The influence and enlightenment of five public health emergencies on public psychology since new century: A systematic review.

BACKGROUND: Since the 21st century, humans have experienced five public health emergencies: the severe acute respiratory syndrome (SARS), type A H1N1 influenza (H1N1), Middle East respiratory syndrome (MERS), Ebola virus disease (EVD), and the new coronavirus pneumonia (COVID-19). They caused a large number of casualties and a wider psychological crisis, which might cause severe consequences such as post-traumatic stress disorder and suicide. AIMS: To reveal the law of formation of public psychological crisis in public health emergencies, and draw lessons from it. To provide ideas for effecti

Effects of Blast-induced Neurotrauma on Pressurized Rodent Middle Cerebral Arteries.

Though there have been studies on the histopathological and behavioral effects of blast exposure, fewer have been dedicated to blast's cerebral vascular effects. Impact (i.e., non-blast) traumatic brain injury (TBI) is known to decrease pressure autoregulation in the cerebral vasculature in both humans and experimental animals. The hypothesis that blast-induced traumatic brain injury (bTBI), like impact TBI, results in impaired cerebral vascular reactivity was tested by measuring myogenic dilatory responses to reduced intravascular pressure in rodent middle cerebral arterial (MCA) segments fro

Preinjury statin use is associated with improved in-hospital survival in elderly trauma patients.

BACKGROUND: Recent evidence demonstrates improved survival for septic and bacteremic patients receiving 3-hydroxy-3-methyl-glutaryl-CoA reductase inhibitors (statins). Victims of severe traumatic injury share similar inflammatory cascades to those seen in septic patients. We hypothesized that elderly (aged >/=65 years) trauma patients might derive a similar outcome benefit from preinjury statin use. METHODS: Using the National Study on the Costs and Outcomes of Trauma database (collected from 69 hospitals) we conducted a retrospective observational cohort study of the effects of preinjury sta

Limb salvage of lower-extremity wounds using free gracilis muscle reconstruction.

An extensive series reviewing the benefits and drawbacks of use of the gracilis muscle in lower-extremity trauma has not previously been collected. In this series of 50 patients, the use of microvascular free transfer of the gracilis muscle for lower-extremity salvage in acute traumatic wounds and posttraumatic chronic wounds is reviewed. In addition, the wound size, injury patterns, problems, and results unique to the use of the gracilis as a donor muscle for lower-extremity reconstruction are identified. In a 7-year period from 1991 to 1998, 50 patients underwent lower-extremity reconstructi

Early risk stratification in traumatic brain injury: An analysis of MEWS and rSIG scores in patients with isolated head trauma.

BACKGROUND: Traumatic brain injury (TBI) is a major cause of trauma-related morbidity and mortality worldwide. Early identification of patients at risk of clinical deterioration is essential for optimizing emergency management. The Modified Early Warning Score (MEWS) and the reverse Shock Index multiplied by the Glasgow Coma Scale (rSIG) are simple, rapidly calculable tools that may assist clinicians in early prognostic assessment. This study aimed to evaluate the prognostic performance of MEWS and rSIG in predicting poor outcomes among patients with isolated head trauma. METHODS: This retros

Valproic acid treatment rescues injured tissues after traumatic brain injury.

BACKGROUND: No agents that are specifically neuroprotective are currently approved to emergently treat patients with traumatic brain injury (TBI). The histone deacetylase inhibitor, high-dose valproic acid (VPA) has been shown to have cytoprotective potential in models of combined TBI and hemorrhagic shock, but it has not been tested in an isolated TBI model. We hypothesized that VPA, administered after isolated TBI, will penetrate the injured brain, attenuate the lesion size, and activate prosurvival pathways. METHODS: Yorkshire swine were subjected to severe TBI by cortical impact. One hour

Isobaric bupivacaine for spinal anaesthesia in acute war injuries.

Twenty nine patients with acute war injuries of the lower limbs or perineum were anaesthetised with intrathecal bupivacaine. Eleven patients (38%) were shocked on arrival with systolic arterial blood pressures less than 90 mmHg; in three the blood pressure was unrecordable . All patients were resuscitated and ready for surgery within 30 minutes of arrival at the field hospital. Hypotension occurred two minutes after injection of bupivacaine in three of the eleven shocked patients, but was corrected with fluids and metaraminol without difficulty. Patients with a fever were found to be less like

Civilian arterial trauma of the upper extremity. An 11 year experience in 267 patients.

Two hundred ninety-eight arterial injuries in 269 upper extremities were reviewed. Penetrating agents accounted for 250 injuries (93 percent) and blunt trauma for 19 (7 percent). Fifty-nine axillary, 126 brachial, 65 radial, and 48 ulnar arteries were damaged. Twenty-six extremities had more than one artery injured. The initial vascular examination revealed no abnormalities or was equivocal in 16 percent of all patients and in 32 percent of those with axillary artery injuries. Adjacent upper extremity structures were injured in 195 limbs (73 percent). Resection and primary anastomosis (54 perc

A technical pitfall in resuscitative thoracotomy: Delayed mechanical myocardial injury from a narrow pericardiotomy.

