Characterization of the median sacral artery course at the sacral promontory using contrast-enhanced computed tomography.

INTRODUCTION AND HYPOTHESIS: Laparoscopic sacral colpopexy (SC) is increasingly utilized in the surgical management of apical prolapse. It involves attachment of a synthetic mesh to the sacral promontory and to the prolapsed vaginal walls. The median sacral artery (MSA) runs close to the site of mesh attachment and is therefore prone to intraoperative injury, which may lead to profound hemorrhaging. The aim of this study was to determine the location of the MSA at the level of the sacral promontory with regard to adjacent visible anatomical landmarks. Surgeons may use this information to reduc

Determinants of aortic bioprosthetic valve calcification assessed by multidetector CT.

BACKGROUND: Cusp calcification is the main mechanism leading to bioprosthetic heart valve (BPV) failure. Recent studies suggest that BPV calcification is an active rather than passive process probably modulated by several mechanisms including lipid-mediated inflammation and dysfunctional phosphocalcic metabolism. OBJECTIVE: To identify the clinical and metabolic determinants of BPV calcification assessed by multidetector CT (MDCT). METHODS AND RESULTS: Presence of BPV calcification was assessed by MDCT in 194 patients who had undergone aortic valve replacement. A calcification score was indi

Severe hemoptysis: From diagnosis to embolization.

Severe hemoptysis is life-threatening to patients because of the asphyxia it causes. The diagnosis and treatment are therefore urgent and chest imaging is essential. Multidetector CT-angiography provides an exhaustive non-invasive assessment which includes localization, mechanisms, causes and severity of the hemoptysis. It is an invaluable step in preparation for endovascular treatment which is the first line invasive therapy, particularly with bronchial arteriography embolization in the majority of cases (over 90%) and erosion or rupture of the pulmonary artery in less than 10% of cases. The predictive value of multidetector CTA on outcomes in patients with below-the-knee vascular injury.

BACKGROUND: Multidetector computed tomographic angiography (MDCTA) has become the gold standard for the early assessment of lower extremity vascular injury. The objective of this study was to evaluate the predictive value of MDCTA documented vessel run-off to the foot on limb salvage rates after lower extremity vascular injury. METHODS: All trauma patients undergoing lower extremity MDCTA for suspected vascular injury assessed at 2 high-volume Level I trauma centers between January 2009 and December 2012. Demographics, clinical data and outcomes (compartment syndrome requiring fasciotomy and

Colonic perforation by a transmural and transvalvular migrated retained sponge: multi-detector computed tomography findings.

Transmural migrated retained sponges usually impact at the level of the ileo-cecal valve leading to a small bowel obstruction. Once passed through the ileo-cecal valve, a retained sponge can be propelled forward by peristaltic activity and eliminated with feces. We report the case of a 52-year-old female with a past surgical history and recurrent episodes of abdominal pain and constipation. On physical examination, a generalized resistance was observed with tenderness in the right flank. Contrast-enhanced multi-detector computed tomography findings were consistent with a perforated right colon

Tissue microscopic changes and artifacts in multi-phase post-mortem computed tomography angiography in a hospital setting: a fatal case of systemic vasculitis.

A 27-year-old man suddenly died in hospital of acute respiratory distress syndrome secondary to severe systemic vasculitis. Multi-phase post-mortem computed tomography angiography followed by scientific autopsy of the thoracic and abdominal cavity and histology was performed, illustrating the advantages and drawbacks of such techniques. Imaging enabled us to examine the cranium, as the family refused cerebral dissection. MPMCTA revealed absence of opacification of the left middle cerebral artery. But parenchymal findings of thoracic and abdominal organs were still difficult to interpret after

Radiologic evaluation of right ventricular outflow tract myxomas.

A 22-year-old man was referred for palpitations. On transthoracic echocardiography, he was found to have a right ventricular outflow tract mass. Further cardiac imaging was conducted by means of transesophageal echocardiography, computed tomography, and cardiac magnetic resonance. Complete surgical resection of the tumor was achieved, and pathologic examination revealed the lesion to be a myxoma. Cardiac tumors located in the right ventricular outflow tract are rare and can present unusual diagnostic and therapeutic challenges. Cardiac computed tomography and magnetic resonance are becoming mo

Impact of multidetector CT-angiography on the emergency management of severe hemoptysis.

BACKGROUND: Multidetector CT-angiography (MDCTA) is commonly used in patients with severe haemoptysis requiring admission to intensive care unit. However, the impact of MDCTA on the management of severe haemoptysis in emergency setting is poorly evaluated. METHODS: We prospectively compared data provided by clinical bedside evaluation (clinical examination, chest-X-ray and fiberoptic bronchoscopy) to MDCTA data in terms of lateralization, location of the bleeding site, etiology as well as impact on the treatment choice. RESULTS: Over a 13-month period, 87 patients (men n=58, median age=61 ye

Prognosis of patients with mild-moderate coronary artery stenosis detected by coronary computed tomography angiography.

BACKGROUND: In addition to the diagnostic performance, coronary computed tomography angiography (CTA) can give important data regarding the prognosis of coronary artery disease (CAD). In this study we aimed to evaluate the prognostic role of coronary CTA in patients with suspected CAD and mild-moderate coronary stenosis. METHODS: A total of 1115 patients (602 male, 54%; age 58.4 ± 11.4) without previous CAD were enrolled. Patients underwent coronary CTA imaging using dual-source 64-slice CT scanner. For categorization of the coronary atherosclerotic plaques (CAP), the coronary system was divi

Relapse of Crohn's disease following ileal pouch-anal anastomosis: MDCT and MR features.

