Longitudinal stability of visual evoked potentials in children and adolescents with hydrocephalus.

There were two purposes for this study: to determine the within-subject changes of visual evoked potentials (VEPs) over time, and to evaluate the usefulness of baseline VEP testing in the early diagnosis of increased intracranial pressure (ICP). Twenty children with hydrocephalus between the ages of 4 and 18 years (18 males, 2 females) were evaluated monthly for up to four months. Flash-evoked VEP tests were done in a standardized manner using a Cadwell Quantum 84 evoked potential stimulation and recording instrument. One participant had abnormal VEPs found during a previous episode of shunt m

Regional dissociation between the cerebral blood flow and gray matter density alterations in idiopathic normal pressure hydrocephalous: results from SINPHONI-2 study.

PURPOSE: The purpose of this study was to elucidate the specific regional cerebral blood flow (rCBF) alterations for idiopathic normal pressure hydrocephalus (iNPH) by comparing the proportional rCBF and gray matter change from those of a normal database at the same point of SPECT and MRI examinations. METHODS: Thirty subjects with iNPH underwent both CBF SPECT and MRI. After normalization, voxel-wise two-sample t tests between patients and 11 normal controls were conducted to compare the regional alteration in the gray matter density and rCBF. RESULTS: The rCBF reduction and the gray matter

[Is hippocampal atrophy a specific change for Alzheimer's disease?].

Although detection of hippocampal atrophy has been proposed for the diagnosis of Alzheimer's disease (AD), atrophic changes in MRI can be found in other dementia diseases. This study was undertaken to determine whether hippocampal atrophy was a specific change for AD. Coronal T 1-weighted images were performed in 36 patients with AD, 40 patients with non-AD including vascular dementia, frontemporal dementia, Parkinson's disease with dementia, dementia with Lewy bodies, progressive supranuclear palsy, and normal pressure hydrocephalus, 9 patients with age-associated memory impairment (AAMI), an

Acute phase reactant alpha 1-antichymotrypsin is increased in cerebrospinal fluid and serum of patients with probable Alzheimer disease.

Levels of alpha 1-antichymotrypsin (alpha 1-ACT) in cerebrospinal fluid (CSF) and serum from patients with probable Alzheimer disease (AD) of both early (e-AD) and late (l-AD) onset assessed by a competitive enzyme-linked immunosorbent assay were higher than those found in controls or in patients with vascular dementia (VD). A negative correlation between CSF levels of alpha 1-ACT and the stage of the disease was present in patients with both e-AD and l-AD. No difference in alpha 2-macroglobulin levels in CSF and serum from patients with e-AD, l-AD, VD, and nondemented controls was found. Seru

EEG Correlates of Spikes in Intracranial Pressure Caused by Transient Ventriculoperitoneal Shunt Malfunction.

A 30-year-old man with recurrent headaches and seizure-like activity and a 26-year-old woman with worsening headaches were admitted to the hospital. Both had ventriculoperitoneal shunts and history of several shunt revisions for congenital hydrocephalus. The ventricle size visualized on computed tomography scans was unremarkable, and shunt series were negative in both cases. Both patients began to present with brief periods of unresponsiveness, and video electroencephalography at that time showed periods of diffuse delta slowing. Lumbar punctures revealed increased opening pressures. Despite n

Abdominal Pseudocyst: A Rare Complication of Ventriculoperitoneal Shunts.

Источник пометил публикацию как отозванную

We present the case of a 69-year-old man patient who was brought with a history of gait disturbances, memory impairment, and urinary incontinence with gradual worsening over the past six months. The patient underwent magnetic resonance imaging of the brain which demonstrated enlarged ventricles, widening of the Sylvian fissure, and narrow sulci at the vertex. Subsequently, the patient underwent a lumbar puncture which revealed a normal opening pressure with normal cerebrospinal fluid analysis. The diagnosis of normal pressure hydrocephalus was established. The patient underwent a ventriculoper

Hydrocephalus, cervical cord lesions, and spinal deformity.

Twenty-six children (4-16 years) had hydrocephalus and spinal deformity; 22 children had scoliosis over 50 degrees, and five children had kyphosis over 100 degrees. Twenty-one children had metrizamide enhanced cord CT scans, and 12 had additional head scans. Two children died, and one was autopsied. Three children had intracranial pressures monitored during spinal surgery. All children had Type I and II Arnold-Chiari malformations, 58% had cord cavitation (syringo/hydromyelia), 67% had cord tethering. The anatomic study showed extensive fibrosis and scarring of the upper cord and brain stem, b

More advanced Alzheimer's disease may be associated with a decrease in cerebrospinal fluid pressure.

In a recent article, elevated cerebrospinal fluid pressure (CSFP) consistent with very early normal pressure hydrocephalus (NPH), was found in a small subset of Alzheimer's disease (AD) patients (possible AD-NPH hybrids) enrolled in a clinical trial for chronic low-flow cerebrospinal fluid drainage. Also in the same study, was another interesting finding that merits further discussion: a substantial proportion of AD patients had very low CSFP. Based on the characteristics of these subjects, we hypothesize that more advanced AD may be associated with a decrease in CSFP. Reduced CSFP among a gro

Primary leptomeningeal melanoma in a child.

Primary malignant melanoma of the leptomeninges is a rare and aggressive tumor in children and accounts for less than 1% of all pediatric malignancies. Usually its symptoms include raised intracraneal pressure resulting from hydrocephalus secondary to tumoral obliteration of basal cisterns, but the passage of time from the initial symptomatology to diagnosis is frequently delayed. A 7-year-old male with primary leptomeningeal melanoma is reported. At the beginning, he presented ataxia and dysarthria followed by symptoms of raised intracranial pressure, complex partial seizures, progressive los

[A Case of Ventriculoperitoneal Shunt Dysfunction in an Adult Secondary to Constipation].

