The potential for bio-mediators and biomarkers in pediatric traumatic brain injury and neurocritical care.

The use of biomarkers of brain injury in pediatric neurocritical care has been explored for at least 15 years. Two general lines of research on biomarkers in pediatric brain injury have been pursued: (1) studies of "bio-mediators" in cerebrospinal fluid (CSF) of children after traumatic brain injury (TBI) to explore the components of the secondary injury cascades in an attempt to identify potential therapeutic targets and (2) studies of the release of structural proteins into the CSF, serum, or urine in order to diagnose, monitor, and/or prognosticate in patients with TBI or other pediatric ne

Quantitative analyses of postmortem heat shock protein mRNA profiles in the occipital lobes of human cerebral cortices: implications in cause of death.

Quantitative RNA analyses of autopsy materials to diagnose the cause and mechanism of death are challenging tasks in the field of forensic molecular pathology. Alterations in mRNA profiles can be induced by cellular stress responses during supravital reactions as well as by lethal insults at the time of death. Here, we demonstrate that several gene transcripts encoding heat shock proteins (HSPs), a gene family primarily responsible for cellular stress responses, can be differentially expressed in the occipital region of postmortem human cerebral cortices with regard to the cause of death. Ventricular tachycardia during arthroscopic shoulder surgery: a report of two cases.

We routinely have performed arthroscopic shoulder surgery under general anesthesia in the beach chair position using epinephrine (0.33 mg/L) saline irrigation. At a 2-week interval, two patients, a 19-year-old man scheduled to undergo an arthroscopic Bankart repair for left traumatic anterior instability and a 49-year-old woman scheduled for an arthroscopic rotator cuff repair for a left rotator cuff tear, were resuscitated by chest compression and defibrillation due to a sudden developed cardiogenic shock following ventricular tachycardia at the time of arthroscopic shoulder surgery. They wer

An evidence-based review on the use of corticosteroids in peri-operative and critical care.

With complex and extensive pharmacological effects, corticosteroids are widely used in many clinical situations. A survey conducted to define the role of corticosteroids in various settings of peri-operative and critical care gave strong evidence to support that the use of corticosteroid is absolutely indicated in patients with adrenal insufficiency, asthma, anaphylaxis, acute spinal cord injury, and increased ICP resulting from brain tumors. As the benefits of corticosteroids are much in evidence, their uses are recommended to extend to postoperative antiemesis, acute respiratory failure (suc

Management of blunt and penetrating colon injuries.

The records of 28 patients with traumatic colon injuries (TCI) were retrospectively reviewed. Sixteen patients (57%) with 17 TCI had blunt trauma, whereas 8 patients (29%) experienced penetrating trauma. Four TCI were from intraluminal injury. Blunt trauma commonly involved the left colon, whereas penetrating trauma usually involved the right or transverse colon. Fifty-nine percent of the blunt TCI were treated with primary repair, including resection and primary anastomosis, as were 88% of the penetrating TCI. Shock, transfusion requirement of more than 4 units, contamination, and associated

Diaphragmatic rupture due to blunt trauma: morbidity and mortality in 42 cases.

We present an analysis of 42 cases of acute rupture of the diaphragm by blunt trauma. There were 31 men (74%) and 11 women (26%); the mean age was 32.8 years +/- 2.4 SEM. At admission, hypovolemic shock was present in 45% of the cases, pelvic fracture in 36%, and severe respiratory distress in 21%. Diaphragmatic rupture was suggested before operation by unilateral elevation of the diaphragm, supradiaphragmatic densities, and displacement of abdominal organs into the thorax, as shown by chest films and GI series in 18 cases (43%). The left hemidiaphragm was injured in 24 cases (57%), the right

Successful ROTEM-guided transfusion therapy in a case of rural paediatric trauma.

Massive transfusion therapy in severe paediatric trauma is a challenge. When this occurs in a rural setting, the potential delays in accessing medical services can lead to worse coagulopathy on presentation. We report successful treatment of a child post-traumatic lower limb amputation where treatment was only initiated 3 hours postinjury due to difficulties in access/retrieval. Rotational thromboelastometry (ROTEM)-guided, goal-directed haemostatic therapy perioperatively utilising a blood product ratio of 2:2:1 units of red blood cell:free frozen plasma:platelet reverted his coagulopathy. Survival with an arterial pH of 6.57 following major trauma with exsanguinating haemorrhage associated with traumatic amputation.

