Assessment of White Matter Changes Using Quantitative T1ρ Mapping in an Open-Field Low-Intensity Blast Mouse Model of Mild Traumatic Brain Injury (mTBI).

Blast-induced mild traumatic brain injury (mTBI) occurs when shock waves travel through blood vessels and cerebrospinal fluid, leading to cerebral demyelination, which results in cognitive impairments and neuropsychiatric issues that impact quality of life. This study aims to evaluate myelin changes in white matter in mice with mTBI induced by an open-field low-intensity blast (LIB) using a newly implemented 3D adiabatic T1ρ prepared fast spin echo (Adiab-T1ρ-FSE) sequence for quantitative T1ρ MRI mapping. Thirty male C57BL/6 mice, including 15 mTBI and 15 sham controls, were scanned on a 3T B

Severe Craniocerebral Injury in a Child Caused by Repetitive Cleaver Strikes: Case Report and Review of the Literature.

Severe craniocerebral injuries are rare in the pediatric population, especially open craniocerebral injuries caused by repeated slashes with sharp instruments. Here, we report an exceptional case of a child with severe open craniocerebral injury caused by repeated chopping blows with a cleaver. The child was admitted with traumatic hemorrhagic shock, and a multidisciplinary team (MDT) was immediately organized for evaluation and treatment decisions. Preoperative imaging showed a comminuted skull fracture accompanied by extensive scalp laceration and brain tissue injury with active hemorrhage.

Hydrogen sulfide mitigates memory impairments via the restoration of glutamatergic neurons in a mouse model of hemorrhage shock and resuscitation.

Impaired long-term memory, a complication of traumatic stress including hemorrhage shock and resuscitation (HSR), has been reported to be associated with multiple neurodegenerations. The ventral tegmental area (VTA) participates in both learned appetitive and aversive behaviors. In addition to being prospective targets for the therapy of addiction, depression, and other stress-related diseases, VTA glutamatergic neurons are becoming more widely acknowledged as powerful regulators of reward and aversion. This study revealed that HSR exposure induces memory impairments and decreases the activati

Rapid prediction of secondary neurologic decline after traumatic brain injury: a data analytic approach.

Secondary neurologic decline (ND) after traumatic brain injury (TBI) is independently associated with outcome, but robust predictors of ND are lacking. In this retrospective analysis of consecutive isolated TBI admissions to the R. Adams Cowley Shock Trauma Center between November 2015 and June 2018, we aimed to develop a triage decision support tool to quantify risk for early ND. Three machine learning models based on clinical, physiologic, or combined characteristics from the first hour of hospital resuscitation were created. Among 905 TBI cases, 165 (18%) experienced one or more ND events (

A Rare Case of Spontaneous Bilateral Subcapsular Renal Hematoma.

Spontaneous bilateral renal subcapsular hematoma is a rare condition. On literature review, only 2 case reports have elucidated possible etiologies for such a presentation; however, no definite conclusions have been made. We present a rare case of a 52-year-old female with diabetes mellitus type 2, chronic kidney disease stage 4, hypertension, hyperlipidemia, prior traumatic brain injury via motor vehicle accident, who presented to our hospital with diabetic ketoacidosis and clinical signs of pyelonephritis; subsequently, imaging demonstrated spontaneous bilateral renal subcapsular hematoma. Subclavian artery avulsion following blunt trauma: A case report and literature review.

INTRODUCTION: Subclavian artery injuries are rare and are associated with a high morbidity and mortality. The majority of patients with blunt trauma to the subclavian artery succumb to their injury before reaching a hospital. In-hospital mortality remains high. PRESENTATION OF CASE: We report the case of a 30-year-old male who presented with complete traumatic subclavian artery avulsion after a motorcycle collision. He presented in hemorrhagic shock. Temporary hemostatic control was achieved with endovascular balloon occlusion followed by operative intervention. DISCUSSION: Prompt diagnosis

Oxygen-Glucose Deprivation (OGD) Modulates the Unfolded Protein Response (UPR) and Inflicts Autophagy in a PC12 Hypoxia Cell Line Model.

Hypoxia is the lack of sufficient oxygenation of tissue, imposing severe stress upon cells. It is a major feature of many pathological conditions such as stroke, traumatic brain injury, cerebral hemorrhage, perinatal asphyxia and can lead to cell death due to energy depletion and increased free radical generation. The present study investigates the effect of hypoxia on the unfolded protein response of the cell (UPR), utilizing a 16-h oxygen-glucose deprivation protocol (OGD) in a PC12 cell line model. Expression of glucose-regulated protein 78 (GRP78) and glucose-regulated protein 94 (GRP94),

A case of splenic rupture: a rare event after laparoscopic cholecystectomy.