OBJECTIVE: The purpose of our study is to use MDCT and MR imaging to describe the normal ileal pouch and to identify features of Crohn's disease (CD) relapse in patients after ileal pouch-anal anastomosis (IPAA). CONCLUSIONS: After total colectomy followed by IPAA, features, optimally evaluated with pelvic MRI, such as fistulas, abscesses, pouch inflammation, and stenoses, indicate CD relapse. Although uncommon, radiologists should be aware that these imaging features strongly favor this diagnosis.

Clinical implications of immediate or later periportal edema in MS-CT trauma scans: surrogate parameter of intravenous fluid status and venous congestion.

Periportal edema (PPE) of the liver in multislice computed tomography (MS-CT) scans that develops immediately (primary PPE [pPPE]) or later (secondary PPE [sPPE]) is not uncommon in severe trauma patients. Although PPE may serve as a marker for blunt abdominal trauma (22-31 % of cases), distinct causes and clinical implications of PPE are unclear. We analyzed the incidence of pPPE and sPPE in 68 MS-CT scans in severe trauma patients (2007-2009). Exclusion criteria were severely burned patients and patients with preexistent liver diseases predisposing to PPE. We divided PPE+ patients into two s

Coronary artery fistula with a giant aneurysm treated by percutaneous transradial embolization.

Coronary artery fistulas are rare anomalies that are very rarely accompanied by an aneurysm. The minimally invasive method of percutaneous transradial embolization, using a thin guiding catheter, was used to treat a coronary artery fistula with an associated giant aneurysm. This technique, not previously described for this type of application, is presented as a case report. The successful outcome of this procedure demonstrated that transradial coronary interventions are useful for treating coronary artery fistulas with an associated giant aneurysm, especially in patients at high risk for conve

The pedicle-facet angle and tropism in the sagittal plane in degenerative spondylolisthesis: a computed tomography study using multiplanar reformations techniques.

STUDY DESIGN: A case-control study. OBJECTIVE: To analyze the facet joint orientation in the sagittal plane [pedicle-facet angle (P-F angle)] and facet tropism, and clarify the relationship between P-F angle and the amount of vertebral slipping in degenerative spondylolisthesis (DS) patients on multislice computed tomography using multiplanar reformations techniques. SUMMARY OF BACKGROUND DATA: Some studies have indicated a correlation between DS and an increased sagittal orientation of the facet joints. However, the facet orientation has not been fully elucidated and it had been measured on

Comparative analysis of third-generation dual-energy CT and IVUS for in-stent restenosis detection.

PURPOSE: Prior studies have assessed in-stent diameter restenosis (ISDR) in coronary arteries using 64-slice multidetector computed tomography coronary angiography (MDCT-CA) compared to invasive coronary angiography (ICA), which is the gold standard. This study aimed to compare the diagnostic accuracy of monoenergetic reconstruction using third-generation dual-source dual-energy CT (DSDECT) to that of ICA reconstruction via adjunctive intravascular ultrasonography (IVUS) for evaluating the ISDR. METHODS: A total of 95 patients with previously stented coronary arteries (involving 110 stents) u…

Pressure- and flow-driven biomechanical factors associate with carotid atherosclerosis assessed by computed tomography angiography.

BACKGROUND AND AIMS: Local biomechanical factors are known to influence atherosclerosis in extracranial carotid arteries. While the role of some flow-driven biomechanical factors has been investigated, the influence of pressure-driven mechanical wall stress (MWS) has received limited attention. In this study, the association of the pressure-driven and flow-driven biomechanical factors with carotid atherosclerosis was examined. METHODS: Carotid arteries (n = 150) with mild-to-moderate stenosis from 75 symptomatic patients (Plaque-At-Risk study) were imaged using multi-detector computed tomogra

Coronary artery volume to left ventricular myocardial mass ratio in detecting primary microvascular angina: a retrospective observational study in Chinese population.

BACKGROUND: The ratio of coronary artery volume to left ventricular myocardial mass (V/M) reflects the hemodynamic association between epicardial coronary arteries and myocardium. Evidence suggests a significant decrease in V/M among patients with primary microvascular angina (PMVA). However, V/M values may be influenced by anatomic and physiological varieties, necessitating independent evaluation in different ethnic groups. We aimed to explore the potential of V/M in predicting PMVA in Chinese population. METHODS: We conducted a retrospective case-control analysis on 23 PMVA patients and 25

Prognostic implications of left ventricular mass-geometry in patients with no or nonobstructive coronary artery disease.

BACKGROUND: Coronary computed tomography angiography (CCTA) is widely used as a first-line noninvasive modality that frequently exhibits no or nonobstructive coronary artery disease (CAD) in clinical practice, along with abnormal left ventricular (LV) geometry on echocardiography. However, the combined prognostic value of these findings has not been well elucidated. Therefore, we aimed to evaluate the prognostic implications of abnormal LV geometry in individuals with no or nonobstructive CAD. METHODS: A total of 5806 subjects with no CAD or nonobstructive CAD (luminal narrowing < 50%) on CCT
