A 63-year-old gentleman presented with a history of worsening left knee pain almost 5 years following bilateral uni-compartmental knee replacements. X-Rays revealed a fracture of the medial tibial plateau and revision surgery was undertaken. A 30-year-old woman with direct carotid cavernous fistula underwent endovascular treatment with detachable balloons via a transarterial route. The patient returned with diplopia 1 year after therapy. On cranial MR imaging, one of the balloons was detected in the proximal portion of the superior ophthalmic vein and was deflated percutaneously with a 22-gauge Chiba needle under CT guidance. The patient's symptoms resolved after balloon deflation. This case report presents a unique complication of endovascular treatment of direct carotid cavernous fistula and its management.

A 42-year-old man with human immunodeficiency viral infection developed cytomegaloviral retinitis that was complicated by retinal detachment and was treated with an intravitreous injection of silicone. Polymethyl methacrylate extrusion through a small tibial defect occurred during revision of a long-stemmed total knee arthroplasty. It caused necrosis and ulceration of the overlying skin. Resection of the protruding cement, debridement of the ulcer, and primary closure effected a cure. This is the first report of cement-induced skin necrosis and first report of tibial cement extrusion during revision of a total knee arthroplasty. Subcutaneous cement extrusion should be considered in the differential diagnosis of pretibial ulcers following total knee arthroplasty with cemented stems.

Seventy-one hips with osteoarthritis secondary to congenital dislocation underwent bipolar arthroplasty with autologous bone grafting (grafted group) or without bone grafting (ungrafted group). In 1984, as an alternate to the acrylic fixation of the components of total hip arthroplasty, the use of an uncemented porous-coated anatomic (PCA) hip system with a beaded porous coating to achieve biologic fixation was initiated. Since then, 44 patients with cementless acetabular components and 35 patients with cementless femoral components have been followed for a minimum of 24 months and an average of 37 months. Although esophageal perforation in children is associated with a spectrum of disease different from the one in adults, management is essentially the same for both groups. Over the past 11 years, 12 patients ranging in age from 2 days to 10 years were treated for 13 perforations. Perforation in the adult is associated with a high mortality rate. BACKGROUND: Hem-o-Lok clips (HOLCs) are widely used in minimal access urological operations due to the advantage of vascular control and suture stabilization.
