Management of liver abscess at teaching hospital.

BACKGROUND: A current assessment of liver abscesses should allow for better understanding of the pathogenesis of the disease and improve the effectiveness of diagnosis and treatment. Amoebic liver abscess occurs more commonly than the pyogenic liver abscess on a worldwide basis. The purpose of the study is to investigate the clinical presentation and management option for liver abscess. METHODS: A prospective observational study was conducted from January 2007 to December 2010, in Kathmandu Medical College Teaching Hospital. We recorded and analyzed the clinical presentations, diagnostic moda

Disseminated mycobacterial infection in AIDS patients: abdominal US features and value of fine-needle aspiration biopsy of lymph nodes and spleen.

BACKGROUND: We evaluated the efficacy of abdominal ultrasound (US) and fine-needle aspiration biopsy (FNAB) in the diagnosis of disseminated mycobacteriosis (DM) in acquired immunodeficiency syndrome (AIDS). METHODS: Twelve AIDS patients (nine male, three female; age range, 22-43 years) with DM underwent abdominal US within 2 days after admission with 3.5- to 5-MHz convex probes and a 7.5-MHz linear probe. All patients underwent FNAB of one or two enlarged abdominal lymph nodes. Eight patients underwent FNAB of the spleen. The aspirated specimens were stained with acid fast for quick examinat

Mycobacterium avium Pleurisy with Bronchopleural Fistula Resulting in Rapidly Progressive Respiratory Failure Diagnosed by Transbronchial Autopsy.

Nontuberculous mycobacterial lung disease usually manifests as a chronic pulmonary infection. We herein report a fatal case of Mycobacterium avium pleurisy in a man with a refractory bronchopleural fistula that led to rapidly progressive pneumonia. A post-mortem transbronchial biopsy was performed. Histopathology revealed an acute lung injury pattern and epithelioid granulomas. Variable number tandem repeat analyses and drug susceptibility testing revealed Mycobacterium avium had acquired macrolide resistance during chemotherapy with rifampicin, ethambutol, and clarithromycin. Clinicians shoul

Soluble programmed cell death ligand-1 as a predictive biomarker for severity and poor prognosis in pulmonary tuberculosis.

BACKGROUND: We aimed to assess whether soluble programmed death-ligand 1 (sPD-L1) could serve as a new biomarker for PTB. METHODS: Plasma sPD-L1 levels in the discovery cohort were analyzed through flow cytometry and validated by sandwich ELISA. Pleural effusion sPD-L1 levels were measured using ELISA. RESULTS: In the discovery cohort, sPD-L1 levels in the severe (SE, n = 44), non-severe (non-SE, n = 34) and HC (n = 10) group were 67.41 (30.14-126.41), 26.75 (11.00-52.35) and 14.6 (10.78-21.91) pg/ml, respectively. The sPD-L1 levels in SE patients were significantly higher than those in both

In vitro pleural fluid clottability and fibrinogen content.

Twenty-three specimens of pleural fluid from 23 patients were examined for quantitative fibrinogen, total protein levels, and for clottability in vitro using the recalcification time. Of the 19 specimens of pleural fluid from patients without loculation, 11 (seven exudates) had no detectable fibrinogen; another 8 (six exudates) had a mean fibrinogen level of 55.0 +/- 10.2 mg percent, and a mean recalcification time of 19.4 +/- 2.6 minutes. The pleural fluids from the four patients with loculation had no detectable fibrinogen. The only fluids containing fibrinolytic activity were from the nonlo

Role of Medical Interventions in Complicated Parapneumonic Pleural Effusion and Empyema.

