Bilateral foot center of pressure measures predict hemiparetic gait velocity.

Stroke is a major cause of disability in the US, leaving most survivors with abnormal motor function, often resulting in hemiparetic gait. Quality of gait in this population has been well characterized by measures of velocity, though velocity alone provides only a general index of functional mobility. Researchers and clinicians should continue to develop a complete characterization of behavioral alterations, but should also work to identify the neuromechanical processes underlying the loss and recovery of locomotor function. We hypothesized that selected foot center of pressure (CoP) measures

Ankle arthrodesis for congenital absence of the fibula.

THE PROBLEM: Bilateral congenital absence of the fibula in a 10-year-old boy. A marked valgus malalignment at the left ankle and a foot with three rays caused pain during standing and walking. Ortheses did not help. Therefore, various treatment options were considered such as amputation of the foot, a supramalleolar correction osteotomy, and a tibiotalar arthrodesis. THE SOLUTION: Correction of malalignment and ankle arthrodesis stabilized with an external mini-fixator while sparing the distal tibial physis. SURGICAL TECHNIQUE: Two skin incisions: one on the medial side visualizing the flexo

The oblique proximal phalangeal osteotomy in the correction of hallux valgus.

An oblique proximal phalangeal osteotomy is introduced as an alternative to the Akin procedure for the treatment of hallux valgus. It consists of a single oblique osteotomy directed from proximal-dorsal to plantar-distal. A retrospective review of 32 patients who underwent 36 oblique proximal osteotomies is presented. The mean follow-up was 11 months (range, 3 to 21 months). Pre- and postoperative radiographic measurements of the distal articular set angles showed an average correction angle of 12 degrees (range, 7 degrees to 22 degrees; SD = 2.99). Mean range of first metatarsophalangeal join

Operative treatment of interdigital neuroma. A long-term follow-up study.

BACKGROUND: The literature regarding the outcome of surgical treatment of interdigital neuroma is incomplete. The purpose of this study was to assess the demographics associated with the presentation of an interdigital neuroma as well as the long-term clinical results of operative resection by a single surgeon. METHODS: A retrospective review of the patient records of one orthopaedic foot and ankle surgeon identified eighty-two patients who had been treated operatively for a primary, persistently painful interdigital neuroma more than three years previously. Of these eighty-two patients, sixt

Conservative treatment measures in hemophilic arthropathy.

Intraarticular bleeding and muscle bleeding occur spontaneously or as a result of trauma in hemophilia A or B. The most common sites for hemarthrosis are the knees joints, elbows and ankles joints. Resorption of intraarticular blood induces reactive hemophilic synovitis. Hyperplasia of the synovium can be followed by recurrent bleedings. These early reactions can change to a chronic synovitis and cause cartilage damage, finally resulting in a complete destruction of the joint. Since 1981 158 adults and 61 children with a hemophilia A or B were treated at the Center for hemophilic disorders Fra

Multistep measurement of plantar pressure alterations using metatarsal pads.

Metatarsal pads are frequently prescribed for nonoperative management of metatarsalgia due to various etiologies. When appropriately placed, they are effective in reducing pressures under the metatarsal heads on the plantar surface of the foot. Despite the positive clinical reports that have been cited, there are no quantitative studies documenting the load redistribution effects of these pads during multiple step usage within the shoe environment. The objective of this study was to assess changes in plantar pressure metrics resulting from pad use. Ten normal adult male subjects were tested du

[Malum perforans diabeticorum: treatment with controlled circulatory blockade and regional intravenous administration of antibiotics].

Orthopedic deformations of the feet play an important role in the etiology of the foot ulcers of diabetic patients, together with neuropathy and vascular impairment. The employment of new techniques in the treatment of diabetic foot has considerably improved its prognosis. These are the transient circulatory blockade (TCB) and the regional intravenous antibiotic administration (RAA). This paper reports on the usage of these techniques in the treatment of 24 diabetic patients, 8 women with ages between 41 and 75 (mean 54.7) years, and 16 men with ages between 41 and 75 (mean 59.3) years who wer

Foot pressure distribution during walking and shuffling.

The insensate foot is vulnerable to tissue damage from areas of repetitive, excessive pressures. It has been previously stated that a shuffling gait with short steps would increase the period of foot flat and thus minimize any excessive local plantar pressures. This theory was quantitatively evaluated with a portable, in-shoe pressure data-acquisition system. Seven pressure sensors were located in the left and right insoles under the metatarsal heads, hallux, and posterior and anterior heels. Plantar pressure data were acquired from ten able-bodied subjects during four minutes of continuous sh

Reliability of medial versus lateral video analysis for footstrike assessment in treadmill and overground running.

