Acute pancytopenia due to megaloblastic arrest in association with co-trimoxazole.

Three patients who were taking co-trimoxazole developed acute pancytopenia due to megaloblastic arrest, and two of the three died while pancytopenic. One patient had a pre-existing megaloblastic anaemia. In the other two patients, there was no macrocytosis or neutrophil hypersegmentation despite the severe megaloblastosis. Because specific treatment is needed urgently, it is important to distinguish megaloblastic arrest from drug-induced hypoplasia.

Evaluation of serum and dietary profiles of vitamin B12 and folate and their association with systemic complications in patients with Crohn's disease.

BACKGROUND/OBJECTIVE: Crohn's Disease (CD) is a chronic inflammatory bowel disease that affects micronutrient levels, impacting metabolic pathways. Homocysteine (Hcy) levels, regulated by folate and vitamin B12, are associated with cardiovascular and extraintestinal manifestations (EIMs). This study aims to analyze the dietary intake and serum concentrations of folate and vitamin B12 and their relationship with systemic complications in patients with CD in remission and active phases. METHODS: This cross-sectional study included 60 patients, and nutrient intake was stratified into tertiles by

Comparative efficacy of biweekly preventive supplementation, with multiple micronutrients and iron folic acid or iron folic acid alone, on hemoglobin concentrations and anemia prevalence in children aged 6-59 months: a randomized controlled trial in rural India.

BACKGROUND: Anemia affects more than two-thirds of children aged <5 y in India, despite biweekly iron folic acid (IFA) supplementation under the Anemia Mukt Bharat national program. Multiple micronutrient (MMN) deficiencies may also contribute to the presence of anemia, but the incremental benefit of MMN supplementation in addition to IFA remains unclear. OBJECTIVES: We aimed to compare the efficacy of biweekly preventive supplementation with MMNs plus IFA compared with IFA alone on hemoglobin concentrations and prevalence of anemia in children aged 6-59 mo. METHODS: In this individually ran

Effect of iron dose in maternal multiple micronutrient supplement on perceived side effects, adherence, acceptability and preferences: protocol for a randomised crossover trial.

INTRODUCTION: Iron-folic acid (IFA) supplementation in pregnancy is recommended by the WHO, with a dose of 60 mg of iron in contexts where anaemia remains a severe public health problem. Iron-containing supplements may cause side effects that affect acceptability and adherence in a dose-response manner. Maternal multiple micronutrient supplements (MMS), which include iron and folic acid plus additional micronutrients, are also recommended in the context of rigorous research, and programmes are considering transitioning from IFA to MMS containing 30 mg of iron. We will evaluate the effect of ir

Impact of Iodized Salt Fortified with Folic Acid, Vitamin B12 and Iron on Reproductive-Aged Women in India.

BACKGROUND: India has a high prevalence of anemia, vitamin B12 deficiency, and maternal folate insufficiency with associated spina bifida and anencephaly (SBA). Fortifying household iodized table salt can be a strategy to prevent several micronutrient deficiencies. OBJECTIVE: To examine the effectiveness of quadruple fortified salt (QFS) on serum folate, vitamin B12, and iron concentrations after 4 months of intervention. METHOD AND MATERIAL: A nonrandomized controlled trial using a pre- and post-intervention evaluation design was implemented from July 1, 2022, to November 30, 2022 in four r

Association of development quotient with nutritional status of vitamins B6, B12, and folate in 6-59-month-old children: Results from the Brazilian National Survey on Child Nutrition (ENANI-2019).

BACKGROUND: Vitamins B6, B12, and folate are essential for the formation and maintenance of the human brain, but studies evaluating these vitamins with early childhood development (ECD) in children under 5 y are limited and controversial. OBJECTIVES: To evaluate the association between vitamins B6, B12, and folate concentrations/status and ECD. METHODS: Data regarding 6520 children aged 6-59 mo from the ENANI-2019 (the Brazilian National Survey on Child Nutrition) were analyzed. ECD was assessed using the Survey of Well-being of Young Children's milestones questionnaire. Vitamin B6 concentra

The role of vitamin B12 in viral infections: a comprehensive review of its relationship with the muscle-gut-brain axis and implications for SARS-CoV-2 infection.

This comprehensive review establishes the role of vitamin B12 as adjunct therapy for viral infections in the treatment and persistent symptoms of COVID-19, focusing on symptoms related to the muscle-gut-brain axis. Vitamin B12 can help balance immune responses to better fight viral infections. Furthermore, data from randomized clinical trials and meta-analysis indicate that vitamin B12 in the forms of methylcobalamin and cyanocobalamin may increase serum vitamin B12 levels, and resulted in decreased serum methylmalonic acid and homocysteine concentrations, and decreased pain intensity, memory

Proton pump inhibitor use and its effect on vitamin B12 and homocysteine levels among men and women: A large cross-sectional study.

BACKGROUND: Previous studies have demonstrated an association between proton pump inhibitors (PPI) use and vitamin B12 deficiency. However, data regarding PPI use and elevated serum homocysteine level, an important marker of vitamin B12 deficiency, are scant. METHODS: Data were collected from medical records of subjects examined at a screening center in Israel. Cross sectional analysis was conducted on 25,953 subjects. Levels of vitamin B12 and homocysteine were compared between subjects who consumed PPI medications and those who did not. RESULTS: The mean age of the study population was 45

Among vitamin B12 deficient older people, high folate levels are associated with worse cognitive function: combined data from three cohorts.

BACKGROUND: Folate fortification of food aims to reduce the number of babies born with neural tube defects, but has been associated with cognitive impairment when vitamin B12 levels are deficient. Given the prevalence of low vitamin B12 levels among the elderly, and the global deployment of food fortification programs, investigation of the associations between cognitive impairment, vitamin B12, and folate are needed. OBJECTIVE: To investigate the associations of serum vitamin B12, red cell folate, and cognitive impairment. METHODS: Data were collected on 1,354 subjects in two studies investi

[Nutrient supplements--possibilities and limitations: part 2].

Pregnant women are at greater risk of an insufficient vitamin and mineral supply. Based on hemodynamic, endocrine and metabolic changes due to pregnancy, the body weight and blood volume increase. These changes result in an increased requirement of most vitamins and minerals while the energy requirement increases by about 10%. Besides iodine (recommended intake as supplement 150 microg/d), iron (recommended intake 30-40 mg/d), vitamin D (recommended intake as supplement 20-50 microg/d), and docosahexaenoic acid (recommended intake 200 mg/d), folic acid is one of the critical micronutrients dur

Dietary supplementation with apple juice decreases endogenous amyloid-beta levels in murine brain.

Folate deficiency has been associated with age-related neurodegeneration. We demonstrate herein that dietary deficiency in folate and vitamin E, coupled pro-oxidant stress induced by dietary iron, increased amyloid-beta (Abeta) levels in normal adult mice. This increase was potentiated by apolipoprotein E (ApoE) deficiency as shown by treatment of transgenic mice homozygously lacking murine ApoE. Dietary supplementation with apple juice concentrate in drinking water alleviated the increase in Abeta for both mouse genotypes. These findings provide further evidence linking nutritional and geneti