BACKGROUND: Complicated parapneumonic effusion (PPE) and empyema require intercostal drainage (ICD), failure of which usually leads to surgery. Intrapleural instillation of fibrinolytic agents (IFT) enhance pleural drainage and reduce the need for surgery. This study aimed to evaluate the role of medical interventions (ICD and IFT) in complicated PPE and empyema. METHODS: In a prospective, cohort study conducted at National Academy of Medical Sciences, Bir Hospital. Patients aged >18 years with complicated PPE and empyema were included. ICD was placed in all. In those with significant residua

Disseminated tuberculosis: Clinical presentation, diagnosis, and outcomes in a tertiary-care hospital in Saudi Arabia.

BACKGROUND: Tuberculosis (TB) is the primary infectious cause of mortality worldwide. Although TB incidence and prevalence are declining, the use of immunosuppressive drugs and the growing prevalence of immunocompromising conditions such as comorbidities, malignancies, and the use of immunosuppressive agents are risk factors for disseminated TB (DTB). This study aims to identify the relevant clinical, laboratory, radiological, and histopathological features of DTB, as well as to assess the typical anatomical distributions and treatment outcomes of patients diagnosed with the disease at King Ab

The development and validation of a diagnostic scoring system to differentiate pulmonary tuberculosis from non-tuberculosis pulmonary infections in HIV-infected patients with severe immune suppression.

BACKGROUND: It remains challenging to differentiate tuberculosis (TB) from non-TB pulmonary infections in HIV-infected patients. Herein, we developed a scoring system aimed to rapidly determine the likelihood of TB or non-TB pathology in HIV-infected patients presenting with pulmonary infections. METHODS: We collected and collated data of hospitalized HIV-infected patients with pulmonary infections, followed by univariate and multivariate data analyses to determine risk variables that were significantly different between HIV/TB patients and HIV/non-TB patients. Subsequently, a regression coef

Tuberculosis in children undergoing hemodialysis.

UNLABELLED: Tuberculosis (TB) remains a public health problem in Tunisia. Its incidence is higher in immunocompromised hosts than in the general population. In children and during hemodialysis, TB is characterized by the frequency of extrapulmonary localizations and diagnostic difficulties. The aim of this retrospective study is to evaluate the incidence of TB in Tunisian children undergoing hemodialysis and to determine its clinical features as well as the results of chemotherapy. METHOD: This retrospective study includes seven TB children among 112 children on hemodialysis at the pediatric

[Clinical evaluation of acid-fast smear examination with light emitting diode fluorescent microscopy].

OBJECTIVE: Fluorescent smear microscopy is one of the recommended methods to detect highly infectious tuberculosis (TB) patients. Recently, fluorescent microscopy using a light emitting diode (LED) as a light source has been developed and introduced. The objective of this study is to evaluate the efficiency of LED fluorescent microscopy. METHOD: The clinical specimens were collected from TB suspects and follow-up patients of mycobacteriosis in Double Barred Cross Hospital through Sept. to Oct. in 2008. The specimens were subjected to the ordinary decontamination/concentration process, and the

Pulmonary sporotrichosis: review of treatment and outcome.

Four culture-documented cases of pulmonary sporotrichosis, three primary infections and one with multisystem involvement, are presented. Two of these patients are the first reported cases of primary lung disease treated with ketoconazole. This antifungal agent appears to be ineffective in eradicating this infection. The four cases, as well as a review of the literature, illustrate several important aspects of this rare disease. Pulmonary sporotrichosis is most commonly found in males with a history of alcohol abuse who are between the ages of 30 and 60. The infection is usually confined to the

[DIFFERENTIATED TACTICS OF VIDEOTHORACOSCOPIC DIAGNOSIS OF THE PLEURAL EXUDATE SYNDROME].

Differentiated tactics of diagnostic videothoracoscopy (VTHS) in a pleural exudate syndrome, which ought to be treated with hydrothorax elimination and artificial pneumothorax creation, was proposed. Further roentgenological investigation permits to create a plan for the operation conduction and a certain anesthesia application. Criteria for the operation planning and the anesthesiological support choice were elaborated. Results of VTHS conduction in 261 patients in Department of Thoracic Surgery were analyzed. The differentiated tactics for the VTHS performance application have had saved the

Spontaneous haemoptysis as a late complication of plombage in a tuberculosis patient.

