Lymphocutaneous Nocardia brasiliensis infection acquired from a cat scratch: case report and review.

Nocardia brasiliensis is a bacterium that is most commonly found in the soil. Traumatic inoculation of N. brasiliensis into the skin is the most typical mode of acquisition of infection due to this organism. To the best of my knowledge, I report the first case of lymphocutaneous N. brasiliensis disease from a penetrating cat scratch of the skin, thereby establishing cats as vehicles for the transmission of this infection. Treatment with penicillin produced a rapid resolution of all signs and symptoms of infection. The efficacy of penicillin against the N. brasiliensis isolate recovered from th

Proliferation of marginal zone cells mimicking malignant lymphoma.

An enlarged axillary lymph node from a 58-year-old woman showed a proliferation of marginal zone cells in nodules or large band-forming aggregates within the cortex. The marginal zone cells also infiltrated the adjacent fatty tissue. They showed polytypic surface immunoglobulins (IgM++, IgG+, kappa+, lambda+). Their immunophenotype (IgD-, CD23-, KiB3-) differed from that of the small lymphocytes of the mantle zone. They were also positive for alkaline phosphatase. The lesion is reactive and must be differentiated from low-grade malignant lymphomas, especially centrocytic lymphoma.

[Malignant tumor-like abdominal lymphoma in Whipple's disease].

A 43-year-old man had severe upper abdominal pain and weight loss of 8 kg for over three months. He underwent a laparotomy because, computed tomography having revealed numerous mesenteric and para-aortic lymph nodes, a malignant lymphoma was suspected. Histological examination of a mesenteric lymph node demonstrated exclusively extraintestinal Whipple's disease. The symptoms completely disappeared after the administration of 1.2 mega U penicillin G and 1 g streptomycin daily for 14 days, followed by twice daily 160 mg trimethoprim and 800 mg sulphamethoxazole.

Fenestration of intraventricular cysts using a flexible, steerable endoscope and the argon laser.

Two infants with hydrocephalus and compartmentalization of the lateral ventricles due to cerebrospinal fluid infection were treated by fenestration of the ventricular cysts using the argon laser through a steerable flexible endoscope. Both children had undergone failed multiple shunting procedures that attempted to drain the lateral ventricular cysts. One child had chronic granulomatous disease and persistent Candida ventriculitis and meningitis. His infection resolved after removal of multiple shunt systems was made possible by endoscopic laser fenestration to convert a multilocular to a unil

Peritonsillar, peripharyngeal, and deep neck abscesses.

Abscesses of the peritonsillar and peripharyngeal regions typically present as a sore throat and as difficulty in swallowing. One of the most ?striking clinical features is difficulty in controlling saliva. Deep neck abscesses usually are secondary to disease in the pharynx, tonsils, or teeth. Initially, attention should be devoted to finding the primary source of infection. Treatment of these abscesses is incision and drainage.

Tetracyclic flavan enantiomers from Dianella ensifolia as inhibitors of CuSO4-induced neutrophil recruitment in zebrafish.

Dianella ensifolia (L.) DC. is a medicinal plant traditionally used externally for the treatment of skin infections, abscesses, lymphadenitis, traumatic swelling, and wounds. To characterize its chemical constituents and identify potential anti-inflammatory compounds, a systematic phytochemical investigation of the roots of D. ensifolia was conducted. Seventeen flavans were isolated and characterized, including two undescribed tetracyclic flavans, diaensifols A and B (1 and 2), which were resolved into two pairs of enantiomers, (+)-1/(-)-1 and (+)-2/(-)-2, five undescribed flavans (3-7), and t…

Chronic Active Epstein-Barr Virus Disease Causes Tubulointerstitial Nephritis and Growth Failure.

BACKGROUND: Chronic active Epstein-Barr virus (EBV) disease is a rare, potentially life-threatening condition characterized by the clonal pr … complications and died 3 months after the diagnosis. Her history was notable for recurrent pyogenic lymphadenitis at the age of 4 years, with a lymph node biopsy at that time remarkable for a positive EBV-encoded small ribonucleic acid in situ hybridization assay, thereby demonstrating the presence of a long-standing EBV infection. CONCLUSIONS: The present cas …

Anatomopathological aspects of osteoarticular lesions associated with Rhodococcus equi infection in two foals.

Rhodococcus equi is an important bacterium that affects foals up to six months of age, especially those that are immunocompromised. The main … tion with focal detachment of the epiphyseal plate. Both animals had pyogranulomatous pneumonia and lymphadenitis with extracellular and intracytoplasmic gram-positive coccobacilli in neutrophils and macrophages. In the first case, R. equi was isolated and confirmed by MALDI-TOF. R. equi infection should be considered in the differential diagnosis of osteoartic …

Pediatric Kikuchi-Fujimoto Disease: Case Report and Review of Cutaneous and Histopathologic Features in Childhood.

Kikuchi-Fujimoto disease (KFD) is a rare condition characterized by necrotizing lymphadenitis and fever, often associated with immune dysregulation. Histologically, it features necrotic foci with abundant histiocytes and plasmacytoid dendritic cells but notably lacks neutrophils and eosinophils. Recent evidence reveals a notable prevalence among pediatric patients, who may exhibit distinct features compared to adults. We reported the case of an 11-year-old girl presenting with persistent fever, cervical adenopathy, and a malar rash, leading to a diagnosis of KFD following lymph node biopsy, wh…

Overlap of Adult-Onset Still Disease and Kikuchi-Fujimoto Disease: A Case Report and Literature Review.

Kikuchi-Fujimoto disease (KFD) and adult-onset Still disease (AOSD) are two rare conditions whose association poses a significant diagnostic challenge. KFD is characterized by subacute necrotizing lymphadenitis of unknown etiology, primarily affecting young adults, and often presents with fever and posterior cervical lymphadenopathy. AOSD is a systemic inflammatory disorder of unclear origin, defined by high-spiking fever, lymphadenopathy, hepatosplenomegaly, hyperferritinemia, and leukocytosis. To date, only 15 cases of the coexistence of both conditions have been reported, providing valuable…