BACKGROUND: Mechanical complications related to the configuration of the pericardiotomy during resuscitative thoracotomy (RT) have not been previously documented. No prior report has visually demonstrated a mechanism by which an insufficient pericardiotomy leaves a sharp margin that directly abrades the beating heart. CASE PRESENTATION: A 62-year-old man in traumatic cardiac arrest underwent left anterolateral RT and right thoracotomy for hemorrhage control. Pericardiotomy was performed anterior to the phrenic nerve, followed by internal cardiac massage. After temporary closure, he developed

Forward Hemorrhage Control During Prolonged Evacuation.

Uncontrolled hemorrhage remains a leading cause of early, potentially preventable death following traumatic injury. Contemporary multicenter data continue to demonstrate that fatalities from hemorrhagic shock cluster within the first hours after injury, reinforcing the operational principle that the critical timeline is from point of injury to initiation of hemostatic resuscitation rather than time to hospital arrival. Ongoing armed conflict, including the war in Ukraine, as well as broader preparedness for future disasters and mass casualty events, underscores the need for evidence-based, pra

[Forensic assessment of injuries to the subaxial part of the cervical spine column].

An analysis of the treatment outcomes of 65 patients with non-fatal subaxial cervical spine (CS) injuries was conducted. Injuries were classified according to the four-column biomechanical classification model for injuries to these structures (STSG, 2006). It was established that the most frequently injured structures are the anterior column of the C4-C7 vertebrae, where complicated compression, burst fractures, fracture-dislocations, and subluxations are most often observed. Uncomplicated fractures of the transverse processes, arches, and articular processes are commonly observed in the later

A Young Patient With Gunshot Wounds.

BACKGROUND: Cardiac herniation secondary to pericardial rupture is a rare but life-threatening complication of severe chest trauma. Prompt recognition is critical, as the initial imaging findings can be subtle, and delayed diagnosis often results in rapid clinical deterioration. OBJECTIVE: To highlight the importance of early diagnosis and intervention in cases of cardiac herniation following traumatic pericardial rupture, with a focus on recognizing key radiographic and computed tomography findings that can guide emergent management. CASE REPORT: A 20-year-old male presented after sustainin

Large supralevator haematoma: a dilemma in diagnosis and management.

This is a case of postpartum haemorrhage following vaginal delivery of a primigravida in her late 20s resulting from a supralevator haematoma. She delivered in a low-resource hospital setting, with a late diagnosis of severe pre-eclampsia and haemolysis, elevated liver enzymes and low platelet count (HELLP), subsequently developed traumatic postpartum haemorrhage and vaginal haematoma, and was referred to our hospital for further management. She was constantly monitored, and serial ultrasonography revealed an expanding paravaginal haematoma. CT, 4 hours after admission, showed a large ill-defi

Expansive Duraplasty - Simple Technique with Promising Results in Complete Cervical Spinal Cord Injury: A Preliminary Study.

BACKGROUND: Complete cervical spinal cord injury is devastating with the currently available treatment modalities offering no hope for improvement. Intrathecal pressure is raised following spinal cord injury due to injured and edematous spinal cord. Due to constraints of the thecal sac, this sets up a vicious cascade leading to further spinal cord injury. Durotomy and expansile duraplasty could potentially prevent this secondary spinal cord injury. The aim of our study is to assess the advantage of durotomy and expansile duraplasty in addition to spinal bony decompression and fixation for trau

Metaphors For Trauma: A Cross-Cultural Qualitative Comparison in Brazil, India, Poland, and Switzerland.

The concept of psychological trauma implies that people experiencing traumatic stress are wounded, thus relating to the metaphor of a physical injury. Although this notion is widely accepted by clinicians and researchers in mental health, there is evidence of a broad range of metaphorical idioms for extremely aversive experiences or catastrophic events across different cultures. In this ethnopsychological study, we aimed to investigate and contrast culturally shared metaphors for trauma among four distinct cultural groups: two indigenous communities (Pitaguary from Brazil, Adivasis from India)

Fatal Pneumococcus Sepsis after Treatment of Late Antibody-Mediated Kidney Graft Rejection.

Antibody-mediated rejection (ABMR) is a major cause of late renal allograft dysfunction and graft loss. Risks and benefits of treatment of late ABMR have not been evaluated in randomized clinical trials. We report on a 35-year-old patient with deterioration in renal function and progressive proteinuria 15 years after transplantation. Recurrent infections after a splenectomy following traumatic splenic rupture 3 years earlier had led to reduction of immunosuppression. Renal transplant biopsy showed glomerular double contours, 40% fibrosis/tubular atrophy, peritubular capillaritis, and positive
