Preliminary Evaluation of 225 Ac/ 177 Lu-DOTATATE PRRT in Progressive Meningiomas : Efficacy, Dosimetry, and Imaging Insights.

BACKGROUND: Progressive meningiomas present a significant therapeutic challenge; there are few therapeutic options in this scenario. Peptide receptor radionuclide therapy (PRRT) targeting somatostatin receptors (SSTRs) is a promising strategy. This study, for the first time, evaluates the efficacy and safety of a combination or sequential use of alpha- emitting [ 225 Ac]Ac-DOTATATE and beta-emitting [ 177 Lu]Lu-DOTATATE PRRT for patients with advanced, refractory meningiomas. PATIENTS AND METHODS: In this pilot study, 7 patients with inoperable or refractory meningiomas and high SSTR expressi

Krenning score enhances integrated biomarker models for survival prediction in PRRT-treated GEP-NET: a retrospective cohort study.

BACKGROUND: Purpose of this study is to evaluate the value of a routinely available baseline SSTR-PET imaging parameter, the Krenning score (KS), within integrated biomarker models for overall survival prediction in gastroenteropancreatic neuroendocrine tumor (GEP-NET) patients undergoing peptide receptor radionuclide therapy (PRRT). METHODS: We retrospectively analyzed 178 patients with GEP-NET who underwent PRRT. At baseline, the KS, clinical, histopathological, and laboratory parameters were integrated and correlated with OS. OS predictors were identified using univariate Cox regression an

Profiling proteomic responses in small intestinal neuroendocrine tumor GOT1 after [177Lu]Lu-DOTATATE therapy.

BACKGROUND: Peptide receptor radionuclide therapy with [177Lu]Lu-DOTATATE is an established treatment for somatostatin receptor-expressing neuroendocrine tumors. Although strong anti-tumor effects have been demonstrated in experimental models, curative responses in patients remain limited. Improved understanding of the molecular responses induced by [177Lu]Lu-DOTATATE may help identify strategies for treatment optimization. This study aimed to characterize proteomic alterations in GOT1 small intestinal neuroendocrine tumor xenografts following [177Lu]Lu-DOTATATE therapy. METHODS: GOT1 tumor-b

Lower 68 Ga-DOTATOC uptake in nonfunctioning pituitary neuroendocrine tumours compared to normal pituitary gland-A proof-of-concept study.

OBJECTIVES: 68  Ga-DOTATOC PET targets somatostatin receptors (SSTRs) and is well established for the detection of SSTR-expressing tumors, such as gastrointestinal neuroendocrine tumors. Pituitary adenomas, recently designated as pituitary neuroendocrine tumors (PitNETs), also express SSTRs, but there has been no previous evaluations of 68  Ga-DOTATOC PET in PitNET patients. The aim of this pilot study was to evaluate the diagnostic properties of 68  Ga-DOTATOC PET in the most common PitNET, ie non-functioning (NF)-PitNET. DESIGN/PATIENTS: NF-PitNET patients (n = 9) and controls (n = 13) were

The utility of 68Ga-DOTATATE PET/CT in localizing primary/metastatic pheochromocytoma and paraganglioma in children and adolescents - a single-center experience.

OBJECTIVES: Pediatric pheochromocytoma and paraganglioma (PPGL) are rare tumors with limited data on the diagnostic performance of 68Ga-DOTA(0)-Tyr(3)-octreotate positron emission tomography-computed tomography (68Ga-DOTATATE PET/CT). We have described our experience of 68Ga-DOTATATE PET/CT in overall and von Hippel Lindau (VHL)-associated pediatric PPGL and compared its sensitivity with that of 131I-meta-iodobenzyl-guanidine (131I-MIBG), 18F-fluorodeoxyglucose PET/CT (18F-FDG PET/CT), and contrast-enhanced CT (CECT). METHODS: Retrospective evaluation of consecutive PPGL patients (age: ≤20 ye

Clinical approach and review of causes of a chylothorax.

