Incubating a Fisherman's Intervention Idea: An Ergonomic Evaluation of Worker Modified Knives Used for Scalloping.

OBJECTIVES: This study aimed to characterize muscular load during real-world commercial scallop shucking in field settings. The study also evaluated the effects of a fisherman-led ergonomic intervention on reducing muscular load and perceived fatigue among professional scallopers during their regular work. METHODS: A repeated-measures, field-based study was conducted with 20 commercial scallopers in the Northeast U.S. Participants performed scallop shucking using both a standard stock knife and a modified (i.e. taped-handle) knife during typical work shifts, with each condition lasting approx

Phased Traumatic Stress Responses Among Caregivers of Children and Adults Recently Diagnosed with Acute Leukemia: A Grounded Theory Study.

A diagnosis of acute leukemia (AL) represents a sudden, life-threatening event that places family caregivers (FCs) at high risk for traumatic stress. While traumatic stress symptoms have been documented among FCs later in the cancer trajectory, little is known about how these responses unfold during the immediate peri-diagnostic period, when acute stress disorder (ASD) may emerge, and early intervention could be most impactful. We conducted a qualitative study using a constructivist grounded theory approach to examine early traumatic stress responses among FCs of adults and children with newly

Interlimb differences in knee joint loading and stress distribution following anterior cruciate ligament reconstruction during stair descent.

BACKGROUND: Despite anterior cruciate ligament reconstruction (ACLR), many patients exhibit persistent interlimb biomechanical differences during high-demand tasks like stair descent, which may contribute to post-traumatic osteoarthritis. However, it remains unclear how these movement adaptations translate into altered internal tissue loading within the knee joint. METHODS: This study therefore integrated motion capture, surface electromyography (EMG), musculoskeletal modeling, and subject-specific finite element (FE) analysis in 34 individuals following unilateral ACLR participated in this s

Implementing Indicator Condition-Guided HIV Screening Alerts in the Emergency Department: Lessons Learned from a Case Study.

IntroductionIndicator condition (IC)-guided HIV testing is recommended internationally as a cost-effective strategy but remains inconsistently implemented, particularly in emergency departments (EDs). Electronic health record (EHR) alerts may improve adherence; however, real-world uptake depends on organizational and specialty-level determinants.MethodsProspective case study conducted between August 2023 and April 2025. A non-interruptive EHR alert was triggered by predefined IC-related laboratory orders and antimicrobial prescriptions. Crucially, the alert was only fired if HIV serology was n

Deep learning for multimodal brain tumor segmentation: Architectures, fusion, robust learning, and deployment perspectives.

Accurate segmentation of brain tumors from multimodal magnetic resonance imaging (MRI) is essential for diagnosis, treatment planning, radiotherapy targeting, and longitudinal assessment. Deep learning has advanced this task through convolutional neural networks, Transformers, state-space models, diffusion methods, and foundation models. However, strong benchmark performance does not guarantee clinical reliability because systems remain vulnerable to missing or degraded modalities, cross-center shift, and subregion-specific failures, particularly in enhancing tumor (ET) and tumor core (TC). Test-retest reliability of pre-cue and anticipatory alpha activity in visual spatial attention.

Alpha-band activity over the parietal-occipital cortex is a canonical neural marker of visual spatial attention. However, the ongoing debate surrounds whether this activity represents as an active mechanism in gating visual information processing or if it merely reflects an epiphenomenal consequence of anticipatory attentional shifts. Despite this debate, the temporal stability of alpha activity in visual spatial attention, an essential aspect for this discussion, remains ambiguous. Notably, our recent findings highlighted the significant impact of pre-cue alpha power on anticipatory alpha act

Prefrontal cortical microglial transcriptome relates to mouse offspring executive function deficits after perinatal opioid exposure in a sex-dependent manner.

Opioid use during pregnancy affects over 7% of pregnancies in the United States. While efforts have been directed at mitigating effects of prenatal opioid exposure acutely in the neonatal period, long-term neurodevelopmental studies in humans remain challenging. Using a preclinical model, we previously found that perinatal morphine (MO) exposure induces sex-dependent executive function deficits in adult offspring, and sexually divergent shifts in microglia phenotype. Therefore, this study used transcriptional profiling to test whether perinatal MO exposure would cause sex-specific transcriptio

From focus to function: Longitudinal insights into infant attention and emerging executive functions via remote webcam eye tracking.

Growing evidence suggests that infant attention may predict subsequent cognitive outcomes. However, prior studies have predominantly tested small samples of infants in tightly controlled laboratory settings that differ from the complex, visually rich environments that infants experience in their day-to-day lives. The present study addresses this gap by measuring infant sustained attention in the home using novel remote webcam eye tracking methodology. A large, demographically diverse sample of 3- to 12-month-old infants (N = 160; 49% = female; 65% from low- to extremely low-income households;

Daily Laryngeal Kinematics and Acoustics Throughout the Menstrual Cycle: A Longitudinal Case Study.

OBJECTIVE: Previous investigations demonstrated voice changes corresponding to menstrual cycle phases, but few explored longitudinal cycle-to-cycle variations. The aim of this longitudinal case study was to describe changes in acoustics, laryngeal kinematics, vocal effort, and ability to produce high soft phonation in relation to menstrual cycle phases over the course of multiple cycles in a single normally cycling individual. METHODS: Data were obtained from a 34-year-old professional voice user who had self-collected daily videostroboscopy, acoustics, vocal effort rating, and high soft phon

Cognitive functioning and intra-individual variability in essential tremor and Parkinson's disease: A comparative study.

