BACKGROUND: Anterolateral soft tissue impingement of the ankle joint is a common consequence of ankle sprain due to excessive supination and adduction of the foot, injuries to the tibiofibular syndesmosis and lateral malleolus fractures.
MATERIAL AND METHODS: Twenty-two arthroscopic procedures to treat anterolateral soft tissue impingement of the ankle joint were performed at the Independent Public Regional Hospital of Trauma Surgery in Piekary Slaskie between 2006 and 2007. The study group included male patients at the mean age of 34 (17 to 55) years. Medical histories revealed ankle sprain

BACKGROUND AND PURPOSE: To some extent, favorable treatment outcomes for physical therapy intervention programs depend on patients attending their clinic appointments and adhering to the program requirements. Previous studies have found less-than-optimal levels of clinic attendance, and a viable option might be physical therapy intervention programs with a large component of home treatment. This study investigated the effects of a standard physical therapy intervention program--delivered primarily at either the clinic or home--on ankle function, rehabilitation adherence, and motivation in pati

OBJECTIVE: To clarify the guidelines and responsibilities of expert witnesses and independent medical evaluators (IMEs).
DESIGN: Literature review and personal opinion.
SETTING: There have been concerns about the objectivity of expert witnesses and IMEs due to potential financial conflicts of interest.
RESULTS: Medical-legal work such as expert witness testimony and independent medical evaluations are a recognized part of the practice of medicine. As such, the opinions and testimony of expert witnesses and IMEs should be held to the same scientific and ethical standards as clinical practice

Ankle sprain is a common acute soft-tissue injury that often results in pain, inflammation, and ecchymosis. In this multicenter, double-blind, randomized parallel-group study, 445 adult patients received celecoxib 400 mg/day, ibuprofen 2,400 mg/day, or placebo for 10 days. Patients had experienced grade 1 or 2 ankle sprains within 48 hours and had moderate to severe ankle pain. Patient's Global Assessment of Ankle Injury responses, given on days 4 and 8, showed that the celecoxib group improved significantly more than the placebo group did, with 67% of the celecoxib group versus 55% of the pla

We treated 52 patients with impingement of the anterolateral soft tissues of the ankle by arthroscopic debridement. All had a history of single or multiple inversion injuries, without instability. One half had negative stress radiographs (stable group), while the others were positive (unstable group). Their mean age was 31 years and there were 35 men and 17 women. The results were assessed at a mean follow-up of 30 months. Three patients (6%) had a fair result, while 49 (94%) had an excellent or good outcome. No difference was found in the final results between the two groups (p > 0.05). We co

Thirty-four patients with the tarsal tunnel syndrome were treated by decompression of the posterior tibial nerve. The condition was bilateral in 3 cases. There were 9 men and 25 women with an average age at operation of 41 years. The average follow up was for 3.8 years. Multivariate analysis showed that the outcome is influenced, in order of importance, by fibrosis around the nerve, the preoperative severity of the condition, a history of sprained ankle, worker's compensation, a long history, and heavy work. The results were favourable when there was a short history, the presence of a ganglion

The ankle joints of 14 healthy volunteers and 16 patients with unstable ankle joints were tested regarding their functional and proprioceptive capabilities. All of them were active athletes. Three tests were used of the study: single-leg stance test, single-leg jumping course test, angle-reproduction test. The influence of three stabilization devices (lace-on-brace/"Mikros", stirrup-brace/"Aircast", taping) on the proprioceptivity of stable and unstable ankle joints was evaluated. The scores of the single-leg jumping course without any stabilizing device (category "standard") ranged between 8.

A postal survey was carried out of the 90 most accessible climbing walls in England, Scotland and Wales to determine the incidence and nature of injuries requiring emergency treatment associated with their use. Over a two year period, representing 1.021 million visits to the 56 walls used by more than 30 climbers per week, 55 significant injuries were recorded. The rate of injury was not related to any identified design or safety feature of the walls, although upper limb injuries were proportionally more common in walls which provided thinner fixed landing mats rather than thicker, moveable cr

Acceleration forces in bungee jumping acting on the head are different in nature and extent from those in merry-go-round, looping and scooter rides. They act mainly in the vertical plane, horizontal accelerations may develop only during uncontrollable vibrations in different directions after slowing down. According to our present knowledge the risks for injuries of the cervical spine and functional disorders of the inner ear in bungee jumping are lower than in merry-go-round, looping and scooter rides. They seem to be enhanced, however, in individuals suffering from diseases of the cervical sp

Despite international concern about the safety of trampolines, they have become increasingly popular in New Zealand. While internationally attention has centred on a relatively few cases of catastrophic cervical spine injury, little research effort has been directed at placing these incidents in a wider context. To redress this, a descriptive epidemiological study of trampoline-related injury in New Zealand was undertaken. National hospitalization and mortality data for a 10-year period revealed 2098 hospitalizations and two deaths. The incidence rate for hospitalizations increased from 3.1 pe

Proper primary care of the injured hand minimizes both temporary and permanent disability. It is essential that the physician undertaking the primary management and treatment of an injured hand appreciate the problem and understand the available procedures and techniques. Thoughtful examination of the injured part, noting loss of voluntary motion or any absence of sensation, and adequate x-ray studies will assist in outlining the appropriate immediate care. By carefully and thoroughly debriding the wound, restoring bony anatomical alignment and obtaining satisfactory skin cover, the stage is s

BACKGROUND: Individuals with chronic ankle instability typically present with abnormal gait patterns favoring the lateral foot. This gait pattern may alter cartilage stress potentially increasing the risk of osteoarthritis development, thus exploring this relationship may provide insights for early interventions. The purpose of this study was to examine the relationship gait biomechanics and talar articular cartilage characteristics.
METHODS: Talar articular cartilage was assessed with ultrasound at rest and after walking for 30-min in twenty-five adults (14 females, 22.6 ± 3.12 years, 168.12

BACKGROUND AND PURPOSE: American football generates the most sports-related injuries in the United States, with tackling as the leading injury mechanism. Overall injury rate at the collegiate level has been reported as 8.61 per 1,000 athlete exposures (AEs) - twice the rate of high school levels; competition injury rates are reported as high as 36.94/1000 AEs. Traumatic hip dislocation is an uncommon injury typically arising from high-energy axial impact with only 2-5.5% occurring during sports activities.
CASE DESCRIPTION: A 22-year-old NCAA Division I football defensive back who experienced