A chylothorax, also known as chylous pleural effusion, is an uncommon cause of pleural effusion with a wide differential diagnosis characterized by the accumulation of bacteriostatic chyle in the pleural space. The pleural fluid will have either or both triglycerides >110 mg/dL and the presence of chylomicrons. It may be encountered following a surgical intervention, usually in the chest, or underlying disease process. Management of a chylothorax requires a multidisciplinary approach employing medical therapy and possibly surgical intervention for post-operative patients and patients who have

Roles of Hippocampal Somatostatin Receptor Subtypes in Stress Response and Emotionality.

Altered brain somatostatin functions recently appeared as key elements for the pathogenesis of stress-related neuropsychiatric disorders. The hippocampus exerts an inhibitory feedback on stress but the mechanisms involved remain unclear. We investigated herein the role of hippocampal somatostatin receptor subtypes in both stress response and behavioral emotionality using C57BL/6, wild type and sst2 or sst4 knockout mice. Inhibitory effects of hippocampal infusions of somatostatin agonists on stress-induced hypothalamo-pituitary-adrenal axis (HPA) activity were tested by monitoring peripheral b

Peptide Receptor Radionuclide Therapy-Induced Hepatotoxicity in Patients With Metastatic Neuroendocrine Tumors.

BACKGROUND: Treatment of metastatic gastroenteropancreatic neuroendocrine tumors (GEP-NETs) with peptide receptor radionuclide therapy (PRRT) is effective in retarding tumor growth. Renal dysfunction, anemia, and thrombocytopenia are well-described treatment-related toxicities. However, hepatotoxicity is not well recognized. PATIENTS AND METHODS: We performed a retrospective cohort study of consecutive patients with GEP-NETs seen in a tertiary NET clinic from January 2010 to September 2013 (n = 211) with the primary study cohort being patients with metastatic disease to the liver (n = 93). A case of ectopic adrenocorticotropic hormone-producing pancreatic neuroendocrine tumor with multiple liver metastases.

Ectopic adrenocorticotropic hormone (ACTH) production by the pancreatic neuroendocrine tumor (p-NET) is relatively rare, and patients with this tumor show poor prognosis. In this study, we present the case of a 64-year-old woman who presented with ectopic ACTH syndrome due to p-NET with multiple liver metastases. Computed tomography revealed that she had multiple masses in the liver and a solid mass in the head of the pancreas. Endocrinological examinations revealed markedly elevated plasma ACTH (735.0 pg/mL) and cortisol (34.7 microg/dL) levels associated with hypokalemia (2.7 mEq/L), diabete

Peptide receptor radionuclide therapy with 177Lu-octreotate in patients with foregut carcinoid tumours of bronchial, gastric and thymic origin.

PURPOSE: Foregut carcinoid tumours have a different embryological origin than other gastroenteropancreatic neuroendocrine tumours (GEP NETs). In the total group of GEP NETs (n = 131), treatment with (177)Lu-octreotate resulted in tumour remission in 47% of patients, with a median time to progression (TTP) of >36 months. As patients with foregut carcinoids may respond differently, we here present the effects of this treatment in a subgroup of patients with foregut carcinoids of bronchial, gastric or thymic origin. METHODS: Nine patients with bronchial, five with gastric and two with thymic car

99mTc-EDDA/HYNIC-octreotate scintigraphy, an efficient method for the detection and staging of carcinoid tumours: results of 3 years' experience.

PURPOSE: At all stages of the disease, serious difficulties are encountered in the imaging diagnosis of carcinoids. Somatostatin receptor scintigraphy (SRS) holds great promise for detecting primary tumours and metastases. 99mTc-EDDA/HYNIC-octreotate should significantly improve the diagnosis of carcinoids in comparison with 111In-Octreoscan owing to the better affinity for SSR2 and the higher count rate. The aim of this study was to assess the diagnostic efficiency of 99mTc-EDDA/HYNIC-octreotate scintigraphy in the detection and staging of carcinoid tumours. METHODS: The study population com

[Somatostatin receptor scintigraphy. A new imaging procedure for the specific demonstration of carcinoids of the small intestine].

