Sickle Trait and Alpha Thalassemia Increase NOS-Dependent Vasodilation of Human Arteries Through Disruption of Endothelial Hemoglobin-eNOS Interactions.

BACKGROUND: Severe malaria is associated with impaired nitric oxide (NO) synthase (NOS)-dependent vasodilation, and reversal of this deficit improves survival in murine models. Malaria might have selected for genetic polymorphisms that increase endothelial NO signaling and now contribute to heterogeneity in vascular function among humans. One protein potentially selected for is alpha globin, which, in mouse models, interacts with endothelial NOS (eNOS) to negatively regulate NO signaling. We sought to evaluate the impact of alpha globin gene deletions on NO signaling and unexpectedly found hum

Duffy phenotype does not influence the clinical severity of sickle cell disease.

The red blood cell Duffy antigen receptor for chemokines serves as a sink for the clearance of chemokines such as interleukin-8 (IL-8) from the circulation. We analyzed the impact of the Duffy phenotype on sickle cell disease (SCD) severity and serum IL-8 levels in 15 Duffy-positive and 36 Duffy-negative sickle cell patients. There was no difference in clinical severity between Duffy-positive and Duffy-negative sickle cell patients. In asymptomatic sickle cell patients the upward deviation of mean serum IL-8 levels was significantly greater in Duffy-negatives (n = 20) than in Duffy-positives (

Middle cerebral artery blood velocity and cerebral blood flow in sickle cell disease.

To understand better the relationship between blood velocity measured by transcranial Doppler and cerebral blood flow measured by the 133Xe inhalation method, we examined 23 patients undergoing evaluation in the Comprehensive Sickle Cell Center at Columbia University. Blood velocity in the middle cerebral artery was directly related to cerebral flow (r = 0.77; p less than 0.05). A multivariate analysis in this sample made it possible to improve this correlation to account for more than 90% of the variability in cerebral blood flow by the use of transcranial Doppler measures of velocity and pul

A hematological survey of preschool children of the United Arab Emirates.

OBJECTIVE: Anemia is common worldwide, particularly in developing countries including states of the Arabian Peninsular. The purpose of this study was to produce a hematological profile of preschool national children of the United Arab Emirates (UAE). METHODS: From April 2000 to October 2000, a cross-sectional community clinic-based capillary blood survey was carried out on a convenience sample of 1-5-year-old Emirati children attending a Primary Health Care Center in Al-Ain, UAE. Those children with capillary hemoglobin (Hb) and mean corpuscular volume (MCV) values below predetermined cutoffs

Low and differential polygenic score generalizability among African populations due largely to genetic diversity.

African populations are vastly underrepresented in genetic studies but have the most genetic variation and face wide-ranging environmental exposures globally. Because systematic evaluations of genetic prediction had not yet been conducted in ancestries that span African diversity, we calculated polygenic risk scores (PRSs) in simulations across Africa and in empirical data from South Africa, Uganda, and the United Kingdom to better understand the generalizability of genetic studies. PRS accuracy improves with ancestry-matched discovery cohorts more than from ancestry-mismatched studies. Within

An Atypical Finding in a Patient With Acute Left Flank Pain.

Hemorrhagic adrenal infarction is a rare and potentially under-recognized cause of acute flank pain. We report the case of a 29-year-old woman presenting with acute left flank pain of two days' duration. Initial contrast-enhanced abdominal CT, including unenhanced and portal venous phases, was interpreted as normal. Retrospective review demonstrated subtle enlargement of the left adrenal gland with mildly reduced enhancement compared with the contralateral gland. Following clinical deterioration with fever, a repeat CT performed five days later showed marked enlargement of the left adrenal gla

Presentation of Bilateral Facial Paralysis in Melkersson-Rosenthal Syndrome.

Introduction. Melkersson-Rosenthal syndrome (MRS) is a neuromucocutaneous disorder characterized by the following classic symptom triad: peripheral facial paralysis, orofacial edema, and scrotal or fissured tongue. It is rare, and since most of the patients are oligo- or monosymptomatic, it makes it difficult to diagnose. Clinical Case. We present a 26-year-old male patient with a history of sickle cell trait, untreated snoring, and left peripheral facial paralysis when he was 11 years old. This was an overall 20-day clinical profile that started with left peripheral facial paralysis, which wa

Renal medullary carcinoma: A national analysis of 159 patients.

BACKGROUND: Renal medullary carcinoma (RMC) is an aggressive malignancy seen predominantly in young males with sickle cell trait. RMC is poorly understood, with fewer than 220 cases described in the medical literature to date. We used a large national registry to define the typical presentation, treatments, and outcomes of this rare tumor. METHODS: The National Cancer Database was queried for patients under 40 years of age diagnosed with RMC from 1998 to 2011. An analysis of patient and tumor characteristics, treatment details, and overall survival (OS) was undertaken, and factors associated

Predictive role of PD-L1 expression in the response of renal Medullary carcinoma to PD-1 inhibition.

