BACKGROUND: We reported a tubeless tracheal resection and reconstruction for the management of benign posttracheostomy tracheal stenosis.
METHODS: A 34-year-old man with stridor, severe respiratory distress, and recurrent pneumonia was referred to our attention for treatment of benign posttracheostomy tracheal stenosis. BACKGROUND: Percutaneous dilatational tracheostomy (PDT) has become an established therapy for patients with prolonged intubation. It is of utmost importance for respiratory weaning of long-term ventilated patients in modern intensive care medicine. The thoracic complications of amebiasis frequently necessitate surgical intervention. Experience with 28 patients is presented. Involvement included the pleura in 19 patients, the lungs in 10, and the pericardium in five. In 25%, more than one site was involved. History revealed iatrogenic injury (tracheostomy, intubation, artificial respiration) to be the reason for tracheal stenosis in each of our 32 cases (18 female, 14 male). During a 14 year period, 22 patients with stricture, acute laryngeal injury, and cancer were treated. Nine patients had postintubation tracheal stenosis and underwent tracheal resection with good to excellent results. Six patients had strictures after tracheostomy. In a retrospective review of one year's experience in the management of neck trauma, 88 cases comprising 42 gunshot wounds, 29 stab or slash injuries, and 17 injuries by blunt trauma were reviewed. Multiple injuries were noted in all groups, but a higher percentage was found in the blunt trauma series. Anesthetic management is reviewed, and emphasis is placed on careful evaluation of the patient's airway, using physical and roentgenographic examination to plan adequate airway management.
