Local distribution and quality of bone mineral density (BMD) in the proximal humerus are important for rotator cuff repair, internal fixations, and arthroplasty. The current study aimed to develop a volumetric BMD (vBMD) measurement technique that can specify the volumetric region of interest in an anatomically reproducible manner and ensures fair comparison of vBMD among intrasubjective or intersubjective measurements. Computed toed into three-dimensional (3D) polygon models. A standardized volumetric block was asmographic images of 10 intact shoulders (from six women and four men, age range

Massive rotator cuff tendon ruptures are not uncommon in older patients. We propose the transfer of the latissimus dorsi muscle for treatment of irreparable ruptures associated with functional impairment and chronic pain. Five women and 11 men were so treated and reviewed with an average follow-up of 26 months. The mean age was 60 years. Four patients also had subscapularis deficiency. Results were assessed with the Constant score and the Oxford shoulder score. Humeral head position was analysed. Statistical analysis was performed by the Wilcoxon non-parametrical test. The Constant score incre

This prospective, randomized investigation evaluated the efficacy of cryotherapy on subjective responses after both open and arthroscopic procedures on the shoulder. Seventy patients were randomly assigned to one of two study groups: (1) continuous cryotherapy group and (2) age-matched control group. Visual analog scales were used to assess subjective responses on postoperative days 1, 7, 14, and 21. On day 1, patients receiving cryotherapy reported significantly less pain during sleep and significantly more comfort in bed and rated their sleep as more restful than the control subjects. During

Rotator cuff and biceps tendons that appeared grossly normal were procured from adult cadavers without a history of shoulder problems. These tendons were analyzed for the amount and type of glycosaminoglycan, type of proteoglycan, and histology. When compared with the distal/tensional region of biceps tendon, the glycosaminoglycan content of supraspinatus, infraspinatus, and subscapularis tendons was 2.5-fold higher and the glycosaminoglycan content of the proximal/compressed region of biceps tendon was 3-fold higher. The ratio of hyaluronic acid to chondroitin sulfate/dermatan sulfate in all

We evaluated the effect of two different anterior capsular shift strategies on load-induced multidirectional glenohumeral motion. Nine cadaveric shoulders were tested on a special test apparatus in which three motions were constrained but which allowed simultaneous measurement of three translations: anteroposterior, mediolateral, and superoinferior. A simulated Bankart lesion was created by detaching the inferior glenohumeral ligament and labrum from the anterior glenoid from 5 mm superior to the anterior band of the inferior glenohumeral ligament to a point just posterior to the infraglenoid

PURPOSE: The resection of the humeral head is normally carried out only with respect to four-part and dislocation fractures in elderly patients. The purpose of this study was to examine whether there is a correlation between the clinical results and the MRI findings.
MATERIAL AND METHODS: 8 patients underwent clinical and MR examination two to 6 years after resection.
RESULTS: A cartilage-like tissue formation between the remaining humeral head and the glenoid fossa, with signal intensities comparable to hyalin cartilage was found in all patients.
CONCLUSION: The good functional results fol

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is a well-established treatment for cuff tear arthropathy and consistently restores active forward elevation. However, when there is a deficiency of the posterior rotator cuff (infraspinatus and teres minor), patients can exhibit lag signs at the side and a "Hornblower's sign." This loss of active external rotation (ER) in abduction can be functionally disabling, and rTSA alone may not reliably restore active ER ability in this clinical scenario. We present a series of rTSA with lower trapezius transfer (LTT) (utilizing tibialis anterior a

BACKGROUND: Retear of repaired rotator cuff tendons worsens patient outcome and decreases patient satisfaction. Superior capsule reconstruction (SCR) was developed to center the humeral head and thus restore the force couple for patients with rotator cuff tears.
PURPOSE: To evaluate whether SCR for reinforcement before arthroscopic rotator cuff repair (ARCR) improves cuff integrity.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: Thirty-four consecutive patients (mean age, 69.1 years) with severely degenerated but reparable rotator cuff tears underwent SCR with fascia lata autogr

PURPOSE: Our purpose was to evaluate the influence of metallic lateralisation of the centre of rotation (COR) in reverse shoulder arthroplasty (RSA) on the incidence of scapular notching and its eventual clinical and radiological consequences.
METHODS: We analysed 140 RSAs with a lateralised design (Arrow, FH Orthopedics, Mulhouse, France) implanted for massive rotator cuff tear with/without arthritis. Mean follow-up was 45 months (range 24-120, standard deviation 20). Patients were evaluated clinically using the Constant and Murley (Clin Orthop Relat Res 214:160-164, 1987) score and active r