We report the survival of a multiply injured patient with exanguinating haemorrhage and an arterial pH of 6.5, following a road vehicle crash. The previously healthy 38 years old male driver veered off the motorway and collided with a tree. The ambulance arrived at the scene 9 min after being called by an eyewitness and, following rapid extrication from the wreckage; the patient arrived in hospital 27 min later (with a GCS of 6), and was immediately intubated. The patient had suffered near-complete amputation of the left leg at upper femoral shaft level, along with multiple distal fractures an

[Infusions of amino acid solutions in the therapy concept of trauma patients. Comparative studies in the early phase of polytrauma].

The early phase of injury is characterized, among other things, by enhanced proteolysis and changed protein synthesis. Posttraumatic catabolism with its negative nitrogen balance exercises a direct influence on the healing process. Our study aimed at finding out whether early application of complete parenteral feeding would improve protein metabolism. Two groups of patients were compared with one another: One group of test persons (n = 28) whom a complete solution (2.5 carbohydrate and 10% AA solution) was given directly after the injury, whereas a control group (n = 71) received parenteral fe

Fluid resuscitation of the thermally injured patient. Current concepts with definition of clinical subsets and their specialized treatment.

The key to successful fluid resuscitation of the thermally injured patient is close monitoring of the clinical response. Individualized resuscitation based on the parameters described allows for approximation of this goal. An eclectic approach used by the authors has been described with reference to clinical subsets that may require specialized resuscitative methods. Efforts should be made to use the smallest volume of fluid needed to optimize end organ perfusion. The goals of fluid resuscitation have been summarized and a variety of formulas have been described. We anticipate that our underst

Depression, post-traumatic stress disorder, and functional disability in survivors of critical illness in the BRAIN-ICU study: a longitudinal cohort study.

BACKGROUND: Critical illness is associated with cognitive impairment, but mental health and functional disabilities in survivors of intensive care are inadequately characterised. We aimed to assess associations of age and duration of delirium with mental health and functional disabilities in this group. METHODS: In this prospective, multicentre cohort study, we enrolled patients with respiratory failure or shock who were undergoing treatment in medical or surgical ICUs in Nashville, TN, USA. We obtained data for baseline demographics and in-hospital variables, and assessed survivors at 3 mont

Current Preclinical and Clinical Evidence of Shock Wave Therapy for Spinal Cord Injury: A Systematic Review.

The diagnosis of spinal cord injury (SCI) remains associated with a poor prognosis due to limited treatment options and the absence of curative therapies. Optimizing treatment strategies is therefore crucial to enhance patients' quality of life, reduce mortality and re-hospitalization rates, and lower overall therapy costs. Shock wave therapy (SWT) is a well-established regenerative treatment option for pathologies of the musculoskeletal system that delivers high-energy acoustic waves. SWT is non-invasive, safe and cost-effective. Preclinical and clinical evidence is emerging, showing the effi

Clinical outcomes of endovascular aortic repair in blunt traumatic aortic injury: A retrospective case series and a CARE-compliant case report.

RATIONALE: Blunt traumatic aortic injury (BTAI) is a high-mortality condition often managed by endovascular aortic repair (EVAR) due to its minimally invasive nature and favorable outcomes. PATIENT CONCERNS: Six patients with BTAI were admitted to our institution from 2014 to 2023. They presented with symptoms such as chest pain or were diagnosed with aortic injury on chest computerized tomography after motor vehicle collisions. DIAGNOSIS: All patients underwent computed tomography arteriography to confirm the diagnosis and assess the aortic injury. INTERVENTIONS: All patients received EVAR

Complete Thoracoabdominal Wall Avulsion and Right Hemidiaphragmatic Rupture Following High-Energy Motorcycle Trauma: A Case Report.

High-energy motorcycle trauma can produce catastrophic thoracoabdominal wall disruption with diaphragmatic rupture, a rare and frequently lethal injury pattern that demands immediate damage control management to interrupt the lethal triad of hypothermia, acidosis, and coagulopathy. We report a 31-year-old man who sustained complete avulsion of the right thoracoabdominal wall with right hemidiaphragmatic crural rupture after being ejected from a motorcycle at high speed. Definitive surgical referral was delayed by eight hours, during which the patient progressed to grade IV hemorrhagic shock. D
