Using stochastic resonance and strength training as part of a rehabilitation programme for recurrent low back pain treatment: a case study.

Low back pain (LBP) is a common disabling health problem that can cause decreased spine proprioception. Stochastic resonance (SR) can influence detection performance, besides improving patients with significant sensory deficits, but have not been thoroughly tested for LBP. This study aimed to examine the application of SR therapy (SRT) and strength training for LBP treatment. The subject was a resistance-trained male in his early thirties. His back pain was unbearable after a strength training session. Standard pain relief alleviated the pain but the LBP developed at a similar intensity after …

Activating the somatosensory system enhances net quadriceps moment during gait.

Quadriceps muscle rehabilitation following knee injury or disease is often hampered by pain, proprioception deficits or instability associated with inhibition of quadriceps activation during walking. The cross-modal plasticity of the somatosensory system with common sensory pathways including pain, pressure and vibration offers a novel opportunity to enhance quadriceps function during walking. This study explores the effectiveness of an active knee brace that used intermittent cutaneous vibration during walking to enhance the peak knee flexion moment (KFM) during early stance phase as a surrog…

Local high-frequency vibration therapy following eccentric exercises reduces muscle soreness perception and posture alterations in elite athletes.

PURPOSE: Exercise-induced muscle damage produces painful sensations (delayed onset of muscle soreness, DOMS). DOMS causes compensatory postural adaptations, which in turn affect athletes' walking and running gait biomechanics. It is still debated whether the postural changes are due to impaired proprioception or pain perception. To disambiguate between these two contrasting hypotheses, we designed a study that tested post-exercise postural adjustments in two groups of athletes: a group who was administered a vibration therapy (VT), to attenuate pain perception, and a control group. METHODS: T…

Predictors of recurrent sprains after an index lateral ankle sprain: a longitudinal study.

OBJECTIVE: To explore and identify the predictors of ankle sprain after an index (first) lateral ankle sprain. DESIGN: Prospective cohort study, Level of evidence II. SETTING: Musculoskeletal research laboratory at the University of Sydney. PARTICIPANTS: A sample of convenience (70 controls, 30 with an index sprain) was recruited. METHODS: Potential predictors of ankle sprain were measured including: demographic measures, perceived ankle instability, ankle joint ligamentous laxity, passive range of ankle motion, balance, proprioception, motor planning and control, and inversion/eversion pe…

Conversely natural teeth, even in a compromised clinical condition, maintain proprioception and can be adapted to the stresses of functional loading. The most demanding task in daily clinical practice is the treatment plan. The role of the clinician is more complicated for the anterior teeth where the patient's expectations and the estheti …

Auditory compensation for head rotation is incomplete.

Hearing is confronted by a similar problem to vision when the observer moves. The image motion that is created remains ambiguous until the observer knows the velocity of eye and/or head. One way the visual system solves this problem is to use motor commands, proprioception, and vestibular information. These "extraretinal signals" compensate for self-movement, converting image motion into head-centered coordinates, although not always perfectly. We investigated whether the auditory system also transforms coordinates by examining the degree of compensation for head rotation when judging a moving…

Reliability and Validity of the Sensory Component of the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI): A Systematic Review.

BACKGROUND: Since 1982, the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) has been used to classify sensation of spinal cord injury (SCI) through pinprick and light touch scores. The absence of proprioception, pain, and temperature within this scale creates questions about its validity and accuracy. OBJECTIVE: To assess whether the sensory component of the ISNCSCI represents a reliable and valid measure of classification of SCI. METHODS: A systematic review of studies examining the reliability and validity of the sensory component of the ISNCSCI publ…

Disruption of visuospatial and somatosensory functional connectivity in anorexia nervosa.

BACKGROUND: Although body image disturbance is considered one of the core characteristics of anorexia nervosa (AN), the exact nature of this complex feature is poorly understood. Task-related functional magnetic resonance imaging studies can only partially explore the multimodal complexity of body consciousness, which is a complex cognition underpinned by aspects of visual perception, proprioception, and touch. The aim of the present study was to explore the functional connectivity of networks involved in visuospatial and somatosensory processing in AN. METHODS: Twenty-nine subjects with AN, …

It is important to realize this when prescribing partial weight bearing in a particular splint as this may result in avoidable complications.

Proximal tibia reconstruction with a porous tantalum cone in a patient with Charcot arthropathy.

Neuropathic arthritis, or Charcot arthropathy, is a rapidly destructive process associated with nerve damage and periarticular insensitivity. Most commonly it is associated with diabetes mellitus, tabes dorsalis, syringomyelia, or other peripheral nerve disorders. Clinical manifestations of this disorder classically include gross deformity, crepitus, lack of proprioception, joint effusion, calor, and decreased awareness of pain in the affected joint. Often the clinical picture is difficult to distinguish from septic arthritis. Radiographic findings are typically worse than portrayed during cli…

STUDY DESIGN: Within-session, within-subject comparison of 2 conditions. OBJECTIVES: To determine the influence of application of a semirigid ankle brace on the excitability of the peroneus longus muscle motoneuron pool as measured by the H-reflex. BACKGROUND: The literature suggests that cutaneous mechanoreceptors can contribute to proprioception, especially during conditions in which skin experiences displacement. Further, skin displacement and stimulation of cutaneous mechanoreptors have been shown to increase motoneuron excitability. METHODS AND MEASURES: H-reflexes and M-waves of the p…