INTRODUCTION: Ventriculoperitoneal(VP)shunts function because of the pressure differential between the intracranial space and the peritoneal cavity. Although chronic constipation is often a cause of VP shunt dysfunction in children, it is not well recognized in adults. We present a case of shunt dysfunction that not only resolved after resolution of constipation but also resulted in overdrainage. CASE REPORT: A 28-year-old woman who had a VP shunt placed 11 years prior for hydrocephalus was referred to our department because of an enlarged ventricle diagnosed with computed tomography(CT). She

Incidental and Clinical Significance of Slit Ventricles in Fixed Pressure Valves.

INTRODUCTION: Slit ventricle syndrome (SVS) is a recognized delayed complication of cerebrospinal fluid (CSF) shunting in children. It had been linked to the use of low-pressure shunts and considered an argument for the use of programmable valves. In this study, we aim to assess the rate of SVS in children that were shunted using fixed-pressure valves. METHODOLOGY: This study is a retrospective cohort study that occurred in King Abdulaziz Medical City, Jeddah, which reviews 100 patients with a median age of 15.5 months that were shunted by using fixed pressure valves during the period from 20

3D Quantitative MRI: A Fast and Reliable Method for Ventricular Volumetry.

PURPOSE: Volumetry of cerebral ventricles is a far more sensitive measure for shunt-induced reduction of ventricular size than traditional 2-dimensional (2D) measures, such as Evans index. However, available ventricle segmentation methods are time-consuming, resulting in limited use in clinical practice. Quantitative MRI (qMRI) obtains objective measurements of physical tissue properties, enabling automatic segmentation of white and gray matter and intracranial cerebrospinal fluid. The aim of this study was to evaluate the reliability and processing time of both manual and manually corrected a

Linear vs volume measures of ventricle size: Relation to present and future gait and cognition.

OBJECTIVE: To compare the clinical utility of volume-based ratios with the standard linear ratio of Evans index (EI) by examining their associations with gait, cognition, and other patient and imaging variables. METHODS: From MRI scans of 1,774 participants in the Mayo Clinic Study of Aging, we calculated 3 ventricle size measures: Evan index (frontal horn width divided by widest width of skull inner table), total ventricular volume, and frontal horn volume as ratios of total intracranial volume. Gait was measured by a timed 25-foot walk and cognition by a composite of psychometric tests. We

Paradigm-shift: radiological changes in the asymptomatic iNPH-patient to be: an observational study.

BACKGROUND: Many radiological signs are known for the diagnosis of idiopathic normal pressure hydrocephalus (iNPH). However, there is little information about these signs in the pre-symptomatic phase. For pathophysiological investigative purposes we conducted a descriptive image analysis study on pre-symptomatic patients. METHODS: Patients that had contact with either the neurological or neurosurgical department of the university hospital Tuebingen from 2010 through 2016 with magnetic resonance images > 3 years before onset of symptoms, were included. The date of onset and severity of symptom

[Potentially reversible dementia].

Reversible dementia is rare and accounts for approximately 1.5% of all dementias. Systematic ancillary investigations aimed at detecting an infectious disease, an endocrine aetiology or a vitamin deficiency are rarely contributive, but remain relevant since reversible dementia may, very rarely, mimic Alzheimer-type dementia. Aetiological investigations are much better selected and contributory when they rely on a precise analysis of the clinical picture (past medical history, age of the patient, cognitive, psychiatric and behavioural symptoms, type of onset, and associated signs) and of cerebr

Fiberscopic fenestration and prophylactic stenting of periaqueductal cysts: technical report on three cases.

The authors report on three consecutive cases of periaqueductal cysts, causing non-communicating hydrocephalus, successfully treated with endoscopic fenestration and aqueductal stenting. Navigation and cyst fenestration were accomplished using a slim (1.1 mm outside diameter) optic fiberscope inserted via a pre-coronal-paramedian burr hole. Third ventriculostomy was also performed after cyst fenestration in two cases. Because of the lack of data regarding such lesions and the possible recurrence with simple fenestration, an aqueductal stent connected to a subcutaneous reservoir was placed, in

Endoscopic aqueductal stenting in the management of pediatric hydrocephalus.

OBJECTIVE: Pediatric patients with long-term shunts may experience specific complications related to the segregation of the supra- and infratentorial spaces along with different pressure regimens, leading to either mesencephalic syndromes during shunt dysfunction or isolated fourth ventricle (IFV). An accepted treatment to reestablish normal CSF pathways and reequilibrate the transtentorial pressures is endoscopic aqueductal stenting (EAS) to avoid restenosis. In the present paper, the authors studied children treated with EAS during the last decade for both IFV and obstructive hydrocephalus,

B-waves in cerebral and spinal cerebrospinal fluid pulsation measurement by magnetic resonance imaging.

OBJECTIVE: Noninvasive measurement of B-waves is possible by magnetic resonance (MR) imaging using echo planar imaging (EPI) sequences. In this study, the proportion of B-waves in the cerebrospinal fluid (CSF) of the spinal canal and in the aqueductus cerebri was evaluated under normal and pathologic conditions, respectively. The proportion of the influence of pulse and respiration on the CSF pulsations was estimated. METHODS: The spinal CSF was evaluated in 7 volunteers at 5 spinal levels (C1, C2/3, C 6/7, T5, and T12). Examination of the CSF frequencies at the aqueduct was performed in 14 v
