Detailed analysis of contrast-enhanced MRI of hands and wrists in patients with psoriatic arthritis.

OBJECTIVE: The objective was to perform detailed analysis of the involved soft tissues, tendons, joints, and bones in the hands and wrists of patients with psoriatic arthritis (PsA). MATERIALS AND METHODS: We reviewed 23 contrast-enhanced MR imaging studies (13 hands and 10 wrists) in 10 patients with the clinical diagnosis of PsA. We obtained clinical information from medical records and evaluated images for the presence of erosions, bone marrow edema, joint synovitis, tenosynovitis, carpal tunnel, and soft tissue involvement. Two board-certified musculoskeletal radiologists reviewed all ima

Single photon emission computerized tomography and reverse gantry computerized tomography findings in patients with back pain investigated for spondylolysis.

BACKGROUND: Athletes with low back pain may have sustained a bone stress injury in the lumbar neural arch, including spondylolysis. Single photon emission computerized tomography (SPECT) and reverse gantry computerized tomography (rg-CT) help localize bone stress response and spondylolyses. OBJECTIVES: To describe the SPECT and rg-CT findings in those patients suspected of suffering from a spondylolysis. STUDY DESIGN: Large retrospective case series. SETTING: Secondary and tertiary care center in England. PATIENTS: A total of 213 patients presenting to the Spinal Unit and Sports Medicine C

Reinfection with SARS-CoV-2 in the Omicron Era is Associated with Increased Risk of Post-Acute Sequelae of SARS-CoV-2 Infection: A RECOVER-EHR Cohort Study.

IMPORTANCE: Post-acute sequelae of SARS-CoV-2 infection (PASC) remains a major public health challenge. While previous studies have focused on characterizing PASC and identifying its subphenotypes in children and adolescents following an initial SARS-CoV-2 infection, the risks of PASC with Omicron-variant reinfections remain unclear. Using a real-world data approach, this study investigates the risks of PASC following reinfections during the Omicron phase in the pediatric population. OBJECTIVE: To investigate the risks of PASC diagnosis and 24 PASC symptoms and conditions after reinfection of

A matter arising: When should inflammatory and autoimmune rheumatic diseases be considered 'early'?

BACKGROUND: Early diagnosis is pivotal for guiding the intensity of clinical monitoring, optimizing therapeutic strategies and preventing organ damage in inflammatory and autoimmune rheumatic diseases (IARDs). This review summarizes current evidence on early diagnostic and therapeutic approaches of some IARDs, including rheumatoid arthritis (RA), systemic sclerosis (SSc) and detection of large-vessel vasculitis (LVV) in polymyalgia rheumatica (PMR), representing distinct pathophysiological mechanisms of joint synovitis, tissue fibrosis and vasculitis, respectively. METHODS: A comprehensive na

MRI findings of peroneal tendon tears do not necessarily correlate to clinical findings in paediatric and adolescent patients.

PURPOSE: Pathologic abnormality of the peroneal tendons are thought to be an under-appreciated source of vague ankle and hindfoot pain in paediatric patients, partly because they can be difficult to diagnose and differentiate from lateral ankle ligament injuries. While magnetic resonance imaging (MRI) is the primary imaging modality used to detect peroneal tendon pathology, previous studies in adults have found that positive MRIs demonstrate a positive predictive value (PPV) of associated clinical findings around 48%. There are no similar known published studies in the paediatric population. O

Ligamentous laxity in children with achondroplasia: Prevalence, joint involvement, and implications for early intervention strategies.

Achondroplasia (ACH), the most common form of skeletal dysplasia, is characterized by severe disproportionate short stature, rhizomelia, exaggerated lumbar lordosis, brachydactyly, macrocephaly with frontal bossing and midface hypoplasia. Ligamentous laxity has been reported as a striking feature of ACH, but its prevalence and characteristics have not been systematically evaluated yet. There is growing evidence that ligamentous laxity can be associated with chronic musculoskeletal problems and may affect motor development leading to abnormal developmental trajectories. This study aimed to asse

Assess perception, practice and lived experiences on use of corrective braces among parents of children diagnosed with clubfoot: A mixed method study.

BACKGROUND: Congenital talipes equinovarus (CTEV) is a congenital musculoskeletal deformity, which further leads to abnormal alignment of the feet in children. The aim of the present study is to assess perception and practice and explore lived experiences on the use of corrective braces among parents of children diagnosed with clubfoot. MATERIAL AND METHODOLOGY: A mixed method study approach with convergent parallel research design was used on 100 parents of children with clubfoot. For the quantitative strand, a total enumerative sampling technique was used to collect the data using self-stru

Orthopaedic Problems in 35 Patients With Organic Acid Disorders.

INTRODUCTION: Organic acid disorders (OADs) are a subset of inborn errors of metabolism that result in a toxic accumulation of organic acids in the body, which can lead to metabolic derangements and encephalopathy. Patients with these disorders are managed by a team of biochemical geneticists and metabolic nutritionists. However, subspecialists such as neurologists and orthopaedic surgeons are often needed to help manage the sequelae of the metabolic derangements. The breadth of orthopaedic sequelae of these disease states is poorly understood. Herein, we describe orthopaedic problems associat

Exome Sequencing for Prenatal Diagnosis in Nonimmune Hydrops Fetalis.

