Biceps to Triceps Transfer in Tetraplegic Patients: Our Experience and Review of the Literature.

Background: Management and indications for surgery in the tetraplegic patient are highly complex because of the substantial functional deficits that they present and their effect on their daily activity. Our purpose was to evaluate the functional outcome in tetraplegic patients who underwent biceps-to-triceps transfer surgery according to Zancolli's modified technique. Methods: This is a retrospective study of 6 biceps-to-triceps transfers using Zancolli's modified technique in 4 patients. Mean follow-up was 45 months. We evaluated each patient's DASH (Disabilities of the Arm, Shoulder and Han

Biomechanics of complex shoulder instability.

Identification and treatment of the osseous lesions associated with complex shoulder instability remains challenging. Further biomechanical testing is required to delineate critical defect values and determine which treatments provide improved glenohumeral joint stability for the various defect sizes, while minimizing the associated complications.

Spontaneous anterior interosseous nerve syndrome: clinical analysis of eleven surgical cases.

The aetiology of anterior interosseous nerve (AIN) syndrome and an optimal treatment strategy remain controversial. Eleven patients with spontaneous AIN syndrome, who were treated by surgical exploration, were reviewed at a mean of 32.5 months after the operation. Eight men and three women were included in the study. None of the patients had a history of trauma and there was no evidence of a neuropathy other than AIN syndrome. Six patients showed complete paralysis of the flexor pollicis longus (FPL) and the flexor digitorum profundus of the index finger (FDS1). Five patients had incomplete pa

Evaluation and treatment of rotator cuff tears.

Rotator cuff injuries are common problems and a frequent reason for patients to present to primary care physicians. These injuries are seen more frequently now with the aging population. These muscles allow for movement of the arm in overhead activities and controlled movements through space. A thorough physical examination can lead to the diagnosis of rotator cuff pathology. Radiographic imaging may offer some insight into the underlying pathology, and magnetic resonance imaging provides for excellent visualization of the rotator cuff. Many rotator cuff tears, especially partial tears, will s

Technical tip: Reconstruction of medial collateral ligament in correction of hallux valgus deformity with primary medial collateral ligamentous insufficiency.

In cases of hallux valgus deformity with primary medial collateral ligamentous insufficiency, there will be an abnormal hallux valgus angle with relatively normal intermetatarsal angle and sesamoid positions. Metatarsal osteotomies may not be effective to correct the deformity. Plication of the attenuated medial capsule may not be strong enough to provide long lasting correction of the hallux valgus deformity. We describe a minimally invasive technique of reconstruction of the medial collateral ligament by means of extensor hallucis brevis tendon graft. This can provide a stronger medial const

Anatomical variations of the human sural nerve and its role in clinical and surgical procedures.

The sural nerve is the most commonly nerve used in nerve transplantation, and so the aim of this study was to determine the variations of the sural nerve in the back of the leg, its relations to the calcaneal tendon and lateral malleolus, and determine the patterns of its distribution on the dorsum of the foot. Twenty-four Egyptian legs and feet were dissected. The results showed that the sural communicating nerve connected with the sural nerve in 87.5%. The predominant site of union between these two nerves was in the lower one-third of the leg and ankle region (62%). There was only one right

The relation of patient satisfaction and functional and cosmetic outcome after correction of the wrist flexion deformity in cerebral palsy.

Wrist flexion deformity in cerebral palsy is treated with flexor carpi ulnaris to extensor carpi radialis brevis transfer. The aim of the study was to assess the outcome of this procedure and analyse the determining factors for patient satisfaction. Fifteen patients were reviewed after a mean follow-up of 23 months. The functional and cosmetic outcome and patient satisfaction were evaluated using patient rated scales. There was a strong and significant correlation between the cosmetic outcome and patient satisfaction, but there was no significant correlation between functional improvement and

Pronator teres transfer to correct pronation deformity of the forearm after an obstetrical brachial plexus injury.

A pronation deformity of the forearm following an obstetric brachial plexus injury causes functional and cosmetic disability. We evaluated the results of pronator teres transfer to correct their deformity in 14 children treated over a period of four years. The mean age at surgery was 7.6 years (5 to 15). The indication for surgery in each case was impairment of active supination in a forearm that could be passively supinated provided that there was no medial contracture of the shoulder and normal function of the hand. The median follow-up was 20.4 months (8 to 42). No patient was lost to follo

A modification in technique of abductor digiti minimi (Huber) opponensplasty.

Abductor digiti minimi opponensplasty is a well-established method of restoring opposition following median nerve trauma or in congenital deficiency of thenar intrinsic musculature. The abductor digiti minimi is detached distally and rerouted through a subcutaneous tunnel for insertion into the area of the thumb metacarpophalangeal joint. The pivot point for this rotation is the pisiform bone and it is at this site that the ulnar nerve is vulnerable to compression by the transferred muscle. We describe a modification of technique which involves complete islanding of the muscle without re-inser

Magnetic resonance imaging after surgical transposition defines the anteroposterior location of the rectus muscle pulleys.

INTRODUCTION: Connective tissue pulleys serve as the functional origins of the rectus extraocular muscles (EOMs) and constrain the sideslip of the posterior EOM bellies after transposition surgery. Anterior to the pulleys, EOM paths appreciably displace to reach their transposed insertions. The inflection points in the EOM paths from minimal posterior displacement to maximal anterior displacement should define the anteroposterior location of the EOM pulleys after transposition. METHODS: Contiguous cross-sectional magnetic resonance images were obtained in planes perpendicular to the long axis

Arthroscopic posterior cruciate ligament reconstruction using a one-incision technique.

Thirty-seven patients with a posterior cruciate ligament injury underwent arthroscopic posterior cruciate ligament reconstruction using a one-incision technique with bone-patellar tendon-bone autograft or allograft. The tibial tunnel was started at the distal end of the graft donor site on the proximal tibia and exited posteriorly at the flat spot 15 mm below the articular margin and just lateral to the midline. The femoral tunnel was made through the lateral anterolateral portal. The 25 mm long proximal bone plug was passed easily through the tibial tunnel using a specially designed suture pu

Semitendinosus tenodesis for repair of recurrent dislocation of the patella in children.

Recurrent dislocation of the patella is more common in girls than in boys. Although several predisposing factors may exist, patellar dislocation is most commonly associated with familial ligamentous laxity. Many surgical repairs have been described to stabilize the patella. We have found the semitendinosus transfer to the patella to result in a predictable, stable patellofemoral joint without risk of injury to the proximal tibial physis. Between January 1990 and December 1997, 29 children have been treated at the Children's Hospital of Eastern Ontario with a semitendinosus transfer for recurre

[Results of brachial plexus surgery and replacement operations in traumatic brachial plexus birth injury-induced paralysis].

Since the development of microsurgical methods, the timing and concepts of surgery have significantly changed in obstetrical palsies. Because of the high axon regeneration potential in children, early and successful microsurgical reconstruction of the brachial plexus provides very encouraging results. Such successful nerve regeneration is the basis for secondary surgery which aims at complete restoration of function of the upper limb.
