[Early prediction of the injured regions in neonatal brain with hypoxic-ischemic encephalopathy by diffusion weighted imaging and measuring their apparent diffusion coefficient].

OBJECTIVE: To elucidate that diffusion weighted imaging (DWI) can be used to predict the injured regions of neonatal brain with hypoxic-ischemic encephalopathy (HIE) in the early phase of injury, and to measure the apparent diffusion coefficient (ADC) values in the multiple regions of the brain. METHOD: The participants in this study were twenty-six infants with HIE from neonatology ward hospitalized between July 2006 and July 2009. Nineteen patients had severe HIE, and seven had moderate HIE. DWI and conventional magnetic resonance imaging (MRI) were performed for each case within the first

Autoregulation-guided intracranial pressure and cerebral perfusion pressure targets in different subtypes and characteristics of traumatic brain injury.

BACKGROUND: Considering the significant heterogeneity of traumatic brain injury (TBI), uniform treatment thresholds applied to all patients may be overly simplistic. Cerebral autoregulation (CA) capacity may vary significantly between TBI patients and may change dynamically over time. Within a precision neurointensive care framework, this study aimed to evaluated the associations between cerebral perfusion pressure (CPP) or intracranial pressure (ICP) and the pressure reactivity index (PRx) in patients with different types of TBI, and to investigated whether PRx modified the associations betwe

Clonidine Protects Endothelial Cells from Angiotensin II-Induced Injury via Anti-Inflammatory and Antioxidant Mechanisms.

Background: Cerebral aneurysm (CA) is a focal or diffuse pathological dilation of the cerebral arterial wall that arises due to various etiological factors. It represents a serious vascular condition, particularly affecting the elderly, and carries a high risk of rupture and neurological morbidity. Clonidine (CL), an α2-adrenergic receptor agonist, has been reported to suppress aneurysm progression; however, its underlying molecular mechanisms, especially in relation to cerebral endothelial dysfunction, remain unclear. This study aimed to investigate the potential of CL to mitigate CA developm

Does routine serial computed tomography of the head influence management of traumatic brain injury? A prospective evaluation.

BACKGROUND: Computed tomography (CT) of the head is the current standard for diagnosing intracranial pathology following blunt head trauma. It is common practice to repeat the head CT to evaluate any progression of injury. Recent retrospective reviews have challenged the need for serial head CT after traumatic brain injury (TBI). This study intends to prospectively examine the value of routine serial head CT after TBI. METHODS: Consecutive adult blunt trauma patients with an abnormal head CT admitted to an urban, Level I trauma center from January 2003 to September 2003 were prospectively stu

Evaluation of Glymphatic System Activity in Pediatric Wilson Disease Patients Using Diffusion Tensor Imaging Along the Perivascular Space.

PURPOSE: Wilson disease is an autosomal recessive disorder of copper metabolism that results in toxic copper accumulation in the liver and brain, thereby causing neurological injury. Previous reports have described astrocyte dysfunction, oxidative stress, glial pathology, and impaired aquaporin-4-mediated water transport in Wilson disease, suggesting a potential disruption of glymphatic fluid dynamics. However, glymphatic function has not been quantitatively assessed in this population using diffusion tensor imaging analysis along the perivascular space (DTI-ALPS). METHODS: This retrospective

Structural Organization of the Corpus Callosum Predicts Attentional Shifts after Continuous Theta Burst Stimulation.

UNLABELLED: Repetitive transcranial magnetic stimulation (rTMS) applied over the right posterior parietal cortex (PPC) in healthy participants has been shown to trigger a significant rightward shift in the spatial allocation of visual attention, temporarily mimicking spatial deficits observed in neglect. In contrast, rTMS applied over the left PPC triggers a weaker or null attentional shift. However, large interindividual differences in responses to rTMS have been reported. Studies measuring changes in brain activation suggest that the effects of rTMS may depend on both interhemispheric and in

Modulation and distribution of extracellular free water and tract deficits in rhesus macaques before and after the initiation of emtricitabine + tenofovir disoproxil fumarate + dotutegravir treatment.

INTRODUCTION: Understanding the specific timing of cART initiation, its effectiveness, and failures, as well as assessing how well the current cART regimens control viral replication and rebound, enhance immune function, and repair or curb early injury in the central nervous system (CNS), is crucial to improving the livelihood of people living with HIV. METHODS: Here, we use an animal model to provide controlled environments to understand how the bodies of Chinese-origin rhesus monkeys, both the immune system and CNS, respond to a combination of emtricitabine (EMTBL/FTC), dolutegravir (DTG),

Broad-spectrum cation channel inhibition by LOE 908 MS reduces infarct volume in vivo and postmortem in focal cerebral ischemia in the rat.

Cation channels conduct calcium, sodium, and potassium, cations that are likely deleterious in the evolution of focal ischemic injury. Diffusion-weighted magnetic resonance imaging (DWI) is a powerful tool for evaluation of acute cerebral ischemia. We studied the effects of a novel, broad-spectrum inhibitor of several cation channels, LOE 908 MS, on acute ischemic lesion development with DWI and on cerebral infarct size using 2,3,5-triphenyltetrazolium chloride (TTC) staining postmortem. Eighteen male Sprague-Dawley rats underwent middle cerebral artery occlusion (MCAO) and were randomly and b

Establishment of an MR-Conditional Porcine Model for Real-Time Assessment of Cerebral Blood Flow During Extracorporeal Circulation.