Scintigraphy with a radiolabelled somatostatin analog represents a new highly specific approach in the diagnostic work-up of receptor-positive APUD tumours and their metastases. We present our preliminary results with somatostatin receptor scintigraphy in 15 patients with histologically proven midgut carcinoid. 5 out of 6 primary tumour sites (83%) and 90% of the known metastatic lesions could be detected; unknown metastatic lesions were seen in 5 patients. Compared with other nuclear medicine procedures somatostatin receptor scintigraphy is able to detect all tumour sites within hours. This a

Quantitative SPECT imaging for predicting therapeutic response to lutetium-177 DOTATATE in neuroendocrine tumors.

OBJECTIVE: As the incidence of neuroendocrine tumors (NETs) increases, Lutetium-177 (Lu-177) DOTATATE targeted radionuclide therapy (TRT) has become a vital treatment strategy. However, its prolonged course makes early prediction of response challenging, and current predictive methods like dosimetry are complex. Therefore, a simpler, earlier prediction method is needed for timely treatment planning. This study aimed to investigate whether quantitative metrics from a single, initial post-therapy single-photon emission computed tomography / computed tomography (SPECT/CT) image could predict the

A Multicenter Phase II Randomized Controlled Trial Comparing 177Lu-Dotatate/Capecitabine Combination Treatment with 177Lu-Dotatate Monotherapy in Patients with Neuroendocrine Tumors.

PURPOSE: Peptide receptor radionuclide therapy (PRRT) with [177Lu]Lu-[DOTA0,Tyr3]octreotate (177Lu-Dotatate) is an effective and safe treatment for patients with metastatic gastroenteropancreatic neuroendocrine tumors (GEP NET). Whereas 177Lu-Dotatate prolongs progression-free survival (PFS) and preserves quality of life (QoL), objective response rates (ORR) remain limited. Capecitabine, as a radiosensitizer, could increase efficacy without increasing 177Lu-Dotatate activity. This phase II randomized controlled trial investigated the additional cytotoxic or radiosensitizing effect of capecitab

Imaging-derived biomarkers from 68Ga-DOTATOC PET/CT scans to predict survival of patients with neuroendocrine tumors after PRRT with 177Lu-DOTATATE.

BACKGROUND: Neuroendocrine tumors have increased in prevalence and diversity in recent years and are often diagnosed at metastatic stages. Compared with nonradioactive systemic treatment with somatostatin analogs, peptide receptor radionuclide therapy (PRRT) has shown superior overall survival benefits for well-differentiated neuroendocrine tumor patients. This study aimed to identify biomarkers from 68Ga‒DOTATOC PET/CT scans to predict survival in patients treated with PRRT in the clinic. METHODOLOGY: This retrospective study analyzed 68Ga-DOTATOC PET/CT data from 67 NET patients undergoing

Impact of hyperglycemia and antidiabetic medication on pancreatic uptake on [68Ga]Ga-DOTATOC PET/CT.

INTRODUCTION: Positron-emission-tomography-(PET)/computed-tomography-(CT) using somatostatin-receptor-(SSTR)-binding radioligands is well established in the imaging of neuroendocrine tumors (NETs). SSTRs are expressed in NETs and endocrine and exocrine tissues, e.g. pancreas, where somatostatin binding to SST2 and SST5 inhibits glucagon and insulin secretion. Pancreatic background activity on SSTR-PET varies widely and is increased in up to 45% of cases. High uptake in the processus uncinatus can obscure NETs or cause false positives. The determinants of elevated pancreatic activity on SSTR-PE