Objective: Cognitive decline may be more prevalent in essential tremor (ET) than previously recognized. Dispersion-based intraindividual variability (IIV-D) is proposed as a marker of neurological integrity. While elevated IIV-D has been noted in Parkinson's disease (PD), it has not been explored in ET. This study investigated cognitive functioning and IIV-D in ET compared to PD and cognitively intact controls (IC). Method: Neuropsychological data from 188 PD, 53 ET, and 135 IC patients were reviewed. Group comparisons on neuropsychological tests were examined. IIV-D was calculated via the int

Estimating Functional Brain Networks by Low-Rank Representation With Local Constraint.

The functional architecture undergoes alterations during the preclinical phase of Alzheimer's disease. Consequently, the primary research focus has shifted towards identifying Alzheimer's disease and its early stages by constructing a functional connectivity network based on resting-state fMRI data. Recent investigations show that as Alzheimer's Disease (AD) progresses, modular tissue and connections in the core brain areas of AD patients diminish. Sparse learning methods are powerful tools for understanding Functional Brain Networks (FBNs) with Regions of Interest (ROIs) and a connectivity ma

Head kinematics of human subjects during laboratory-induced ladder falls to the ground.

INTRODUCTION: Fall-induced traumatic brain injury (TBI) is considered one of the most serious occupational injuries in construction. Given the frequency of falls from ladders, knowledge of head kinematics during ladder falls to the ground may help inform any potential improvement to construction safety helmet design and improve their protection against head injury. Therefore, the goal of this descriptive study was to measure head kinematics during laboratory-induced ladder falls to the ground. METHOD: Eighteen young adults wearing a hockey helmet simulated construction tasks that challenged t

Testing Theory-Based Messages to Encourage Women at Average Risk for Breast Cancer to Consider Biennial Mammography Screening.

BACKGROUND: The US Preventive Services Task Force does not recommend routine annual mammography screening for women aged 40-49 at average risk. Little research has been done to develop theory-based communication interventions to facilitate informed decision-making about reducing potentially low-value mammography screening. PURPOSE: Evaluate the effects of theory-based persuasive messages on women's willingness to consider delaying screening mammography until age 50 or have mammograms biennially. METHODS: We conducted a randomized controlled communication experiment online with a population-b

Interprofessional Learning and Psychiatric Expertise in Mental Health Courts.

BACKGROUND: Interprofessional collaboration is crucial to reduce overincarceration of people with severe mental illness. Learning how to collaborate occurs in 2 complementary ways. One model emphasizes cognitive tasks: becoming familiar with the values and knowledge of other disciplines. Another model emphasizes practical interactive skills: calibrating one's preexisting expertise to the demands of the local workplace. This qualitative study assesses the 2 models in the case of psychiatrists in a multidisciplinary mental health court who learned to divert people with psychiatric disease from j

Enhancing Diagnostic Decision-Making: Ensemble Learning Techniques for Reliable Stress Level Classification.

An intense level of academic pressure causes students to experience stress, and if this stress is not addressed, it can cause adverse mental and physical effects. Since the pandemic situation, students have received more assignments and other tasks due to the shift of classes to an online mode. Students may not realize that they are stressed, but it may be evident from other factors, including sleep deprivation and changes in eating habits. In this context, this paper presents a novel ensemble learning approach that proposes an architecture for stress level classification. It analyzes certain

Predicting neurobehavioral performance of resident physicians in a Randomized Order Safety Trial Evaluating Resident-Physician Schedules (ROSTERS).

OBJECTIVES: Mathematical models of human neurobehavioral performance that include the effects of acute and chronic sleep restriction can be key tools in assessment and comparison of work schedules, allowing quantitative predictions of performance when empirical assessment is impractical. METHODS: Using such a model, we tested the hypothesis that resident physicians working an extended duration work roster, including 24-28 hours of continuous duty and up to 88 hours per week averaged over 4weeks, would have worse predicted performance than resident physicians working a rapidly cycling work ros

A global analysis of the determinants of maternal health and transitions in maternal mortality.

The reduction of maternal mortality and the promotion of maternal health and wellbeing are complex tasks. This Series paper analyses the distal and proximal determinants of maternal health, as well as the exposures, risk factors, and micro-correlates related to maternal mortality. This paper also examines the relationship between these determinants and the gradual shift over time from a pattern of high maternal mortality to a pattern of low maternal mortality (a phenomenon described as the maternal mortality transition). We conducted two systematic reviews of the literature and we analysed pub

'Steep learning curves' to 'Smooth Sailing': A reappraisal of telegenetics amidst the COVID-19 pandemic.

The COVID-19 pandemic has pushed medical providers to trial telemedicine on a scale that lacks precedent. In genetic medicine, nearly overnight genetics providers were asked to transition to telemedicine platforms, irrespective of their previous experience with these modalities. This push to telegenetics prompted a reappraisal of the practice, as genetics providers learned firsthand about the feasibility, benefits, and drawbacks of telegenetics and telesupervision, all of which raise questions about the potential incorporation of these platforms beyond the pandemic. Adding to nascent literatur