The endemic spread of tuberculosis after World War II and the deficiency of appropriate antituberculous drugs had led to a renaissance of the surgical tuberculosis therapy until the early 1950s. Late complications of plombage performed decades before are rare and are mainly related to infection and/or migration of the inserted foreign material and are scarcely recognized today. We report on a 73-year-old male patient, who was admitted to the emergency room of our hospital with acute massive haemoptysis for four days. On physical examination the patient presented with decreased breath sounds ov

Development and validation of artificial intelligence-assisted volumetric response criteria in pleural mesothelioma (ARTIMES): a retrospective, multicohort, multicentre study.

BACKGROUND: Response evaluation in pleural mesothelioma is challenging because its crescent growth pattern is poorly captured by diameter-based criteria. We aimed to develop and validate artificial intelligence (AI)-assisted volumetric response criteria (ARTIMES) based on automated tumour segmentation and biologically derived thresholds. METHODS: In this retrospective, multicentre study, we included 10 926 CT scans from 2080 patients from 14 cohorts. A subset totalling 1176 CT scans from routine care (Netherlands Cancer Institute-Antoni van Leeuwenhoek Hospital) and trial cohorts (INITIATE, N

[Role of surgery in the treatment of pulmonary aspergillosis].

INTRODUCTION: This article reviews the different forms of pulmonary disease caused by aspergillus and discusses the possible surgical treatments. The most well known is the classic aspergilloma which develops as a fungal ball in the centre of a pre-existing pulmonary cavity. STATE OF KNOWLEDGE: One can distinguish simple (few symptoms, thin walled cavity without immediate complications) and complex forms (patient generally unwell, thick cavity, complications). In the complex form, surgical intervention must be considered as a last resort. In the simple form, surgery is relatively benign and p

Ultrasound findings in Kaposi sarcoma patients: overlapping sonographic features with disseminated tuberculosis.

BACKGROUND: Focused Assessment with Sonography for HIV-associated TB (FASH) is a diagnostic tool for extra-pulmonary tuberculosis (TB) in symptomatic patients with advanced HIV. As Kaposi's sarcoma (KS) is also prevalent in this patient population, changes due to KS may mimic TB findings and clinical interpretation of target FASH findings can be challenging. We aimed to describe sonographic findings in patients with KS. METHODS: We performed a prospective observational study at Lighthouse clinic at Kamuzu Central Hospital, Lilongwe, Malawi, in consecutive patients with newly diagnosed KS, wit

Rethinking Annotation Granularity for Overcoming Shortcuts in Deep Learning-based Radiograph Diagnosis: A Multicenter Study.

PURPOSE: To evaluate the ability of fine-grained annotations to overcome shortcut learning in deep learning (DL)-based diagnosis using chest radiographs. MATERIALS AND METHODS: Two DL models were developed using radiograph-level annotations (disease present: yes or no) and fine-grained lesion-level annotations (lesion bounding boxes), respectively named CheXNet and CheXDet. A total of 34 501 chest radiographs obtained from January 2005 to September 2019 were retrospectively collected and annotated regarding cardiomegaly, pleural effusion, mass, nodule, pneumonia, pneumothorax, tuberculosis, f

Application of mNGS in the Etiological Diagnosis of Thoracic and Abdominal Infection in Patients With End-Stage Liver Disease.

BACKGROUND: Despite the obvious advantages of metagenomic next-generation sequencing (mNGS) in etiological diagnosis of various infectious diseases, there are few reports on etiological diagnosis of suspected thoracic and abdominal infections in patients with end-stage liver disease (ESLD). METHODS: Seventy-three ESLD patients were enrolled from January 2019 to May 2021 due to suspected complicated thoracic and abdominal infections with poor response to empirical anti-infective treatment. Pleural effusion and ascites samples of these patients were collected for mNGS detection and conventional
