Down beating nystagmus: magnetic resonance imaging and neuro-otological findings.

Twenty-four patients with down beating nystagmus (DBN) underwent magnetic resonance imaging (MRI) of the head. MRI provided diagnostic images in Arnold-Chiari malformation (6 cases), cerebellar atrophy (6 cases), 1 case with a prepontine-medullary epidermoid tumour and was helpful in the diagnosis of 2 patients with multiple sclerosis and 1 with a ponto-cerebellar infarct. Multiple cerebral hemisphere lesions were found in 6 patients (5 of them over 60 years of age) in whom no diagnosis was made. All cases of DBN, plus 3 additional patients with Arnold-Chiari malformation and other types of ny

Preservation of the auditory nerve function after translabyrinthine removal of vestibular schwannoma.

OBJECTIVE: To evaluate the function of the postoperative auditory nerve preserved after translabyrinthine (TL) vestibular schwannoma (VS) removal. METHODS: Fifteen patients, who underwent unilateral VS resection via a TL approach, were preserved auditory nerve anatomically. The size and location of VS were measured on MRI preoperatively. After surgery, the electrical stimulation test (EST) or electrically evoked auditory brainstem response (EABR) was performed. RESULTS: Four cases (27%) out of fifteen patients were retained the functional integrity of the auditory nerve after surgery. The ma

Vestibular evoked myogenic potentials in middle ear effusion.

CONCLUSION: The vestibular evoked myogenic potential (VEMP) responses were significantly affected by middle ear effusion (MEE). The VEMP was either non-responsive or significantly delayed. However, after the conductive hearing loss (CHL) was significantly reduced with tympanic aspiration, the VEMP latencies and asymmetry ratio returned to the range of healthy controls. In addition, the recovery of VEMP response was prompt and immediate. OBJECTIVE: To evaluate the VEMP in various degrees of CHL resulting from MEE and to clarify the responses after the CHL was reduced. MATERIALS AND METHODS: V

Evaluation of the dizzy patient.

The differential diagnosis of dizziness is difficult, yet challenging. There continues to be a need for improvement in assessment and evaluative procedures for patients with symptoms of dizziness. The otologist must exclude a wide variety of potential disorders which could be sources of the symptoms. The key to a well-made differential diagnosis is a very carefully taken, thorough, medical history, and the integration of that history with the physical examination, the audiometry, and the vestibular evaluation.

[Multivariate statistical analysis in stabilometry in human upright standing (second report)--pattern recognition of a stabilogram].

A pattern of a figure which is displayed by an X-Y recorder or by a polygraph in stabilometry may present several qualitative facts about the standing ability of a test-subject. The purpose of this study was to make a quantitative evaluation of the body stability in both normal and ataxic patients who were standing erect. The technique utilised was pattern-recognition, composed of principal component analysis and discriminant analysis. Stabilometrical examinations were carried out in 20 normal subjects, 39 with peripheral vestibular disturbance and 11 with disturbance of the central origin. Ea

Vestibular dysfunction in vitamin D receptor mutant mice.

The vitamin D endocrine system is essential for calcium and bone homeostasis. Vitamin D deficits are associated with muscle weakness and osteoporosis, whereas vitamin D supplementation may improve muscle function, body sway and frequency of falls, growth and mineral homeostasis of bones. The loss of muscle strength and mass, as well as deficits in bone formation, lead to poor balance. Poor balance is one of the main causes of falls, and may lead to dangerous injuries. Here we examine balance functions in vitamin D receptor deficient (VDR-/-) mice, an animal model of vitamin D-dependent rickets

Disentangling balance impairments in spinal and bulbar muscular atrophy.

Spinal and bulbar muscular atrophy (Kennedy's disease) has been associated with balance dysfunction and falls. However, postural control has not been studied quantitatively. Here, we quantified upright stance and aimed to disentangle the role of vestibular, proprioceptive and oculomotor deficits. Static balance was assessed in Kennedy patients (n = 7) during quiet stance on a force platform under different visual and proprioceptive feedback conditions. Vestibular function was assessed with the video head impulse test. Sural nerve neurography was employed to evaluate the severity of peripheral

Cerebellar ataxia, neuropathy, vestibular areflexia syndrome (CANVAS) with chronic cough and preserved muscle stretch reflexes: evidence for selective sparing of afferent Ia fibres.

The aim of this study was to describe five patients with cerebellar ataxia, neuropathy and vestibular areflexia syndrome (CANVAS) with chronic cough and preserved limb muscle stretch reflexes. All five patients were in the seventh decade of age, their gait imbalance having been initiated in the fifth decade. In four patients cough antedated gait imbalance between 15 and 29 years; cough was spasmodic and triggered by variable factors. Established clinical picture included severe hypopallesthesia predominating in the lower limbs with postural imbalance, and variable degree of cerebellar axial an

Speech-in-noise perception in unilateral hearing loss: Relation to pure-tone thresholds and brainstem plasticity.

We investigated speech recognition in noise in subjects with mild to profound levels of unilateral hearing loss. Thirty-five adults were evaluated using an adaptive signal-to-noise ratio (SNR50) sentence recognition threshold test in three spatial configurations. The results revealed a significant correlation between pure-tone average audiometric thresholds in the poorer ear and SNR thresholds in the two conditions where speech and noise were spatially separated: dichotic - with speech presented to the poorer ear and reverse dichotic - with speech presented to the better ear. This first result

Antisense oligonucleotide therapy rescues disruptions in organization of exploratory movements associated with Usher syndrome type 1C in mice.

Usher syndrome, Type 1C (USH1C) is an autosomal recessive inherited disorder in which a mutation in the gene encoding harmonin is associated with multi-sensory deficits (i.e., auditory, vestibular, and visual). USH1C (Usher) mice, engineered with a human USH1C mutation, exhibit these multi-sensory deficits by circling behavior and lack of response to sound. Administration of an antisense oligonucleotide (ASO) therapeutic that corrects expression of the mutated USH1C gene, has been shown to increase harmonin levels, reduce circling behavior, and improve vestibular and auditory function. The cur

The contribution of otoneurological evaluation to tinnitus diagnosis.

Tinnitus has been reported for nearly 80% of patients referred to the otolaryngology services. Usually, its evaluation is based on tonal and vocal audiometry, tympanometry, brainstem-evoked potentials, electrocochleography, and otoacoustic emissions. However, as the cochleovestibular system works as a unit, the use of vestibular tests has been proposed to evaluate tinnitus. Many patients with tinnitus have altered vestibular test results even in the absence of vestibular symptoms. This finding accounts for the indication of complete vestibular and audiological evaluation of tinnitus. The aim o

Unexpected complication of posterior canal occlusion surgery for benign paroxysmal positional vertigo.

OBJECTIVE: The purpose of this report was to illustrate how an unusual complication of posterior canal occlusion surgery for benign paroxysmal positional vertigo (BPPV) may be recognized and prevented. Recurrence of BPPV after occlusion surgery of the posterior semicircular canal has not previously been reported in the literature, to the authors' knowledge. Failure of occlusion not only permits the continued symptoms of BPPV but also burdens the patient with the additional morbidity of a fistula of the PSCC. The authors describe the successful treatment of a patient with recurrent and incapaci