Tendon-bone insertion trauma is prevalent in both rotator cuff and anterior cruciate ligament injuries, which are frequently encountered conditions in the field of sports medicine. The main treatment for such injuries is reconstructive surgery. The primary determinant impacting this process is the graft's capacity to integrate with the bone tunnel. In recent years, researchers have attempted to use a variety of methods to facilitate tendon-bone healing after reconstructive surgery. Such as the implantation of biological materials, cytokines and the local application of permanently differentiat

OBJECTIVE: To review common repetitive strain injuries (RSIs) that occur in the workplace, emphasizing diagnosis, treatment, and etiology of these conditions.
QUALITY OF EVIDENCE: A MEDLINE search from January 1966 to June 1999 focused on articles published since 1990 because RSIs are relatively new diagnoses. MeSH headings that were explored using the thesaurus included "cumulative trauma disorder," "overuse injury," and "repetitive strain injury." The search was limited to English articles only, and preference was given to randomized controlled trials.
MAIN MESSAGE: Repetitive strain injur

BACKGROUND: Shoulder pain is a common problem in the general population and is responsible for prolonged periods of disability, loss of productivity, absence from work and inability to carry out household activities. Rotator cuff problems account for up to 70% of shoulder pain problems and are the third most prevalent musculoskeletal disorder after those occurring in the lower back and neck. Rotator cuff surgery has high failure rates (25-50% within 12 months), and as a result, there is a pressing need to improve the outcome of rotator cuff surgery. Patch augmented surgery for rotator cuff rep

Synovial chondromatosis is classified as either primary or secondary. Primary synovial chondromatosis results from a proliferation of chondrocytes in the synovial membrane leading to the formation of cartilaginous loose bodies. Secondary synovial chondromatosis is a rare condition characterized by the growth of separated particles from the articular cartilage or osteophytes in joint diseases. The present article aims to report the secondary chondromatosis of the shoulder and to discuss the clinical manifestations, pathogenesis, diagnosis, histological findings and management of this condition.

Damage to the biceps tendon is often seen in conjunction with rotator cuff tears. However, controversy exists regarding its role in the shoulder and its optimal treatment. A previous study determined that biceps tendons were detrimentally affected in the presence of rotator cuff tears in the rat model and this damage worsened over time. However, whether this damage progresses at later time points to provide a chronic model is unknown. The objective of this study was to determine the changes in the biceps tendon in the presence of a cuff tear over time. Our hypothesis was that histological, com

The understanding of rotator cuff disease has increased exponentially since Codman drew attention to this pathology in the early 1900s. Although challenging, the surgical treatment of massive rotator cuff tears is rational, with treatment decisions based on physical examination, imaging, biologic, and patient factors. Arthroscopy can be used to treat ancillary pain generators, débride necrotic tissue, and possibly restore balance to the force couples about the shoulder. Tendon transfers may be effective in restoring functional strength to irreparable, ineffectual muscle units. Arthroplasty is

BACKGROUND: Shoulder movement patterns differ between limbs of children with unilateral brachial plexus birth palsy. To better understand the interlimb differences we examined the glenohumeral and scapulothoracic joint contributions to arm elevation.
METHODS: Sixteen children with brachial plexus birth palsy, 4-12 years of age participated. Shoulder 3D kinematic data were collected using a magnetic tracking device during arm elevation with the involved and non-involved limbs for three trials each at a fixed rate. Based on maximum arm elevation in the involved limb the children were divided in

This is an unusual case report of an elderly patient with spontaneous acute rupture of rotator cuff complicated by large subacromial hematoma. The hematoma fluid was aspirated with significant reduction of pain. The shoulder movements were restored after one month with conservative therapy. The compensatory role of preserved deltoid muscle is considered in the rehabilitation of normal shoulder movement, which may replace aggressive immobilization or surgical procedures in the elderly population. Investigation of hematoma fluid compared with blood 24 hours after the event revealed severe lactic

The purpose of this study was to determine the morphologic characteristics of the glenoid osteophyte and its relationship to rotator cuff tears. Eighty-six cadaveric shoulders were studied. After inspection of cuff pathology, the glenoid was examined with soft x-rays. Histologically, degenerative changes of the articular cartilage and the labrum were assessed. There were 30 shoulders with rotator cuff tears (35%). Two types of glenoid osteophyte were identified: angular and hooked. Both osteophytes were commonly observed in the anterior to inferior portions of the glenoid. The length of hooked