BACKGROUND AND PURPOSE: Neurological injury remains a major complication of pediatric cardiac surgery and is closely related to alterations in cerebral blood flow during extracorporeal circulation (ECC). However, the real-time assessment of cerebral perfusion under these conditions has been limited by the lack of magnetic resonance (MR)-compatible perfusion systems. The aim of this pilot feasibility study was to establish a porcine model enabling simultaneous cardiopulmonary bypass (CPB) and real-time MR-based assessment of cerebral blood flow during simulated pediatric cardiac surgery. Cerebral blood flow in patients with diffuse axonal injury--examination of the easy Z-score imaging system utility.

To evaluate the utility of easy Z-score imaging system (eZIS) in 27 diffuse axonal injury (DAI) cases. Twenty-seven DAI patients were examined with an magnetic resonance imaging (MRI) T2* sequence and with eZIS (seven women, 20 men; age range, 19-35 years; median age: 26.6 years). In this investigation, we excluded patients who exhibited complications such as acute subdural hematoma, acute epidural hematoma, intracerebral hematoma, or brain contusion. We examined the neuropsychological tests and correlated with findings from MRI/eZIS. Furthermore, we evaluated the degree of ventricular enlarge

Outcomes and Mechanisms Associated With Selective Thalamic Neuronal Loss in Chronic Traumatic Brain Injury.

IMPORTANCE: The chronic neuronal burden of traumatic brain injury (TBI) is not fully characterized by routine imaging, limiting understanding of the role of neuronal substrates in adverse outcomes. OBJECTIVE: To determine whether tissues that appear healthy on routine imaging can be investigated for selective neuronal loss using [11C]flumazenil (FMZ) positron emission tomography (PET) and to examine whether this neuronal loss is associated with long-term outcomes. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study, data were collected prospectively from 2 centers (University of

Age, Multidomain Lifestyle Intervention, and White Matter Integrity: Secondary Analysis of the POINTER Randomized Clinical Trial.

IMPORTANCE: Multidomain lifestyle interventions slow cognitive decline in at-risk older adults, but effects on cerebrovascular injury and whether any effect differs by age or across magnetic resonance imaging (MRI) markers remain unclear. OBJECTIVES: To test whether a structured multidomain lifestyle intervention, vs a self-guided program, alters trajectories of cerebrovascular MRI markers and whether baseline diffusion MRI estimates subsequent vascular injury. DESIGN, SETTING, AND PARTICIPANTS: POINTER Imaging was a prespecified secondary imaging analysis of the US POINTER (US Study to Prot

White matter-cognition mapping in pediatric cancer survivors: a two-cohort NODDI-Bingham study.

UNLABELLED: Childhood cancer survivors (CS) treated with chemotherapy frequently experience long-term cognitive impairments, commonly referred to as CRCI. Whether these deficits reflect disrupted white matter microstructure, altered microstructure-cognition relationships, or both remains unclear. Here, we applied the NODDI-Bingham model, which quantifies neurite density (NDI), isotropic free water (ISO), and orientation dispersion along primary (ODIp), secondary (ODIs), and total (ODIt) fiber axes, to characterize biophysically specific white matter microstructure in CS and examine its relatio

Multi-tract multi-symptom relationships in pediatric concussion.

BACKGROUND: The heterogeneity of white matter damage and symptoms in concussion has been identified as a major obstacle to therapeutic innovation. In contrast, most diffusion MRI (dMRI) studies on concussion have traditionally relied on group-comparison approaches that average out heterogeneity. To leverage, rather than average out, concussion heterogeneity, we combined dMRI and multivariate statistics to characterize multi-tract multi-symptom relationships. METHODS: Using cross-sectional data from 306 previously concussed children aged 9-10 from the Adolescent Brain Cognitive Development Stu

Lattice-Tip Focal Ablation Catheter That Toggles Between Radiofrequency and Pulsed Field Energy to Treat Atrial Fibrillation: A First-in-Human Trial.

BACKGROUND: The tissue selectivity of pulsed field ablation (PFA) provides safety advantages over radiofrequency ablation in treating atrial fibrillation. One-shot PFA catheters have been shown capable of performing pulmonary vein isolation, but not flexible lesion sets such as linear lesions. A novel lattice-tip ablation catheter with a compressible 9-mm nitinol tip is able to deliver either focal radiofrequency ablation or PFA lesions, each in 2 to 5 s. METHODS: In a 3-center, single-arm, first-in-human trial, the 7.5F lattice catheter was used with a custom mapping system to treat paroxysm

Regional vulnerability of longitudinal cortical association connectivity: Associated with structural network topology alterations in preterm children with cerebral palsy.

Preterm born children with spastic diplegia type of cerebral palsy and white matter injury or periventricular leukomalacia (PVL), are known to have motor, visual and cognitive impairments. Most diffusion tensor imaging (DTI) studies performed in this group have demonstrated widespread abnormalities using averaged deterministic tractography and voxel-based DTI measurements. Little is known about structural network correlates of white matter topography and reorganization in preterm cerebral palsy, despite the availability of new therapies and the need for brain imaging biomarkers. Here, we combi