BACKGROUND: Renal medullary carcinoma is one of the rarest malignancies arising from the kidney. Despite various aggressive therapeutic regimens, mortality remains significantly high (95%) with a median overall survival of 5 months. Furthermore, the scarcity of this malignancy renders randomized clinical trials impossible. We examined the expression of programmed death ligand 1 (PD-L1) in two new renal medullary carcinoma cases, investigated their responses to the PD-L1 inhibitor nivolumab and explored the predictive role of the rate of PD-L1 expression in such response. Management and outcomes of patients with renal medullary carcinoma: a multicentre collaborative study.

OBJECTIVE: To describe the management strategies and outcomes of patients with renal medullary carcinoma (RMC) and characterise predictors of overall survival (OS). PATIENTS AND METHODS: RMC is a rare and aggressive malignancy that afflicts young patients with sickle cell trait; there are limited data on management to date. This is a study of patients with RMC who were treated in 2000-2015 at eight academic institutions in North America and France. The Kaplan-Meier method was used to estimate OS, measured from initial RMC diagnosis to date of death. Cox regression analysis was used to determi

Applying personal genetic data to injury risk assessment in athletes.

Recent studies have identified genetic markers associated with risk for certain sports-related injuries and performance-related conditions, with the hope that these markers could be used by individual athletes to personalize their training and diet regimens. We found that we could greatly expand the knowledge base of sports genetic information by using published data originally found in health and disease studies. For example, the results from large genome-wide association studies for low bone mineral density in elderly women can be re-purposed for low bone mineral density in young endurance a

Frequencies of some human genetic markers and their association with Plasmodium falciparum malaria in the Niger Delta, Nigeria.

BACKGROUND & OBJECTIVES: There is paucity of information on the association between Plasmodium falciparum malaria and some human genetic markers in the Niger Delta region of Nigeria. Hence, a study was undertaken in children to assess the current level of subclinical malaria due to P. falciparum. METHODS: Blood groups ABO and Rhesus factor, haemoglobin electrophoretic pattern, G-6-PD deficiency status and malaria were determined among 240 apparently healthy children in a crosssectional descriptive study using standard procedures. RESULTS: The prevalence of P. falciparum malaria in this regio

Characteristics of African blood.

Genetic markers in people of African ancestry and tables comparing Africans and Europeans are compiled to illustrate the blood differences. The existence of the African genetic characters, R0 (cDe), D-u and Ee (e-s, ce-s) in an Rh-Hr system are discussed. The high incidence of R0 (81.9%) in Africans is evident. Studies of ABO and the sickle cell trait in East Africa reveal the African tribes responsible for the original populating of Africa. The pygmoids of Uganda and Zaire, the Nilotes of West Nile, and the Bantu of Uganda are possibly responsible. Subgroups of A antigen in Africans and Europ

DICER1-mutated renal neoplasia: A series of 5 cases demonstrating the spectrum of cystic nephroma, Wilms tumor, and anaplastic sarcoma.

OBJECTIVES: The DICER1 gene encodes a ribonuclease essential for microRNA processing and regulates diverse cellular functions. Germline DICER1 alterations underlie DICER1-related tumor predisposition syndrome, which can lead to renal neoplasia. Herein, we studied the spectrum of DICER1-altered renal neoplasia. METHODS: Our institutional archives were queried for DICER1-mutated renal neoplasia. Relevant clinicopathologic features and molecular profiling results were summarized. RESULTS: We report five renal tumors that reinforce the clinicopathologic spectrum of DICER1-associated renal neopla

Glycated albumin and fructosamine do not improve accuracy of glycaemic control assessment in patients with conditions reported to affect HbA1c reliability.

AIMS: HbA1c testing in African populations may be limited due to high prevalence of hemoglobinopathies, anaemia, malaria and renal impairment. We aimed to assess the performance of glycated albumin (GA) and fructosamine in comparison to HbA1c for determining glycaemic control in Africans living with type 2 diabetes. METHODS: We compared the relationship between fructosamine, GA, and HbA1c with mean continuous glucose monitoring (CGM) glucose and assessed the impact of sickle cell trait (SCT), anaemia and renal impairment on the relationship between each measure and CGM glucose. RESULTS: The

Evaluating the Causal Link Between Malaria Infection and Endemic Burkitt Lymphoma in Northern Uganda: A Mendelian Randomization Study.

BACKGROUND: Plasmodium falciparum (Pf) malaria infection is suspected to cause endemic Burkitt Lymphoma (eBL), but the evidence remains unsettled. An inverse relationship between sickle cell trait (SCT) and eBL, which supports that between malaria and eBL, has been reported before, but in small studies with low power. We investigated this hypothesis in children in a population-based study in northern Uganda using Mendelian Randomization. METHODS: Malaria-related polymorphisms (SCT, IL10, IL1A, CD36, SEMA3C, and IFNAR1) were genotyped in 202 eBL cases and 624 controls enrolled during 2010-2015