BACKGROUND: The cause of most fetal anomalies is not determined prenatally. Exome sequencing has transformed genetic diagnosis after birth, but its usefulness for prenatal diagnosis is still emerging. Nonimmune hydrops fetalis (NIHF), a fetal abnormality that is often lethal, has numerous genetic causes; the extent to which exome sequencing can aid in its diagnosis is unclear. METHODS: We evaluated a series of 127 consecutive unexplained cases of NIHF that were defined by the presence of fetal ascites, pleural or pericardial effusions, skin edema, cystic hygroma, increased nuchal translucency

A pilot study of myofascial release therapy compared to Swedish massage in fibromyalgia.

Fibromyalgia (FM) is characterized by widespread muscle pain and soft tissue tenderness. However, a lack of definitive muscle pathology has made FM both a diagnostic and a treatment puzzle. Much of the evidence for pathology in FM lies in the central nervous system - in particular abnormal amplification of pain signals in the spinal cord - a manifestation of central sensitization. An emerging body of evidence posits that peripheral pain generated from the muscles and fascia may trigger and maintain central sensitization in FM. Since FM patients so frequently seek manual therapy to relieve musc

UTE-T2∗ mapping detects sub-clinical meniscus injury after anterior cruciate ligament tear.

OBJECTIVE: Meniscus tear is a known risk factor for osteoarthritis (OA). Quantitative assessment of meniscus degeneration, prior to surface break-down, is important to identification of early disease potentially amenable to therapeutic interventions. This work examines the diagnostic potential of ultrashort echo time-enhanced T2∗ (UTE-T2∗) mapping to detect human meniscus degeneration in vitro and in vivo in subjects at risk of developing OA. DESIGN: UTE-T2∗ maps of 16 human cadaver menisci were compared to histological evaluations of meniscal structural integrity and clinical magnetic resona

Chronic neck pain disability due to an acute whiplash injury.

Several theories about musculoskeletal pain syndromes such as whiplash-associated disorder (WAD) suggest that pain and muscle activity interact and may contribute to the chronicity of symptoms. Studies using surface electromyography (sEMG) have demonstrated abnormal muscle activation patterns of the upper trapezius muscles in the chronic stage of WAD (grade II). There are, however, no studies that confirm that these muscle reactions are initiated in the acute stage of WAD, nor that these muscle reactions persist in the transition from acute neck pain to chronic neck pain disability. We analyze

Improved visualization of the middle glenohumeral ligament of the shoulder with internal rotation in axial MR arthrography: a comparative positional analysis.

OBJECTIVES: This study investigates the contribution of rotational maneuvers of the shoulder joint to the MR arthrographic visualization of the middle glenohumeral ligament (MGHL). METHODS: In this prospective study, 181 MR arthrography examinations referred to our clinic between May 2019 and January 2024 were evaluated for the MGHL. In addition to conventional MR arthrography sequences, axial MR arthrography images were obtained in internal rotation, external rotation, and neutral positions of the shoulder to demonstrate the MGHL. The arthrographic images obtained on a 3T MR scanner were ana

Persistently low serum alkaline phosphatase in adults: prevalence and clinical characteristics in a large tertiary care hospital.

BACKGROUND: Alkaline phosphatase (ALP) plays an essential role in skeletal mineralization and bone metabolism. While elevated ALP levels are commonly investigated in clinical practice, persistently low ALP values are often overlooked despite their potential association with hypophosphatasia (HPP), a rare metabolic bone disorder caused by pathogenic variants in the ALPL gene. This study aimed to determine the prevalence and clinical characteristics of adults with persistently low ALP in a large tertiary care hospital. METHODS: We conducted a retrospective review of adult patients who underwent

MRI of Knee Joint Lesions: An Observational Study of Traumatic, Degenerative, Cystic, and Neoplastic Pathologies.

Background Knee joint disorders are a major cause of musculoskeletal morbidity and functional disability worldwide, involving a broad spectrum of traumatic, degenerative, cystic, and neoplastic pathologies. Accurate evaluation of these conditions is essential for appropriate clinical assessment. MRI provides detailed visualization of intra-articular and periarticular structures due to its superior soft tissue contrast and multiplanar capability. However, there remains a need for region-specific observational data describing the spectrum of MRI findings in routine clinical practice. Materials a

Markerless gait analysis for children and adolescents with juvenile idiopathic arthritis using a machine learning pipeline.

BACKGROUND: Musculoskeletal diseases, such as Juvenile Idiopathic Arthritis (JIA), can lead to altered joint mobility and changes in gait pattern. While marker-based motion capture systems are considered the gold standard, their use is limited by high costs, technical complexity, time-intensive procedures, and specialized staff requirements. This study proposes and evaluates the feasibility of a markerless gait analysis approach for children of all ages, based on a machine learning pipeline, validates it against marker-based systems, and describes JIA-related gait differences. METHODS: Sagitta

Altered Central Motor Control After ACL Reconstruction: A Muscle Synergy Analysis During High-Risk Landing.

PURPOSE: Anterior cruciate ligament reconstruction (ACLR) patients face elevated secondary injury risk due to residual neuromuscular deficits, yet the neuromuscular coordination strategies underlying altered muscle coordination remain unclear. Muscle synergy (MS) analysis offers a valuable framework for elucidating these mechanisms. This study characterized MS patterns in ACLR patients during high-risk landing and their correlation with lower-extremity biomechanics. METHODS: Twenty-two patients who had undergone unilateral ACLR performed a single-leg drop-land-lateral jump task using both aff
