumatic Brain Injury and Stroke Theranostics.

Traumatic brain injuries (TBI) and stroke are major causes of morbidity and mortality in both developing and developed countries. The complex and heterogeneous pathophysiology of TBI and cerebral ischemia-reperfusion injury (CIRI), in addition to the blood-brain barrier (BBB) resistance, is a major barrier to the advancement of diagnostics and therapeutics. Clinical data showed that the severity of TBI and stroke is positively correlated with the number of neutrophils in peripheral blood and brain injury sites. Furthermore, neutrophil extracellular traps (NETs) released by neutrophils correlat

Acute reperfusion therapy via occluded vertebral artery using a guiding sheath for posterior circulation tandem occlusion: illustrative case.

BACKGROUND: Vertebral artery (VA) size, anatomy, and occlusion status should be considered when selecting endovascular access for basilar artery mechanical thrombectomy. In a patient with concomitant basilar artery and VA occlusion and a patent but hypoplastic contralateral VA, the occluded VA should be selected. The authors report a technique that utilizes advancing a guiding sheath with attached dilator via an occluded VA. OBSERVATIONS: A 65-year-old male presented with disturbed consciousness because of an acute infarction of the brainstem and cerebellum caused by a basilar artery occlusio

Exposure of the External Carotid Artery Through the Posterior Neck Triangle, Cadaveric Surgical Simulation: 2-Dimensional Operative Video.

The external carotid artery (ECA) is a robust extracranial donor used for high-flow cerebrovascular bypass procedures. It is usually exposed through the anterior triangle of the neck and may be used to revascularize recipients in the anterior or upper posterior cerebral circulations. However, when a high-flow bypass to the posterior circulation is indicated, oftentimes the patient needs to be put in the prone position (or variants thereof). In such situations, accessing the ECA through the anterior triangle of the neck can be challenging. Therefore, using a technique that enables the surgeon t

Malperfusion in Type A Dissection: Consider Reperfusion First.

Acute type A aortic dissection (ATAAD) is a vascular catastrophe, with a mortality of 1% per hour for the first 48 hours without surgical intervention. Of the diverse causes of morbidity and mortality associated with ATAAD, malperfusion, which complicates 20%-50% of cases, is particularly lethal. Although malperfusion can affect any vascular bed, this review focuses on the 3 most devastating: coronary, cerebral, and visceral malperfusion syndromes (MPS). Essentially, there are 3 methods of restoring flow to malperfused areas: central repair, fenestration, and direct revascularization of affect

Emergency EC-IC bypass for symptomatic atherosclerotic ischemic stroke.

Previous studies have shown that extracranial-intracranial (EC-IC) bypass surgery has no preventive effect on subsequent ipsilateral ischemic stroke in patients with symptomatic atherosclerotic internal carotid occlusion and hemodynamic cerebral ischemia. A few studies have assessed whether an urgent EC-IC bypass surgery is an effective treatment for main trunk stenosis or occlusion in acute stage. The authors retrospectively reviewed 58 consecutive patients who underwent urgent EC-IC bypass for symptomatic internal carotid artery or the middle cerebral artery stenosis or occlusion between Jan
