Efficacy and Safety of GLP-1 RAs in Children and Adolescents With Obesity or Type 2 Diabetes: A Systematic Review and Meta-Analysis.

IMPORTANCE: Obesity affects 1 in 5 children and adolescents, increasing the risk of type 2 diabetes (T2D). Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are among the few pharmacotherapy options available for this population, necessitating a comprehensive evaluation of efficacy and safety. OBJECTIVE: To assess the efficacy and safety of GLP-1 RAs in children and adolescents (<18 years) with obesity, prediabetes, or T2D. DATA SOURCES: A systematic search was conducted in PubMed, Embase, and the Cochrane Central Register of Controlled Trials (CENTRAL) for randomized clinical trials (RC

A Longitudinal Multicenter Study Comparing the Effectiveness of Pharmacological and Lifestyle-Only Interventions in Adults With Prediabetes for Preventing Acute Hyperglycemic Crises and Progression to Type 2 Diabetes Mellitus.

BACKGROUND: Prediabetes is a high-risk metabolic state that, if not effectively managed, can progress to type 2 diabetes mellitus (T2DM) or acute hyperglycemic crises. Early intervention is, therefore, critical to reduce the burden of diabetes-related morbidity. OBJECTIVES: The primary objective of this study was to compare the long-term effectiveness of pharmacological therapy vs. lifestyle-only interventions in preventing the progression of prediabetes to T2DM. The secondary objectives were to assess the incidence of acute hyperglycemic crises, as well as the long-term adherence and metabol

Metformin Improves Insulin Sensitivity and Vascular Health in Youth With Type 1 Diabetes Mellitus.

BACKGROUND: Cardiovascular disease is the leading cause of mortality in type 1 diabetes mellitus (T1DM) and relates strongly to insulin resistance (IR). Lean and obese adolescents with T1DM have marked IR. Metformin improves surrogate markers of IR in T1DM, but its effect on directly measured IR and vascular health in youth with T1DM is unclear. We hypothesized that adolescents with T1DM have impaired vascular function and that metformin improves this IR and vascular dysfunction. METHODS: Adolescents with T1DM and control participants underwent magnetic resonance imaging of the ascending (AA)

Acinar cell carcinoma of the pancreas in childhood.

BACKGROUND: Advances in cancer immunotherapy have generated encouraging results in multiple malignancies refractory to standard chemotherapies. As the use of immune checkpoint inhibitors (ICI) proliferates, the incidence of autoimmune side effects associated with these agents, termed immune related adverse events (irAE), is expected to increase. The frequency of significant irAE in ICI treated patients is about 10-20% and early recognition is critical to prevent serious morbidity and even mortality. New onset autoimmune diabetes mellitus (DM) associated with immune checkpoint inhibitor treatme

The Effect of Early Response to GLP-1 RA Therapy on Long-Term Adherence and Persistence Among Type 2 Diabetes Patients in the United States.

BACKGROUND: Treatment adherence and persistence are crucial to achieve glycemic control in patients with type 2 diabetes (T2D). Early response to a new therapy may lead to improved treatment adherence and associated outcomes. OBJECTIVE: To assess the effect of early response to glucagon-like peptide-1 receptor agonist (GLP-1 RA) therapy, as indicated by reduced hemoglobin A1c (A1c) and body weight, on long-term adherence and persistence. METHODS: Adults aged ≥ 18 years with T2D initiated with GLP-1 RA therapy after January 1, 2010, were identified from the IBM Explorys Therapeutic Dataset. P

Epidemiology of Hypertension in Bangladeshi People with Type 2 Diabetes Attending an Urban Healthcare Center: Impact of the 2017 American College of Cardiology (ACC)/American Heart Association (AHA) Hypertension Clinical Practice Guidelines.

Background The American College of Cardiology (ACC) and the American Heart Association (AHA) lowered the threshold for defining hypertension (HTN) and for control status in 2017. The effect of the new cutoff on the prevalence of HTN and its burden among Bangladeshi people with type 2 diabetes mellitus (T2DM) has not been well studied. This study aimed to assess the prevalence and predictors of HTN, along with the blood pressure (BP) control status among Bangladeshi people with T2DM using the traditional cutoff (HTN: systolic BP (SBP) ≥140 mmHg and/or diastolic BP (DBP) ≥90 mmHg, controlled HTN

Short-term intensive insulin therapy could be the preferred option for new onset Type 2 diabetes mellitus patients with HbA1c > 9.

Type 2 diabetes mellitus (T2DM) is a heterogeneous disease. Currently, the typical clinical therapeutic pathway for the disease consists of the stepwise addition of antihyperglycemic preparations over time, followed lastly by insulin therapy when functional β-cell capacity is severely deteriorated. Recognizing the complexity of disease management, personalized (precision) medicine approaches may enable the physician to tailor diabetes treatment based on HbA1c levels, body mass index (BMI), efficacy, risk of hypoglycemia, risk of weight gain, age, safety, cost, and even genetic characteristics.