BACKGROUND: Laparoscopic cholecystectomy (LC) is generally safe and well-accepted. In rare cases, it is associated with complications (intra- e postoperative bleeding, visceral injury and surgical site infection). Splenic lesion has been reported only after direct trauma. We report an unusual case of splenic rupture presenting after "uncomplicated" LC. CASE PRESENTATION: A 77-year-old woman presented with distended abdomen, tenderness in the left upper quadrant and severe anemia 12 hours after LC. Clinical examination revealed hypovolemic shock. Abdominal computed tomography confirmed the dia

Proteomic and phosphoproteomic analyses of the soluble fraction following acute spinal cord contusion in rats.

Traumatic spinal cord injury (SCI) causes marked neuropathological changes in the spinal cord, resulting in limited functional recovery. Currently, there are no effective treatments, and the mechanisms underlying these neuropathological changes are not completely understood. In this study, two-dimensional gel electrophoresis coupled with mass spectrometry was used to investigate injury-related changes in the abundance (SYPRO Ruby stain) and phosphorylation (Pro-Q Diamond stain) of proteins from the soluble fraction of the lesion epicenter at 24 h following SCI. Over 1500 SYPRO Ruby-stained spo

Gastrocnemius injury during running: a case report.

Incidence rate of running related injuries is high but forces acting on the body during injury are hardly known. During a data collection session in the investigator's laboratory, one subject sustained a medial gastrocnemius muscle strain injury while running across a force platform. This case report aimed to investigate the causes of this acute running injury by comparing the running speed, external forces, joint kinetics and kinematics, and muscle-tendon complex length of the injured trial to five preceding noninjured trials. The injured trial was characterized by faster running speed, highe

Fatal Clostridium septicum infection in a patient with non-Hodgkin's lymphoma undergoing multimodal oncologic therapy.

BACKGROUND: Clostridium septicum infections are rare but often fatal. They are known to be associated with a compromised immune system, e.g. with hematologic malignancies. CASE REPORT: We report on a spontaneous, non-traumatic C. septicum infection in a patient with non-Hodgkin's lymphoma undergoing irradiation due to abdominal bulk. Irradiation was delivered in daily fractions of 1.8 Gy. After 7 fractions, the patient suddenly developed severe pain in the lower leg. Clinical examination showed no pathological findings. However, a rapid progression of symptoms with motoric and sensoric defici

Acceleration induced head injury in the monkey.I. The model, its mechanical and physiological correlates.

A system has been developed which is capable of inducing brain injuries of graded severity from mild concussion to instantaneous death. A pneumatic shock tester subjects a monkey to a non-impact controlled single sagittal rotation which displaces the head 60 degrees in 10-20 msec. Results derived from 53 experiments show that a good correlation exists between acceleration delivered to the head, the resultant neurological status and the brain pathology. A simple experimental trauma severity (ETS) scale is offered based on changes in the heart rate, respiratory rate, corneal reflex and survivabi

Penetrating injuries to the iliac arteries.

Despite advances in the management of traumatic truncal and peripheral vascular injuries, penetrating trauma to the iliac arteries carries a high mortality. Among more than 600 patients with arterial trauma seen at the Ben Taub General Hospital between January 1958 and December 1977, eighty-three had penetrating injury to the iliac arteries. Thirty-two patients (39 per cent) died within thirty days of injury, none of these dying within 48 hours of injury. Injuries were managed by resection and end-to-end anastomosis (36 per cent), lateral arteriorrhaphy (27 per cent), ligation (20 per cent), a

Factors influencing immediate prognosis in acute renal failure, with special reference to prophylactic hemodialysis.

Five hundred patients with acute renal failure were admitted to the Department of Nephrology of Necker Hospital between 1966 and 1970; 279 of them were treated before and 221 after beginning systematic prophylactic hemodialysis. The frequency of complications and the survival rate between both groups were compared. No significant improvement was obtained in patients with severe pre-existing diseases or extensive extrarenal lesions; such patients usually died from nonuremic complications before, or despite, dialysis. However, adequate treatment of severe septic shock, when present, was an impor

Synaptic GABA dysfunction of thalamocortical neurons impairs sleep spindle morphology and recovery from fearful memories.

Sleep spindles are rhythmic electroencephalographic signatures of non-rapid-eye-movement sleep. Their dysregulation has been implicated in several neuropsychiatric illnesses. Spindles have a characteristic waxing and waning shape, but the cellular and circuit mechanisms controlling their shape are not well understood. Recent but sparse research has implied that sleep spindle shape becomes abnormal in post-traumatic stress disorder (PTSD). PTSD patients have dysfunctional GABA A receptors in midline thalamic regions, areas involved in the orchestration of sleep spindles. We modelled this GABA A

Association between prehospital field to emergency department delta shock index and in-hospital mortality in patients with torso and extremity trauma: A multinational, observational study.

Hemorrhage, a main cause of mortality in patients with trauma, affects vital signs such as blood pressure and heart rate. Shock index (SI), calculated as heart rate divided by systolic blood pressure, is widely used to estimate the shock status of patients with hemorrhage. The difference in SI between the emergency department and prehospital field can indirectly reflect urgency after trauma. We aimed to determine the association between delta SI (DSI) and in-hospital mortality in patients with torso or extremity trauma. Patients with DSI >0.1 are expected to be associated with high mortality.
