[Preoperative evaluation of translabyrinthine cholesteatomas by MRI].

PURPOSE: Hearing preservation is one of the major goals of surgical resection of invasive cholesteatomas. Patients were prospectively evaluated using a 3D-CISS MR acquisition in order to improve the detection of perilymphatic fistulae. MATERIALS AND METHODS: 16 patients (10 M, 6 F) presenting with a primary (1 case) or a secondary (post-otitis) middle ear cholesteatoma extending to the osseous labyrinth (as defined by HR-CT) were evaluated at MR (1.5 T Vision, Siemens). The 3D-CISS sequence (TR: 12.25 ms, TE: 5.90 ms, flip angle: 70 degrees, slice thickness: 1 mm, matrix: 307 x 512, FOV: 200)

Surgical anatomy of the singular nerve.

Rare cases of chronic persistent positional vertigo do not respond to physiotherapy. For the treatment of these cases Gacek suggested singular nerve neurectomy as a new surgical procedure. The aim of the present study was to investigate the surgical anatomy of the singular nerve in order to evaluate the exact topography. In 25 cadaver temporal bones the posterior ampullary ("singular") nerve was prepared. The topographical correlation between singular nerve, posterior semicircular canal, and the round window membrane was evaluated. The average length of the nerve was 4.2 mm, its diameter 0.6 m

Initial symptoms and retrospective evaluation of prognosis in Menière's disease.

Clinical studies on an initial symptom and a long-term course of vertigo and hearing impairment and retrospective evaluation of the prognosis were performed in Menière's disease. One hundred and fifty-one patients (67 males and 84 females) with Meniere's disease were treated in the Neuro-otological clinic, Kitasato University Hospital from 1990 to 1995. Ages ranged from 17 to 77 years (mean 47.3 years) at the onset of the disease when the first vertigo attack occurred. There were 106 (70.1%) in their 30s, 40s and 50s, and 28 (18.5%) aged 60 years or over. Seventy-eight patients visited the cli

Cranial nerve preservation after stereotactic radiosurgery of intracanalicular acoustic tumors.

We reviewed our initial stereotactic radiosurgery experience in 10 patients with intracanalicular acoustic tumors managed by radiosurgery during a 5-year period. These patients constitute 4.7% of acoustic tumor patients who underwent Gamma Knife radiosurgery during this period. Tumor volume stabilization was achieved in 8. Two patients had initial growth followed by delayed growth arrest. Preservation of preoperative hearing was achieved in all patients in the immediate postoperative period and in 8 of 10 at 1 year. No patient had developed facial or trigeminal nerve dysfunction at the last fo

Infralabyrinthine approach to vestibular neurectomy in Menière's disease.

The infralabyrinthine approach to vestibular neurectomy was performed in 9 patients with unilateral Menière's disease. According to the AAOO (1972) criteria, 7 of 9 cases were graded as class B and the remaining 2 cases as class C. Otherwise, according to the AAO-HNS (1985) criteria, 6 patients who could be followed over 2 years were all graded as 'complete' at the vertigo control. The compensation of the spontaneous vestibular signs was rapid in the first 2 postoperative weeks, though an occasional imbalance on movement persisted even 3 years after the operation. No specific caloric reaction

The visual vertigo syndrome.

Neuro-otological and posturography findings in 15 patients with visually induced vertiginous symptoms (visual vertigo, VV) are reported. Most patients were considered to have peripheral vestibular disorders but one had cerebellar degeneration and another a brainstem stroke. Five patients showed abnormally large body sway induced by full-field visual motion stimulation; including the 2 patients with CNS disease. Four of these 5 patients had additional strabismic symptoms (diplopia, squint surgery and ocular muscle weakness). VV may occur if patients with balance disorders show high visual field

Neuroma of the facial nerve masked by chronic otitis media.

The absence of facial twitching, weakness, or palsy makes the diagnosis of facial neuroma difficult. The authors report a case of neuroma of the horizontal portion of the facial nerve masked by the presence of a chronic ear. A woman with a long history of discharge and hypoacousia in her left ear presented with acute dizziness. Examination revealed grade 3 horizontal right nystagmus, left anacousia, and the appearance of an epitympanic cholesteatoma. Computed tomography (CT) was performed after the vestibular condition improved. The clinical diagnosis of chronic otitis media with cholesteatoma

Thalamic infarctions: differential effects on vestibular function in the roll plane (35 patients).