Although video analysis is widely used to assess footstrike pattern (FSP) and footstrike angle (FSA), the reliability of these measures from medial and lateral views remains poorly understood, particularly across treadmill and overground running. This study evaluated the intra-rater, inter-rater and inter-side reliability of FSP classification and FSA measurement between treadmill and overground running. This study involved seven novice runners on an indoor treadmill and nine experienced runners during an outdoor marathon-distance run. Synchronized medial- and lateral-view videos were recorded

Analysis of time-varying impact points in soccer kicking using curved foot geometry.

The impact phase in soccer kicking, while extremely brief, determines both ball velocity and rotation. Quantifying the moment governing this rotation requires precise, time-resolved identification of the impact point; however, a robust methodology has not yet been established. This study developed a more accurate approach to determine the impact point by incorporating the curved geometry of the foot's impact area, enabling comprehensive calculation of mechanical variables of ball motion. To capture foot surface geometry under static conditions, 105 reflective markers were applied at high densi

Research on surrounding rock deformation characteristics and support optimization measures for tunnel TBM crossing through fault fracture zones.

The tunnel boring machine (TBM) is a core equipment for mountain tunnel engineering, but it often faces problems such as surrounding rock deformation, collapse, and TBM entrapment when crossing fault fracture zones. Taking the TBM crossing of F1 fault in the Pilot Tunnel of Daliangshan No.1 Tunnel as the engineering case, this study adopted a combined method of laboratory tests, numerical simulation, and field monitoring to clarify the deformation characteristics of surrounding rock during TBM's passage through the fault fracture zone, and proposed and verified effective reinforcement measures

Stable Lord Madreporic Femoral Stem 32 Years After Revision Total Hip Arthroplasty: A Case Report.

The Lord total hip prosthesis was discontinued because of high rates of failure of the threaded acetabular component and marked proximal femoral bone atrophy related to stress shielding. We report an exceptionally long-term follow-up of a Lord femoral stem that remains stable 32 years after implantation. In 1994, a 37-year-old tailor underwent left revision total hip arthroplasty (THA) using the Lord system. Fifteen years later, in 2009, he presented in our orthopedic department with pain and substantial functional impairment. Imaging demonstrated aseptic loosening of the Lord acetabular cup w

Validity and reliability of running gait measurement with the ViMove2 system.

Running biomechanics have traditionally been analysed in laboratory settings, but this may not reflect natural running gait. Wearable technology has the potential to enable precise monitoring of running gait beyond the laboratory. This study aimed to evaluate the analytical validity and intra-session reliability of temporal running gait outcomes measured by the ViMove2 wearable system in healthy adults. Seventy-four healthy adults (43 males, 31 females, aged 18-55 years) wore the inertial device, ViMove2 on the tibia. Participants ran on a treadmill for one minute at various speeds (8, 10, 12,

Clinical characteristics and identification of novel CNOT1 variants in three unrelated Chinese families with Vissers-Bodmer Syndrome.

Vissers-Bodmer Syndrome, an autosomal dominant disease, is a neurodevelopmental disorder characterized by global developmental delay, intellectual disability, hypotonia and autistic features with a highly variable phenotype. It is caused by variants in the CCR4-NOT transcription complex, subunit 1 gene (CNOT1). However, the pathophysiologic mechanism of the Vissers-Bodmer Syndrome remains unclear. Notably, this syndrome has not been previously reported in the Chinese. In this study, we utilized whole exome sequencing to identify three novel variants in the CNOT1 gene, encompassing one frameshi

Molecular docking and MD simulations reveal protease inhibitors block the catalytic residues in Prp8 intein of Aspergillus fumigatus: a potential target for antimycotics.

Resistance to azoles and amphotericin B especially in Aspergillus fumigatus is a growing concern towards the treatment of invasive fungal infection. At this critical juncture, intein splicing would be a productive, and innovative target to establish therapies against resistant strains. Intein splicing is the central event for the activation of host protein, essential for the growth and survival of various microorganisms including A. fumigatus. The splicing process is a four-step protease-like nucleophilic cascade. Thus, we hypothesise that protease inhibitors would successfully halt intein spl