We determined the subjective visual vertical (SVV), ocular torsion (OT), skew deviation, and lateral head tilt in 35 patients with acute thalamic infarctions (14 paramedian, 17 posterolateral, and four anterior polar) and in five patients with mesodiencephalic hemorrhages to obtain the tonic effects on vestibular function in the roll plane. Eight of 14 paramedian infarctions had complete ocular tilt reaction (OTR) with contraversive head tilt, skew deviation, OT, and SVV tilt. The OTR was due to ischemia of the rostral midbrain tegmentum, including the interstitial nucleus of Cajal (INC), and

Late audiovestibular consequences of radical radiotherapy to the parotid.

It has been claimed that the inner ear is generally resistant to the effects of radiotherapy schedules commonly used for treating head and neck cancer. A study was made of 28 patients previously irradiated for parotid pleomorphic adenoma to assess the effect on the audiovestibular apparatus. The dose most commonly used was 50 Gy in 15 daily fractions over 20 days and the median interval between radiotherapy and assessment was 14 years. Of 28 patients 15 had significant hearing deficit on the irradiated side on audiometry and this was predominantly sensorineural in type. In addition seven of th

Evaluation of a three-stage management program for Menière's disease.

A three-stage program for management of Menière's disease, consisting of diuretics, endolymphatic sac surgery, and intratympanal installations of gentamicin was tested in a peripheral county without neurosurgical facilities during 1981-1985. All 106 patients in the program were admitted at least once to the only ENT department of the area. The authors believe that these cases represented the most serious part of a total incidence estimated to be about 6.5 times higher than what the present material represent. When evaluated at the end of the period according to the AAOO 1972 classification , o

Cochlear implant candidates: assessment with CT and MR imaging.

Eighty-seven patients with severe to profound hearing loss were evaluated for possible placement of a multichannel cochlear implant hearing device. After initial clinical screening, 42 patients underwent computed tomographic (CT) examination. Five of these patients were also examined with magnetic resonance (MR) imaging. Twenty-two patients received implants. CT of the middle and inner ear was normal in 24 patients (57.1%) and showed labyrinthine ossification in 12 (28.6%), cochlear or fenestral otosclerosis (or both) in four (9.5%), and congenital cochlear malformation in two (4.8%). Otoconia in health and disease. A review.

Mammalian otoconia are made up of inorganic calcium carbonate crystallites and organic substance, which is thought to be sulfated glycosaminoglycans and glycoprotein. Mouse otoconia are formed in situ, and the primitive otoconia assume spindle, trigonal, dumbbell, or quadrilobed forms. As the otoconia mature, they assume the typical barrel-shaped body with pointed tips formed by three surfaces. A variety of otoconial disorders have been reported. Otoconial degeneration can result from ototoxic drugs, infection, trauma, and aging. Abnormal otoconia are also known to occur as a result of genetic

[Mechanism of Dabugan Decoction in treatment of generalized anxiety disorder based on network pharmacology and experimental verification].

This study aims to explore the mechanism of Dabugan Decoction in the treatment of generalized anxiety disorder(GAD) based on network pharmacology, molecular docking, and animal experiments. Network pharmacology and molecular docking technology were used to obtain the possible targets and related signaling pathways of Dabugan Decoction in the treatment of GAD. The GAD rat model was established, and the corresponding drugs were given by gavage after randomization. After 28 days of continuous intervention, the anxiety state of rats was detected, and the pathological changes of the hippocampus wer

[Cogan's syndrome revealed by haemorrhagic glairous diarrhoea].

INTRODUCTION: Cogan's syndrome is defined by the combination of non syphilitic interstitial keratitis and inner ear dysfunction, similar to Meniere's disease. OBSERVATION: Cogan's syndrome was revealed by haemorrhagic glaireous diarrhoea in a 21 year-old woman whose medical history included Hirschsprung's disease. Cerebral MRI revealed viral-like bilateral neuritis of the eighth cranial nerve, never described in the literature before. The auditory and visual disorders regressed after treatment with corticosteroids. CONCLUSION: The early diagnosis of Cogan's syndrome permits the cure of the v
